Extractions: I was diagnosed as having ulcerative colitis when I was 21 years old, due to a couple of episodes of bloody diarrhea. The second episode, I had to stop taking the medication because of side effects. Instead, I eliminated dairy products from my diet and took aloe vera juice, acidophilus, etc, and the symptoms went away. I am now 45, and haven't had a recurrence for almost 20 years. My question is: what would normally be recommended for follow-up monitoring in this case? I've had occasional flexible sigmoidoscopies, and a colonoscopy about 10 years ago... all of which showed no problems. Dear bublynski

On this page you will find out about my experience with untreatable ulcerative colitis and my decision to have an ileoanal pull through. As my picture shows, I was in very good physical condition as an Army Artillery Officer. When I became sick with UC, I lost 20 pounds because I was having 10-12 bouts of diarrhea a day. History Six months after the initial surgery, I am now able to do everything I could do prior to become sick from Ulcerative colitis. On the other pages of have my hospital experience, some colonoscopy pictures and my post hospital experience. The following pages include some graphic pictures of my abdominal incision after debriding because of an infection. My purpose is to show what happened to me so that a situation such as this will not be as shocking if it occurs to someone else. Before all the surgery took place, I thought I would go in have the colon removed, rest and 6 weeks later have the reduction completed. So much for fantasy, it took 4 months from the initial surgery until reduction. The presentation of Ulcerative Colitis is that of persistent and recurrent bloody diarrhea, with urgency and cramping abdominal pain. Other symptoms of fever, weight loss, anorexia, and vomiting also may occur. The course of the disease varies greatly. The evaluation and treatment is similar to both Crohn's disease and the nonspecific inflammatory bowel disease. In summary, ulcerative colitis is a significant disease of otherwise healthy young adults. The basic cause seems to be an immune dysregulation with the local production of damaging free radicals stimulated by an environment high in iron which makes these reaction more severe and frequent. This leads to damage to the lining of the colon followed by bleeding and the production of more iron to "fuel the fire" since the colonic mucosa is not filled with antioxidant enzyme systems like other tissues it is vulnerable to oxidant molecular injury.

Extractions: In February of 1997 a chapter of my life started that would forever change the way I viewed my world and the health so many of us in life take for granted..I passed some blood while using the bathroom one day and as you would expect It scared me a little but I dismissed it as something bad I ate or some rawness down there..you know..nothing major..forget about it right We'll it started appearing more and more frequently however I never really felt bad other than the blood making me a little weak.. I passed it off as nothing and figured it was just a normal part of my movements now..Life goes on and ready for the next day...We'll I finally mentioned it to my wife and well she did not think it was so normal and urged me to make an appt with our family doc..so I did with much resistance..C'mon you know how us guys are with doctors and hospitals..Were invincible!! I went and saw our family P.A. who dismissed it as probably a innocent Roid and recomended I see a Colorectal Doctor to confirm this..Well after about another month of the symptoms now combined with some kind of mucus I finally did make that appt with a colorectal doc..Were up to April 97 now and still bleeding and also starting to take a toll on my health!! The Colon Rectal doc told me it was roids and performed three small banding procedures that hurt like hell to remove these supposed hemmies!! To this day I doubt he ever did do anything significant because guess what The bleeding and mucus got worse and now more symptoms began such as Severe cramping and Bloody Diareha. I started going 10,15,20 times a day and it was all blood..I got so weak that I could barely lift my head some days let alone get out of bed!!

I do not have an inflammatory bowel disease, but have collected some sites that might be helpful for those who are looking for information. In my opinion the doctors will let you go on as long as you don't complain too much. Before World War II, people who had Ulcerative Colitis (UC) badly enough died, probably of peritonitis when their colon perforated. The problems were inadequate surgical technique in joining the ileum to the stomach wall and primitive appliances failing to adequately seal the collecting device to the body. Surgeons refused to operate because the exteriorised bowel became inflamed, contracted and eventually obstructed. Collecting devices were useless and most people were allowed to perforate. The few who were operated on lived solitary lives (because of the odour) while many died, unable to cope with the complications in the area of the stoma. After the war, a new surgical technique was developed by Professor Bryan Brookes MD MChir FRCS Hon FRACS — an everted stoma (spout). By turning the ileum inside out, the inner mucosa surface was then the surface in contact with the effluent, and this method was so successful it was eventually used worldwide and is referred to as the "Brooke Eversion Technique". This new technique and rapid advances in materials technology, invariably saved peoples lives and undoubtedly gave sufferers an opportunity of a new life from the 1960s. For my Brooke ileostomy and the "simple" bag that I use are marvels of modern technology and they are fundamental to my quality of life. I will describe how I apply this technology in the management of my ileostomy and thus sustain a normal life. I am fortunate that my illness was totally cured by the removal of the colon; others are plagued by complications of Crohns, adhesions, fistulas, prolapse, hernia and other problems but I regret that my techniques will not be sufficient for them.

Medical Information Ulcerative Colitis and Marijuana Annals of Internal Medicine, Volume 112, Number 6, 15 March, 1990, p. 471 To the Editor: The apparent protective effect of cigarette smoking on ulcerative colitis (1) suggests that inhaled plant products might affect the disease process. We report a case suggesting a relation between ulcerative colitis and smoking marijuana. In 1972, a 23-year-old woman developed abdominal pain, diarrhea, and rectal bleeding. Sigmoidoscopy to 15 cm revealed diffuse cobblestone ulcerations, and a single-contrast barium enema showed numerous small marginal irregularities in the distal sigmoid and rectum. Ulcerative colitis was diagnosed, and her symptoms waxed and waned despite treatment with sulfasalazine, librax, and intermittent prednisone. Discouraged, she stopped all medications in 1975. In 1976, she noted that smoking marijuana resulted in fewer stools, more stable body weight, and fewer, milder exacerbations . Her typical intake was a pipeful once or twice daily, which she maintained despite her dislike for its euphoric effects. Cessation repeatedly resulted in exacerbations within a few weeks; resumption led to some improvement in a day or two, with maximal effect after several weeks. She was smoking marijuana daily when, after having 2 months of symptoms, she was hospitalized at the Mary Hitchcock Memorial Hospital in 1983. She was febrile and had an erythrocyte sedimentation rate of 103 mm/h. Sigmoidoscopy to 16 cm showed diffuse small friable ulcerations with granularity. A biopsy of the lower rectal fold showed marked chronic inflammation extending into the submucosa without granulomas. She improved promptly with prednisone. At home she resumed taking marijuana. Over 15 months her prednisone dose was tapered and sulfasalazine therapy begun. Sigmoidoscopy to 20 cm was normal, and a single-contrast barium enema showed a diffuse granular pattern with a "pipe-like" apearance, compatible with inactive colitis.
