While studies have validated the role of emboli from the cervical carotid artery bifurcation as a cause of stroke, the role of a compromise of hemodynamics has remained less clear. Dr. Howard Yonas has focused much of his effort over the past 20 years to develop the technologies needed to differentiate patients at increased hemodynamic risk. Outcome studies performed by our group were some of the first to define the use of physiological studies for the definition of a group at increased stroke risk. Cerebral blood flow measurements with stable xenon/CT were pioneered at the University of Pittsburgh and this technology is now finding wide utility for the diagnosis of complex cerebrovascular disorders at centers around the world. The neurosurgical team, lead by Dr. Yonas, has evaluated and treated thousands of patients with cervical carotid disease. In order to reduce the risk of vascular evaluation, the team has pursued the use of CT angiography as a 3-D means of characterizing extra as well as intra cranial vascular anatomy. This technology provides high resolution information about plaque morphology that equals and may exceed that provided by invasive angiography and does so with minimal risk. MRI and MR angiograpy are also available as valuable tools that can help in the examination of the cerebral vasculature.

Extractions: Vascular Arteriovenous Malformations Arteriovenous malformations are a collection of of thin walled blood vessels which are massed together like a tangle of spaghetti (see the pictures). They are a literally a birth mark on the brain, they can occur anywhere on the surface of the brain. There is always a risk of these fragile vessels rupturing and causing a subarachnoid or intracerebral haemorrhage. They can also act as an irritant and produce fits. Treatment for these malformations in the past was either surgery or a wait and see policy if the lesion was in an inaccessible position. Many studies indicate that Gamma Knife surgery has a high clinical efficacy and a significantly reduced morbidity rate (production of side effects) compared with microsurgery in the treatment of the difficult lesions. The chances of complete obliteration of the lesion is around 90% for small lesions and this percentage decreases as the size of the lesion increases. It is often the treatment of choice for patients not wanting to risk conventional surgery or not able to have surgery. Gamma Knife is also very useful in treating arteriovenous malformations(AVMs) in or near critical structures, such as the brain stem. The Sheffield Centre for Stereotactic Radiosurgery has treated over 2,300 patients with these lesions since 1985.

Extractions: For a long time, venous disorders have been the poor relation in the family of surgery. When the subject of venous disease is mentioned, physicians and others think only of varicose veins. These are generally regarded as being dull and uninteresting. Yet, a recent study performed in San Diego by Professor Michael Criqui using contemporary methods of evaluation including duplex Doppler ultrasound found that there was a striking prevalence of venous disorders in a community-based population. This population was middle-aged to elderly and it was found that the most common entity occurring in more than 80% of women and half of the men were telangiectasias, commonly known as spider veins or thread veins. In addition, women had twice the prevalence of varicose veins as men. Ethnic differences were also found in this study with Caucasians having the highest prevalence and the black and Asian populations the lowest. At this time, venous disorders are managed by a variety of loosely associated physicians who can be thought of as a multidisciplinary team. Central to their work is a vascular laboratory employing ultrasound technology, which can provide diagnostic information for proper therapeutic planning. Therapeutic options have become increasingly broad. Surgical options have been vastly expanded through employment of radio frequency (RF) energy and quartz laser fibers that perform the task formerly referred to as surgical stripping. The role of injection treatments, or sclerotherapy, has become increasingly well defined, and the advent of duplex ultrasound technology has allowed this to play an important part, both in the operating room and in the clinic where sclerotherapy is practiced. Sclerosant foam is visible on the horizon and may actually replace some saphenous vein surgery.

Extractions: Air embolism, vascular disorders in plants Air embolism, a phenomenon feared by scuba-diving enthusiasts, also occurs in plants. Air bubbles can form in the vessels that transport the water taken up from the soil towards the aboveground parts of the tree, and this can have serious consequences because there is the risk that the leaves will no longer be supplied with water and will therefore dry out irreversibly. Drought and frost are the two climatic factors than can lead to embolism. For the last fifteen years or so, an INRA team has been studying the reasons behind this phenomenon as well as the ability of various tree species to resist it. The information obtained could be used to select tree varieties that can withstand embolism. In plants, pressure in the vascular system is usually low, whereas it is high in animals. Depression in plant vessels is caused by the leaves drawing up water. In fact, leaves function rather like pumps: they take water up from the soil via the tree's conducting vessels, and they release this water into the atmosphere through transpiration. The atmosphere is what drives this pump: the dryer the weather, the more the leaves are subjected to high evaporative demand. Depression can therefore be quite considerable. The vascular system is vulnerable and runs the risk of being drained out if air enters the vessels. Two phenomena are capable of causing embolism: frost and drought. During periods of drought, depression in tree vessels can reach extreme values. Under such conditions, bubbles of air can be drawn in through the vessel walls. This susceptibility to embolism varies considerably from one species to another, and the most resistant are those that are also known to be highly resistant to drought, such as cedar and holm oak for example. When depression in the vessels reaches the threshold for embolism, the plant responds by limiting its water losses: it closes up its stomata, minuscule orifices on the leaf surface, through which the carbon dioxide necessary for growth is absorbed but also through which water is released. The stomatal closure threshold corresponds to the embolism threshold: it would therefore appear that the tree will limit transpiration so as to prevent its conducting vessels from being damaged. Tree resistance to embolism in the summer is indeed an important element in understanding how they adapt to drought.
