Extractions: Spina Bifida is a fault in the spinal column in which one or more vertebrae (the bones which form the backbone) fail to form properly, leaving a gap or split, causing damage to the central nervous system. To help understand what it is, let us explain about the composition of the nervous system. The central nervous system consists of the brain and the spinal cord. All activities are controlled by the brain which receives information from touching, seeing, feeling, tasting and hearing - responding to this information by initiating the appropriate movements of different parts of the body. Messages from the brain are carried to different parts of the body by the spinal cord which runs down the centre of the spinal column. This communication system for the body is very important and needs protection The spine is made up of 33 bones or vertebrae. The vertebrae have two main functions. One is to provide anchorage for muscles so that we can move as we dictate to those muscles. The other is to provide protection to the spinal cord. The central nervous system and spine develop between the 14th and 28th day after conception. Spina bifida occurs when there is a failure of development of the boney canal which surrounds the brain and spinal cord. In the spine, the affected vertabrae have a defect posteriorly (at the back) so that a boney ring does not completely surround the spinal cord. This leaves a gap so that, instead of the posterior arm being whole it is divided - that is bifid. The fault may occur in one or more of the vertabrae but it is most common around waist-level.

Extractions: SPINA BIFIDA - A NEURAL TUBE DEFECT Spina Bifida is a Latin term meaning split spine. It is the name given to a group of birth defects which interfere with the development of the central nervous system: the brain, the spinal cord and the nerve tissues. HOW IT STARTS The central nervous system begins to develop from the ectoderm (the layer of cells from which the brain and spinal cord develop) in the third week following fertilisation when the embryo is only 3-5mm long. Prior to this the ectoderm resembles a flat group of cells running down the middle of the embryo. This flat sheet begins to change, however, and folds to form a groove. (Diagrams (a) and (b).) The edges of the sheet eventually come together to form a tube which later develops into the spinal cord and the brain. (Diagrams (c) and (d)) This structure is called the neural tube . Once the neural tube closes it sinks into the embryo and is covered by a layer of skin. The spinal vertebrae (bony covering) then begin to form around the tube. Normally the neural tube closes by the twenty-eighth day after conception. However, if the tube fails to close properly, a neural tube defect will occur. Neural tube defects include the conditions of anencephaly, encephalocele, and spina bifida.

The NTD occurs when the vertebrae and/or spinal cord of the fetus fail to develop properly, and results in varying degrees of damage to the spinal cord and nervous system. The damage is permanent. SB is usually accompanied by hydrocephalus . Treatment is urgent to minimize further neurological damage in the opening in the spine. The three most common types of SB are: Myelomeningocele - the severest form in which the spinal cord and its protective covering, the meninges , protrude from the opening in the spine. Meningocele - the spinal cord develops normally, but the meninges protrudes from the opening created by damaged or missing vertebrae and may be exposed. Occulta - one or more vertebrae are malformed, and covered by a layer of skin.

Extractions: The diagnosis can of ten be made on the basis of a high resolution ultrasound or elevation of maternal alpha fetal protein levels. Other times it is not recognized until birth. It is widely accepted that most children born with spina bifida should be aggressively treated both medically and surgically. Recommendations are for early closure of the back and spinal cord as well as management of hydrocephalus if indicated. The important thing to realize is that this operation is designed specifically to prevent infection or ongoin g trauma to the exposed spinal cord and not to return function which is not present at birth. This is not an insignificant distinction. Hydrocephalus is usually present in @80-90% of children with spina bifida and should b e treated when determined, usually before discharge from the hospital. As many different parts of the body and nervous system are potentially involved, the child will need to see numerous physicians and therapists for follow up. In addition to physical therapy and early intervention, the child may need treatment for incontinence as well as bracing for the legs or spine. In many areas of the country this follow up is provided through multidisciplinary spina bifida clinics. These clinics are staff ed by the various physicians and therapists whose specialties pertain to the treatment of spina bifida.
