An obese man, the patient is complaining of continued intertrigo candida below the overhang of his abdomen. At his last visit 2 weeks ago, Michelle prescribed nystatin powder to be applied at bedtime and in the morning to prevent further episodes, but the patient hasn't had long-lasting relief. What other options should Michelle consider prescribing for this patient? A. A. Michelle is dealing with one of the most common themes in dermatology: we rarely cure patients. Intertrigo candida is a great example of why we can't offer a simple cure for this and similar conditions. So why do we even need the specialty? My answer is that as with most specialties, dermatology is pattern recognition; you see something over and over again, and you become acquainted with it so that you can anticipate it. The advice I would give Michelle is to look at the pattern presented with this patient. She should ask her patient questions about the timing of his breakouts, the OTC remedies he's tried, and the soaps and lotions he uses. Michelle should also understand that intertrigo is a separate condition from candidiasis. When the skin occludes and rubs on itself, moisture collects causing maceration. A number of treatments have become available which treat these relatively common conditions by attacking the causative agent. These include lithium salts, which inhibit small colonies of P. ovale , and ketoconazole and other imidazoles which interfere with steroid synthesis in yeasts. This is a British multicentre double-blind, randomised, placebo-controlled study of an ointment containing 8% lithium succinate (Efalith) compared with base ointment. The study [1] was designed to be cross-over, but is also analysed as a parallel design (158 patients) which is somewhat easier to understand. Patients were instructed to apply the ointment sparingly to affected areas twice daily for four weeks. Using 100mm visual analogue scales (VAS), patients assessed redness, scaling, greasiness, itching and the overall impression of treatment. All were significantly better with lithium succinate, with redness, scaling and itching being most improved. There was evidence that the effects of treatment extended at least four weeks after stopping.

Extractions: Acne rosacea is a misleading term: rosacea and acne are two entirely different conditions, although they can and do appear together. Clogged skin pores and bacterial infections cause acne. Rosacea occurs when blood vessels move to just below the skin's surface, leading to blotchy red patches. These blotches fade and then return, becoming more permanent over time. The blackheads, whiteheads and pimples associated with acne are not associated with rosacea. Red bumps called papules are. These papules are usually solid and hard. The papules range in size from small bumps that resemble the measles or chicken pox, all the way to larger, penny-shaped nodules. Left untreated, larger nodules can cause rhinophyma (see below). About half of all sufferers have symptoms of ocular rosacea , where symptoms affect the eyes. The eyelids may become inflamed and develop small red bumps, or develop scales and crusts after a night's sleep. The eyes may become bloodshot with a "gritty" feeling, as if a piece of sand were in the eye. Sensitivity to bright light is common. Occasionally, eyelash loss occurs.
