    Extractions: Speech is different from voice. This is an important but often missed distinction. Voice is the vibrating sound made by blowing air through your vocal folds. The sound is not being modified on an ongoing basis by the upper airway tract. If the vocal folds could be separated from your throat, tongue, nasal passages etc. we would only hear an unadulterated buzz. However, the upper airway cavities add resonance and, thus, character to the sound. As long as the sound is not being actively manipulated by the upper airway, an abnormality of this vibrating sound would be a voice disorder. Speech is any buzzing sound that is continuously being modified. Thus an electrolarynx or a pair of vocal folds can be functioning quite well, but if the tongue or palate do not move appropriately, the perceived disorder is one of speech and not voice. If one accepts this distinction, then testing for a voice disorder by listening to running or conversational speech adds a significant amount of noise and haziness to the diagnostic process and makes discerning which type of vocal disorder is present, extremely difficult. Listening to pure tones under different settings of pressure, loudness and pitch should be adequate to diagnose or characterize a vocal disorder. The abnormality or disease process will be traced to the larynx, vocal folds themselves, or the manipulation of the vocal folds. A speech disorder will not become apparent until word manipulations take place. A disorder of speech will be an incorrect production of vowels and consonants. The cause of a speech disorder will be found in the tongue, lips, palate, or nose - or the central nervous system manipulation of these structures. It will not be found in the voice box or larynx.

Mr. Segalman, who works for the California Department of Rehabilitation, has cerebral palsy that affects his speech. About 13 years ago he came up with a solution- Speech to Speech. At that time, he chaired the California Relay Service Advisory Committee, which required him to be in contact with many deaf people using the TTY relay system. ''When I called the TTY relay using the voice line, I discovered that most of the operators understood me. People with excellent hearing have a good chance of understanding many speech disabilities,'' said Mr. Segalman, who holds a doctorate and is a professor. ''That gave me the idea that a service which 'revoiced' the utterances of people with speech disabilities would enable them to use the telephone,'' he said. In the mid-1990s, the professor's idea became a reality when he introduced a telephone relay service in California to assist those with speech disorders.

    Extractions: Paula A. Square Dr. Square’s research focuses on motor speech disorders, particularly apraxia of speech, and on the interface between language formulation and motor speech planning. Studies of both adults and children are on-going. Two pathological populations with verbal production disorders are studied: children with moderate to severe developmental speech disorders and verbal expressive language disorders and adults with acquired aphasia and co-existing motor speech disorders. The nature(s) of the verbal production disorder, linguistic and motoric, and the interaction of these two domains are of primary concern. Studies of the assessment and treatment of the disorders is a second line of research. With her colleague, Deborah Hayden, a hierarchical model of disordered speech motor control has been developed. The model allows for the characterization of aberrant motor control at one or more of three levels: physiological support for speech, focal oro-motor control and speech movement sequencing. The existence and nature of motor speech control deficits at these distinct levels has been studied in both populations and the theoretical model has been validated using behavioural data. Validation studies using kinematic data are planned. The aim of this line of work is to improve the efficiency and effectiveness of clinical interventions. A third line of research is in collaboration with Drs. Eric Roy and Sandra Black and focuses on the conceptual basis of limb praxis. This work is being undertaken in the Neurocognitive Laboratories at Sunnybrook Hospital and is funded by the Ontario Mental Health.
