Editorial Review

Book Description
In this exciting symposium, the editor brings to print important new information on AIDS and how HIV affects the brain. Each chapter focuses on one or more of the cell types that reside in or traffic through the central nervous system (CNS). Each of these cells is important to considerations of the pathogenesis of the CNS. Neurologists, AIDS physicians, and other professionals caring for AIDS patients will find that this “cell-based” view provides a unique perspective and that it will guide and stimulate future investigation of this clinically important and pathogenetically intriguing disorder. The editor also introduces some general considerations for therapeutic intervention of AIDS dementia complex (ADC).

The contributors to The Cellular Basis of Central Nervous System HIV-1 Infection and the AIDS Dementia Complex deal with the cells and mechanisms involved in HIV-1 brain infection and the resultant ADC. Each author was asked to review the involvement of their assigned cells in CNS HIV-1 infection and how these cells might be involved in the pathology and process of brain injury associated with ADC. Readers will be enlightened on the functional roles of various cells and how these cells and mechanisms might fit into the broader picture of ADC pathogenesis. ... Read more

Book Description

The last decade has seen tremendous gains in our understanding of the profound impact of HIV/AIDS on brain functioning. This comprehensive handbook is designed for mental health clinicians, especially those without extensive neurological training, who need to be able to recognize and address HIV-associated cognitive/motor impairment, dementia, and related disorders. The book presents the most current information on neurological and neuropsychiatric manifestations of HIV disease, promotes early identification of neurologically related mental status disturbances, and details common psychiatric features seen in these patients. Treatment issues are covered in depth, with chapters on pharmacology, psychotherapeutic approaches, and occupational therapy. Special attention is given to ethical and legal issues that may confront helping professionals, patients, and significant others.

Book Description
This volume presents in-depth reviews of the pathogenesis andclinical aspects of the AIDS dementia complex (ADC), a neurologicaldisorder directly caused by HIV-1. The opening section examines theprincipal forces involved in the viral pathogenesis of ADC, i.e., HIV-1neurotropism, immune system interactions with viruses, and cytokinecircuits in the brain. An extensive section on the neuropathology andpathogenesis of ADC includes studies on the role of macrophages andmicroglia, the cellular neuropathology of HIV encephalitis, and therole of cytokines and amino acid excitotoxicity in mediating neuronalinjury. The concluding section focuses on clinical features, evaluation, and management of HIV-related psychiatric disorders, ADC,and pediatric HIV encephalopathy. ... Read more

Description:
Learning and memory impairments are frequently observed in patients suffering from AIDS Dementia Complex (ADC). These effects have been linked to the presence of gp120, an HIV viral coat glycoprotein. The present study investigated the possibility that gp120 prevents the uptake of extracellular glutamate by astrocytes, leading to an interruption of the glutamate-glutamine cycle and a subsequent impairment of memory. Ten microliters of 10nM gp120 was bilaterally injected into the region of the intermediate medial mesopallium of day-old chicks at various times before, or after, training using a single-trial passive avoidance task. Gp120 was found to significantly impair memory retention when injected 10-40min after training. Memory impairments were evident within 5min of gp120 administration and remained evident 24h later. Further, the amnestic effect of gp120 could be overcome with glutamine or with precursors of glutamate synthesis, but only weakly by glutamate. These results support the conclusion that the amnestic effect of gp120 is due to an impaired uptake of glutamate by astrocytes and a subsequent interruption of glutamine supply to neurones. The data indicate that the glutamate-glutamine cycle may be a useful therapeutic target in the treatment of ADC. ... Read more
