Extractions: Selective Mutism Organization Selective Mutism is a term used for children who will talk at home but not talk in school or other social situations. It is unknown why this happens, but typically, Selective Mutism follows some sort of traumatic experience. Although children with this disorder have normal language skills, they refuse to use those skills in the presence of strangers. So deeply rooted is this phenomenon, that I once worked with a young girl with Selective Mutism that, when in pain, would actually open her mouth as if wailing while tears poured down from her eyes without making a sound. A rare disorder, Selective Mutism is found in less than 1% of the population and typically occurs before the age of five. The disorder may subside in a few weeks or months, or may persist for years, depending on the child.

Children with SM persistently lack the ability to speak in some settings (for example, school) but not in others (for example, home). In most children, SM is a passing phase and the ability to communicate in all settings is regained by adulthood. SM may, however, continue throughout adolescence or prevent the young person from acquiring age-appropriate social skills. SM may affect a child's ability to talk with specific members of the immediate family at home as well as teachers or peers at school or in other social situations. Children respond or make their needs known by nodding their heads, pointing or by remaining expressionless or motionless until someone correctly guesses what they want. SM may affect a child's educational performance but progress at school is not impeded in those situations where speaking is not required. SM is often not recognised because a child's behaviour is attributed to shyness or embarrassment. In the past, there was a tendency to perceive SM children as stubborn and oppositional. More recent research, however, indicates that in most cases SM is the result of crippling anxiety. Stressful early experiences, a clash of cultures between home and school, language difficulties, and/or unaccustomed expectations at school, may predispose a child to SM.

Selective mutism primarily affects children. What is going on in the body? The majority of people who exhibit selective mutism appear to have some type of anxiety disorder . A person who has this condition usually has a full understanding of language in most situations. In other cases, though, they appear as if they have a total lack of language where one would expect oral speech, such as in school. This condition may extend over a period of time, at least 1 to 2 months and longer. This occurs in the absence of any specific medical problem that may prevent speech. What are the signs and symptoms of the condition? Symptoms of selective mutism include: refusing to talk in one or more social situations, including school a speech disturbance that interferes with achievement in work or school when the condition lasts at least 1 month or more Selective mutism does NOT appear to be related to: lack of familiarity with the spoken language degree of comfort with speaking in social situation communication disorders, such as stuttering or aphasia

A child might have a physical handicap that is responsible for trouble hearing, processing, or producing language. It could also be true that the physical parts are working properly, but there are some mental processes that don't work quite right. Another possibilty is that the child is fully capable of doing what you expect, but he hasn't been taught well enough to do it yet. It could also be true that the child is not hearing or speaking properly on purpose. It is also a possibility that an emotional problem like anxiety or depression is interfering with his ability to use language. Therefore, at Down To Earth, we do a whole lot of assessment before we do any treatment. We try to pick one theory at a time and either rule it out or find some evidence that we're on the right track. Imagine how much time would be wasted trying to convince a kid to speak better if he simply cannot because something in his mouth isn't shaped right.
