Product Description
Complex Regional Pain Syndrome, also known as Reflex Sympathetic Dystrophy Syndrome (RSD), is a complex form of neuropathic pain associated with hyperpathia, intense neurovascular instability, neuroinflammation, and limbic system dysfunction. Understanding this self-perpetuating pain - which "never stops" - requires unbiased knowledge of physiology and pathology. Based on the bestselling Chronic Pain: Reflex Sympathetic Dystrophy, Prevention, and Management, the second edition classifies the different stages of RSD and describes the qualitative and quantitative differences between natural endorphins and synthetic narcotics. ... Description:
Recognising the laterality of a pictured hand involves making an initial decision and confirming that choice by mentally moving one's own hand to match the picture. This depends on an intact body schema. Because patients with complex regional pain syndrome type 1 (CRPS1) take longer to recognise a hand's laterality when it corresponds to their affected hand, it has been proposed that nociceptive input disrupts the body schema. However, chronic pain is associated with physiological and psychosocial complexities that may also explain the results. In three studies, we investigated whether the effect is simply due to nociceptive input. Study one evaluated the temporal and perceptual characteristics of acute hand pain elicited by intramuscular injection of hypertonic saline into the thenar eminence. In studies two and three, subjects performed a hand laterality recognition task before, during, and after acute experimental hand pain, and experimental elbow pain, respectively. During hand pain and during elbow pain, when the laterality of the pictured hand corresponded to the painful side, there was no effect on response time (RT). That suggests that nociceptive input alone is not sufficient to disrupt the working body schema. Conversely to patients with CRPS1, when the laterality of the pictured hand corresponded to the non-painful hand, RT increased ~380 ms (95% confidence interval 190 ms-590 ms). The results highlight the differences between acute and chronic pain and may reflect a bias in information processing in acute pain toward the affected part. ... Read more

Abstract:
Chronic post-surgical pain (CPSP) is an under-recognised and prevalent healthcare problem associated with significant morbidity and potential economic costs. Risk factors include the type of surgery, pre-existing pain, re-operation, nerve damage, moderate-to-severe acute post-operative pain, neurotoxic radio or chemotherapy and psycho-social factors. CPSP has a multifactorial aetiology, principally nerve injury and wound inflammatory response, leading to peripheral and central sensitisation. The extent of wound hyperalgesia following abdominal surgery correlates with the incidence of CPSP but not with acute pain outcomes, reflecting the relative importance of central sensitisation in the development of CPSP. The contributions of genetics, gender, age, opioid-induced hyperalgesia, pre-existing pain disorders and psycho-social factors to the pathogenesis of CPSP have yet to be clarified. The prevention of CPSP includes limiting nerve and tissue injury at the time of surgery and in some cases using preventive analgesia techniques such as regional neural blockade or low-dose ketamine infusion. Other strategies such as education, patient surveillance, management of psycho-social factors and functional rehabilitation may also be beneficial, although there are no data to support this. Further research is required to develop 'predictive tools' and to examine the effects of multimodal ''protective'' analgesia and multidisciplinary approaches in the prevention and treatment of CPSP. ... Product Description
A comprehensive Guidebook to treatment options and the latest medical advances for Reflex Sympathetic Dystrophy.

Reflex sympathetic dystrophy (RSD) is a complex, poorly understood disorder that is characterized by chronic, severe pain and progressive changes in skin, muscle, and bone. Although the precise causes of RSD are unknown, it often occurs following an injury, often minor in nature. Some experts believe that RSD represents an exaggerated response of the sympathetic nervous system to some form of injury or insult (eg. surgery) to the area resulting in chronic, severe, sometimes debilitating pain. Although the signs and symptoms of RSD vary depending upon the clinical stage of the disorder, the one common feature shared by all 3 clinical stages (early, established, or late RSD) is pain.

Because currently there is no cure for RSD, the goals of treatment include: 1) controlling and minimizing pain to the greatest extent possible; 2) restoring function to the RSD-affected limb; 3) preventing progression of the disease process to the late stage; and 4) improving the patient's quality of life and psycholocial functioning.

The MediFocus Guidebook on Reflex Sympathetic Dystrophy will inform you about the signs and symptoms of RSD, the treatment options including drug therapy, sympathetic nerve blocks, chemical and surgical sympathectomy, physical therapy, and other methods used for controlling pain and improving quality of life. You will also learn about new, experimental treatments that are currently under investigation that may be effective for the management of RSD.

You will also learn about the doctors, hospitals, and medical centers that are at the leading edge in conducting clinical research about RSD. A list of important questions to ask your doctor, and a valuable directory of resources, organizations, and support groups that can help patients with RSD are included in this Guidebook.
