Extractions: A clinical trial currently recruiting participants. The purpose of this study is to determine accurate and current data regarding the incidence of disability and death in patients with Thromboangiitis Obliterans or TAO (Buerger's Disease) and the relationship of tobacco exposure to the development of TAO. TAO is a rare disease characterized by inflammation (swelling, redness or pain) of the small and medium peripheral arteries (blood vessels in the extremities). This inflammation frequently leads to blockage of arteries and symptoms of foot and hand pain and non-healing ulcers of the toes and fingers. TAO is found only in individuals with a history of tobacco exposure. Enrollment Goal = 35 Enrollment to Date = 12 Age: 18 to 50 years 15 participants with TAO 10 participants who use tobacco and have no evidence of arterial disease 10 healthy participants More detailed eligibility criteria...
Extractions: Age 18 to 50 years (less than 50 years at time of diagnosis) Current or past tobacco use in the form of cigarette, pipe, cigar or smokeless tobacco (chewing tobacco) Objective evidence for medium or small arterial occlusive disease: angiography (contrast or magnetic resonance) or noninvasive arterial studies (continuous wave doppler, segmental arterial pressures, and pulse volume recordings) demonstrating segmental occlusion of one or more peripheral medium or small arteries. For this study these vessels are limited to the radial, ulnar, palmar arch, digital (upper or lower), peroneal, anterior tibial, and/or posterior tibial arteries. Rarely, an arterial biopsy may provide or establish the diagnosis of TAO. No evidence for a source of emboli or other cause of arterial occlusive disease such as Takayasu's arteritis or collagen vascular disease associated vasculitis

Extremities involved with inflammatory occlusions of the more distal arteries, resulting in circulatory insufficiency of the toes or fingers. Thromboses of superficial veins may also occur- Course is intermittent and amputation may be necessary, especially if smoking is not stopped. Buerger's disease is an episodic and segmental inflammatory and thrombotic process of the arteries and veins, principally in the limbs. The cause is not known. It is seen most commonly in men between ages 25 and 40 who smoke. The effects of the disease arc almost solely due to occlusion of the arteries. The symptoms are primarily due to ischemia, complicated in the later stages by infection and tissue necrosis. The inflammatory process is intermittent. with quiescent periods lasting weeks, months, or years. The arteries most commonly affected are the plantar and digital vessels in the foot and those in the lower leg. The arteries in the hands and wrists may also become involved. Different arterial segments may become occluded in successive episodes: a certain amount of recanalization occurs during quiescent periods. Superficial migratory thrombophlebitis is a common early indication of the disease.
