Extractions: MassageToday.com On a recent visit to the National Library of Medicine, I came across an article published in a refereed Russian journal.1 This article was devoted to the hot topic of chiropractic, and I'm happy to provide an abstract of the published results for the readers of Dynamic Chiropractic. It is accepted in manual medicine that certain cases of cardiovascular and cerebrovascular diseases are concurrent with vertebrogenious disorders that can cloud the clinical picture. In this article, special indications for manual therapy in patients exhibiting the stages of cerebral vascular disorders, such as disturbances of neuromuscular functions in the cervical and thoracic locomotor apparatus, were elaborated. Dr. Ezhov studied 100 patients (61 men and 39 women) in the initial stages of cerebral vascular disorders. They ranged in age from 35 to 55, and presented with complaints of headache, dizziness, tingling, fatigue, memory impairment, and sleep disturbances. All patients had neurological complications of dystrophic and degenerative disease based in the cervical and/or thoracic regions. The patients were evaluated using psychological and psychiatric tests, ECG, EEG, reoencephalography (indirect evaluation of cerebral blood circulation using cutaneous electrodes), and blood biochemistry. X-ray findings showed marginal sclerosis of the lamina of vertebral bodies with diminished cervical lordosis and thoracic kyphosis, decreased disc height, and marginal osteophites. The majority of patients presented with posture and gait changes; subluxations were found in C0-C2 (86 percent), C5-C6 (88 percent), C7-T1 (96 percent), T10-L2 (63 percent), and L4-S1 (82 percent), along with multiple trigger points in the temporal, occipital, trapezius, scalenus, and sternoclaidomastoideus muscles.

Spinal Cord Injury Vascular Disorders The vascular supply of the spinal cord originates superiorly from the vertebral arteries and, in the low cervical and thoracic area, from branches of the aorta, often from a particularly large and important low thoracic branch. Branches of the anterior spinal artery supply the anterior 2/3 of the cord, and branches of the posterior spinal artery supply the posterior 1/3. INFARCTION Because most of spinal circulation is supplied by two or three major arterial branches, segments of the cord in the watershed area between the branchesaround the 2nd to 4th thoracic segmentsare especially vulnerable to ischemia. Vascular thrombosis is uncommon, however, and vascular compression (eg, from tumors, acute disk compression) or occlusion from remote causes (eg, aortic surgery, dissecting aneurysm) more commonly causes infarction than does intrinsic disease of spinal arteries. Rarely, polyarteritis nodosa causes occlusion of spinal arteries. Sudden pain in the back and in the distribution of the affected segment is followed by bilateral flaccid weakness and a dissociated sensory loss with impaired pinprick and temperature sensation below the level of the infarct. The distribution of the anterior spinal artery is usually affected, so that touch, proprioception, and vibration sense are likely to be spared because they are conducted in posterior tracts of the cord. As with all infarcts, the deficit is most marked during the first few days and may partially resolve with time.

Extractions: THE ANTICARDIOLIPIN SYNDROME Some lupus patients have an antibody to a substance called cardiolipin, which causes a false-positive result in the blood test for syphilis. Consequently, lupus sufferers may be repeatedly treated with penicillin in the erroneous belief that they have a venereal disease. (In some cases, a false-positive test for syphilis may be the first manifestation of lupus.) It has been shown that the anticardiolipin antibody can be present for unknown reasons, as well as in association with other connective tissue diseases, and need not be restricted only to lupus patients. The presence of anticardiolipin antibody is associated with a tendency to increased blood clot formation. This tendency may cause strokes or heart attacks in young people, recurrent phlebitis, multi-infarct dementia (a form of senile dementia due to multiple small strokes), and recurrent miscarriage. The test for anticardiolipin antibody is now routinely performed in anyone suffering these problems, and is particularly important for women who recurrently miscarry. The antibody is believed to cause miscarriage by stimulating thrombosis of the small veins in the placenta. Anticardiolipin syndrome is treated with anticoagulant drugs, generally either low-dose aspirin or the Coumadin-type of anticoagulants, depending on the severity of the clinical manifestation and the duration for which the anticoagulation would have to be maintained. Pregnant women are given lowdose aspirin or, occasionally, heparin, since Coumadin would threaten the health of the fetus.

Vascular Medicine specialists are trained in the diagnosis and treatment of vascular disease, a broad range of conditions affecting the blood vessels or circulatory system. Often thought of as "hardening of the arteries", "poor circulation" or even as varicose veins, vascular disease can be present in any part of the body where there are arteries and veins that transport blood. Vascular disorders occur when blood flow becomes impaired by structural defects, such as weakened or thickened arterial walls (i.e. aneurysms or inflammation), or by obstructions such as blood clots, fatty deposits and atherosclerosis. Approximately 15 million Americans have vascular disease, and at least half of them have no symptoms. It is important to diagnose these conditions accurately and early in the course of the disease because they can cause progressive discomfort, long-term disability, amputation and death. Vascular disease can lead to heart attack, stroke, ruptured blood vessels, blood clots, kidney failure and results in approximately 100,000 amputations each year. The cost to society of heart disease and stroke amounts to more than 150 billion dollars per year!

"Athero"-sclerosis is a disease in which fatty material is deposited on the wall of your arteries, which narrows the arteries and eventually restricts blood flow. Atherosclerosis is a common disorder of the arteries. Fat, cholesterol, and other substances accumulate in the walls of arteries and form "atheromas" or plaques. Eventually, the fatty tissue can erode the wall of the artery, diminish the elasticity (stretchiness) of the artery, and interfere with the blood flow.
