Myocardial viability has become one of the most importantissues in clinical cardiology. In particular, absence or presence ofviability may be decisive in patient management, and the decision toperform angioplasty (PTCA) or bypass surgery (CABG) is frequentlybased on the accurate assessment of viability. Althoughechocardiography and conventional nuclear medicine techniques mayprovide valuable information on viability, positron emissiontomography (PET) is currently considered to be the gold standard forthe assessment of myocardial viability. The simultaneous evaluation ofmyocardial metabolism and perfusion allows precise delineation andaccurate quantification of residual myocardial viability in affectedregions. In addition, accurate quantification of myocardial perfusionalone may provide insight into the basic mechanisms of syndrome X andmay assist in the appropriate clarification of this clinicallycomplicated, but frequently occurring phenomenon. Besides that, cardiac PET may deepen our insight into metabolism andperfusion of cardiac muscle disease, particularly in hypertrophiccardiomyopathy. Furthermore, receptor imaging studies with PET willbecome important as the study of cardiac neurohumoral regulation inheart failure has gained in interest. Cardiac Positron Emission Tomography: Viability, Perfusion,Receptors and Cardiomyopathy describes the most recentdevelopments in cardiac PET as these are related to myocardialviability and myocardial perfusion studies of syndrome X.
