    Extractions: Trichollomania Trichollomania is characterized by the repetitive pulling on one's hair resulting in noticeable hair loss. Patients typically experience an increasing sense on tension immediately before pulling out the hair or when attempting to resist the behavior. Pleasure, gratification or relief is felt upon hair pulling. Researchers at our Institute hypothesize that two forms of trichotillomania may exist. Individuals who experience tension before pulling and gratification upon pulling may have a form of trichotillomania thought to be a type of obsessive compulsive spectrum disorder. For others who experience no awareness or pleasure, hair pulling may be a habit. Further research will help clarify these issues. The "obsessive-compulsive spectrum" refers to a series of major psychiatric conditions defined by the presence of obsessions and compulsions. Obsessions are intrusive ideas, thoughts, or images that cause much anxiety and distress. Compulsions are repetitive behaviors or mental acts performed to reduce the anxiety produced by obsessions. You may have one without the other. The current psychiatric diagnostic manual (DSM-IV) provides the following definitions: An increasing sense of tension immediately before pulling out the hair or when attempting to resist the behavior.

The sites of hair pulling include scalp, eyelashes, eyebrows, axillary, body, or pubic. Hair pulling tends to occur in episodes, exacerbated by stress, or conversely sometimes worsened by relaxation (e.g., when reading a book or watching television). The person generally feels tension before pulling hair, which is relieved by the pulling. All other causes of hair loss, including medical and dermatological must be considered and ruled out before the diagnosis of Trichotillomania is confirmed. Associated Features and Disorders Associated features of Trichotillomania include: examining the hair root; twirling it off; pulling the strand between the teeth, or trichophagia (eating hairs). Nail biting, scratching, gnawing, and excoriation may be associated with Trichotillomania. Persons with Trichotillomania may also have Mood Disorders (depression), Anxiety Disorders, or Mental Retardation. There can be histological findings. Biopsy samples may reveal short and broken hairs. Histological examination will reveal normal and damaged follicles. There should be inflammation in the affected area.

Trichotillomania Trichotillomania affects 2-3% of all people with hair loss making it a fairly common condition. Studies have identified the scalp as being the most commonly affected area. Approximately 70% of cases involve scalp hair loss, 50% involve eyebrows and or eyelashes, 30% include pubic hair, 20% body hair, and about 10% involve facial hair. As well as plucking the hair, affected individuals may then chew or eat the hair. Hair eating is known as trichophagia while hair chewing may result in hair balls (trichobezoars). About 40% of cases involve hair chewing while 10% of affected individuals eat their hair. Eating hair is rather unwise as it is very irritating to the stomach and may lead to digestion problems and ulcers. The affected individual is most often unaware of what they are doing and arrive at the dermatology clinic complaining of hair loss from an unknown cause. Dermatologists have been known to resort to making a video of the individual to prove to them that they are unconsciously plucking their hair. Trichotillomania can affect both children and adults although the mean age of first onset is 13 years. Both boys and girls are affected in childhood but the adult version of trichotillomania almost always affects women (Jaspers 1996).
