Extractions: Information and approaches to help children communicate The term communication (speech) disorders encompasses a wide variety of problems in language, speech, and hearing. Speech and language impairments include articulation problems, voice disorders, fluency problems (such as stuttering), aphasia (difficulty using words, usually as a result of a brain injury), and delays in speech and/or language development. Speech and language delays may be due to many factors, including environmental factors or hearing loss. Hearing impairments include partial hearing and deafness. Deafness may be defined as a loss sufficient to make auditory communication difficult or impossible without amplification. There are four types of hearing loss. Conductive hearing losses result from diseases or obstructions in the outer or middle ear and are usually helped with a hearing aid. Sensorineural losses arise from damage to the sensory hair cells of the inner ear or the nerves that supply it and may not respond to a hearing aid. Mixed hearing losses are those in which the problem occurs both in the outer or middle ear and in the inner ear. A central hearing loss results from damage to the nerves or brain. Many communication disorders result from other conditions such as learning disabilities, cerebral palsy, mental retardation, a cleft lip or cleft palate.

Stuttering affects four times as many males as females. People who stutter are as intelligent and well-adjusted as non-stutterers. There are no instant miracle cures for stuttering. In fact, it is a resonance disorder! Voice disorders are caused by dysfunction of the larynx. Hypernasality is caused by a dysfunction of the velopharyngeal mechanism. Medical management for voice disorders is by otolaryngologists (ear, nose and throat specialists) and surgical management for hypernasality is by a plastic surgeon or craniofacial surgeon who participates on a cleft palate-craniofacial team. Velopharyngeal incompetence or VPI is the inability of the velum (soft palate) and related musculature to close the nasopharynx, separating the oral and nasal cavities for the production of oral consonants. A VPI can be caused by a deficiency of the velum or an increased size of the pharynx. The latter is difficult to diagnose without imaging.
