We have hypothesized a distinction between the processes required to control the timing of different classes of periodic movements. In one class, salient events mark successive cycles. For these movements, we hypothesize that the temporal goal is a requisite component of the task representation, what we refer to as event-based timing. In the other class, the successive cycles are produced continuously. For these movements, alternative control strategies can optimize performance, allowing timing to be emergent. In a previous study, patients with cerebellar lesions were found to be selectively impaired on event-based timing tasks; they were unimpaired on a continuously produced task. In the present study, patients with Parkinson's disease were tested on repetitive movement tasks in which timing was either event-based or emergent. Temporal variability on either type of task did not differ between on- and off-medication sessions for the Parkinson's patients nor did patient performance differ from that of controls. These results suggest that the basal ganglia play a minimal role in movement timing and that impairments on event-based timing tasks are specific to cerebellar damage. ... Read more

Since neurology is so complex, neurology appointments are so short, and I have so many questions about what is going on in my head, I decided to educate myself on the subject. After reading 'Adrenaline, the Inner World', I moved on to this text.I was pleasantly surprised that the text is well organized, the narrative is clear, simple and direct and on the whole, eminently readable.I applaud this text for being neither too simple nor too complex for a relatively well educated person to understand. Thanks

4-0 out of 5 stars Great for intermediates and updated treatment guidelines
This book is great for a quick review of specific diagnoses and common laboratory techniques, and it has many useful tables suited for "point of care". Treatment guidelines are often more updated and clear than those given in the big books (i.e. "Neurology in Clinical Practice" and "Adam And Victor's").

It is not recommended if you are a beginner in neurology, because it dose not deal with examinations skills or a differential approach by symptoms and signs. In the latter case I strongly recommend "Clinical Neurology (Lange Medical Books) by the great Michael J. Aminoff and the general neurology chapters in Harrisons, especially "Approach to the Patient with Neurologic Disease, ...with Peripheral Neuropathy, and ...with Muscle Disease". The general section in Harrisons: "Nervous System Dysfunction" and some specific chapters (Seizures and Epilepsy, MS, WD and many others) are also recommended.

To make this book even better I would suggest the authors to expand the sections on differential diagnoses listed for each major disease topic, i.e. to highlight the most characteristic features distinguishing among related diagnoses. Also inclusion of more diagnostic criteria would be nice (e.g. IHS).

Overall a great book if it is used properly. Already looking forward to a new and updated edition

It is widely accepted that the cerebellar and basal ganglia control circuits contribute to the programming of movement. Converging evidence from neuroimaging, limb control, and neuropsychological studies suggests that (1) people with cerebellar disease have reduced ability to program movement sequences in advance of movement onset and (2) people with Parkinson's disease are unable to maintain a programmed response or to rapidly switch between responses. Despite a substantial supporting literature, no studies have addressed these potential areas of speech programming disruption for speakers with ataxic and hypokinetic dysarthria. Control participants and adults with dysarthria completed speech reaction time protocols designed to capture these aspects of utterance preparation.
