The area is bordered by the collar bone, the first rib, and the anterior and middle scalene muscles. Thoracic outlet syndrome occurs when the nerves and blood vessels between the neck and shoulder are compressed. Extractions: A Patient's Guide to Cumulative Trauma Disorders Thoracic Outlet Syndrome (TOS) is a condition affecting the shoulder, arm, and hand. This condition is a very frustrating problem - both for the patient and for the physician. It is extremely difficult to prove that the diagnosis of Thoracic Outlet Syndrome is correct, because there is no test that has a high degree of accuracy in showing the problem. Usually, the diagnosis is made after all other causes of the symptoms have been ruled out - a frustrating and slow process sometimes! The nerves and blood vessels that run into the arm and hand start at the side of the neck. They exit from the side of the spine through small openings between each vertebra called foramen . As they leave the spine the nerves are referred to as nerve roots . The individual spinal nerve roots then begin to join together to form the nerves that will run into the arm and hand. The nerves travel between two muscles in the neck (the scalene muscles), over the top of the rib cage, under the collar bone (clavicle), through the arm pit (axilla) and down the arm to the hand. The area where the nerves and vessels leave the neck between the two scalene muscles and over the first rib is know as the Thoracic Outlet. There are probably several causes of TOS. Extractions: Role of Acupuncture in Thoracic Outlet Syndrome Dr. M Tariq, Dr. Jawad Zahir and Dr. Salman Ahmad Saleem Abstract This report describes a case of Thoracic Outlet Syndrome (TOS) which was successfully treated with acupuncture after failed conservative management with a variety of interventions. Webmaster's Note: This is a brief summary of this case report, the full text of which is available in the paper and electronic versions of the Journal. A 21 year old woman had suffered with pain in the neck, scapular region and chest, with brachialgia and with paraesthesia of the left hand, for three and a half years. She had been treated without any lasting relief with a variety of modalities, including NSAIDs, opiods, physiotherapy, short wave diathermy, ultrasound and nerve block therapy. She was treated daily with electroacupuncture to points ST38, LI4, LI11, LI15, and TE14, for 10 days, with some improvement. She was then given three further week-long courses, at fortnightly intervals.

The symptoms are: Usually, the connective tissue has shortened, thickened around a bundle of nerves (bracio-plexis) as a result of repetitive movement. The conventional treatment is surgery on the connective tissue that is pinching or entrapping a nerve. Usually, it is surgically cut. Rolfing can stretch the connective tissue and free the nerve that is pinched. The result is no more pain.

During the last four years, 555 upper extremities of 494 patients were diagnosed as having thoracic outlet syndrome. Forty-five patients (9.3%) were diagnosed as having other entrapment neuropathy in one extremity. Ten cases were complicated by cervical radiculopathy, 15 by carpal tunnel syndrome, 11 by cubital tunnel syndrome, five by radial tunnel syndrome, two by ulnar tunnel syndrome, two by both carpal tunnel and cubital tunnel syndrome, and one by both cubital and ulnar tunnel syndrome.
