From the author: Objective: Posttraumatic stress disorder (PTSD) is commonly comorbid with alcohol-use disorders. Abnormalities in the hypothalamic-pituitary-adrenal (HPA) axis response are common in both disorders. The objective of this study was to investigate HPA axis reactivity to the cold pressor task (CPT) among individuals with alcohol dependence, PTSD, and comorbid alcohol dependence and PTSD. Method: Participants were 119 individuals with alcohol dependence only (n = 31), comorbid alcohol dependence and PTSD (n = 28), PTSD without alcohol dependence (n = 30), and a control group (n = 30). Subjective response, adrenal corticotropic hormone (ACTH), and cortisol were measured before, immediately after, and for 120 minutes after each subject completed the CPT. Results: There were significant group and gender differences found in the subjective and ACTH response, with significantly higher subjective stress ratings and decreased ACTH response in the alcohol-dependent, PTSD, and comorbid alcohol dependent-PTSD groups compared with the control group. Conclusions: There were differences in the HPA axis and subjective response to the CPT between the control group and both the alcohol and PTSD groups. The HPA response in the comorbid alcohol-PTSD group was not significantly different than that of the alcohol-only or PTSD-only groups.

The present study compared high-risk triggers and substance use situations among 72 (34 men, 38 women) individuals with alcohol (AD) or cocaine dependence (CD), with or without comorbid PTSD. Consistent with the self-medication hypothesis, individuals with PTSD reported significantly greater use of substances in response to negative situations, such as unpleasant emotions and physical discomfort, as compared to individuals without PTSD. CD individuals were significantly more likely than AD individuals to report using in temptation situations, regardless of PTSD status. Also, CD individuals with PTSD reported greater use of cocaine during pleasant times with others, as compared to those without PTSD. Based on cognitive concepts of personality disorders as well as on the bio-social model of borderline personality disorder (BPD), a 34-item instrument, the questionnaire of thoughts and feelings (QTF) was developed for the assessment of feelings, strategic cognitions, and assumptions characteristic for BPD. In different studies, item- and factor analyses were conducted with a dataset of N=646 clinical and non-clinical participants. Cross-sectional data as well as longitudinal data are available including several other measures on personality and personality disorders: DSM-IV SCID II dimensional score, a personality inventory (PSSI), the German version of the personality disorder beliefs questionnaire (PBQ), as well as overall severity of symptoms (GSI) of the SCL-90-R. Internal consistency of the scale is excellent, Cronbach's @a=0.91. One-week test-retest reliability was also high r"t"t=0.81. Good convergent and discriminant validity was obtained for correlation with SCID dimensional scores and discrimination of BDP patients and other clinical and non-clinical groups. A comparison of scores in a sample of 26 patients with BPD during crisis intervention and 8 months later indicated the sensitivity of the QTF to measure change over time. Description:
Work-related musculoskeletal disorders account for the largest single category of lost-time occupational injury or disease episodes in industrialized countries. In this study we analyzed the impact of the psychosocial work environment, conceptualized by the demand-control-support model, on the incidence of low back and neck injury in a cohort of 1221 public transit operators followed for 7 years and 6 months. The two main exposure variables were ''job strain'' (mismatch of high psychological demands and low decision latitude) and ''iso-strain'' (job strain plus exposure to low social support at work). Analyses controlled for demographic factors, physical workload, and pain at baseline. For low back injuries, increased hazard rates were found for job strain and iso-strain based on tertiles, with hazard ratios (HR) of 1.30 (95% CI=0.96-1.75) and 1.41 (95% CI=0.98-2.01), respectively. Job strain and iso-strain based on median split or analyzed as continuous variables were not associated with low back injuries. For neck injuries, job strain and iso-strain based on median split showed HRs of 1.27 (95% CI=0.99-1.63) and 1.33 (95% CI=1.01-1.77), respectively. Job strain and iso-strain based on tertiles had HRs of 1.52 (95% CI=1.13-2.05) and 1.73 (95% CI=1.21-2.45), respectively. When analyzed as continuous variables, a one-point increase on the job strain and iso-strain scales led to an 8% (95% CI=0.98-1.19) and 14% (95% CI=1.02-1.27) increased hazard of neck injuries, respectively. This study shows the importance of the psychosocial work environment in the etiology of musculoskeletal injuries among transit operators. Since reviews have shown that psychosocial workplace conditions in this occupational group can be changed, these findings indicate a potential for prevention at the workplace. ... Read more
