From the author: Peritoneal dialysis is one of the treatments for end stage renal disease that requires considerable expertise. When neonates or children develop end stage renal disease, both the patient and family members must be provided with substantial education and training. The organization of education and training is paramount, especially as length of stay is limited with managed care. The development of an organized interdisciplinary approach for this instruction and pediatric patient teaching protocol for management of home peritoneal dialysis therapy is discussed in this article. The protocol provides learning content, methodologies and patient/family outcomes and is accompanied by a documentation tool that serves as communication for directing team member reinforcement of content until outcomes are met. Since completion of the protocol, eight pediatric patients have been placed on this teaching plan with extremely positive outcomes.

From the author: The purpose of this qualitative study was to explore the definitions and sources of hope in patients with end stage renal disease (ESRD) receiving chronic hemodialysis. A convenience sample was recruited from a population of chronic hemodialysis patients from two dialysis centers in a rural area of the Pacific northwest. Study participants consisted of 9 men and 5 women between the ages of 43 and 81 (M = 62.5) who had been on chronic hemodialysis an average of 8.3 years. The data collection process consisted of an audio-taped interview guided by the pre-established research questions. The results of this study provide examples of the experience of hope in patients with ESRD on chronic hemodialysis. Hope is a multifaceted human response. The participants in this study were able to adapt to situational changes by attaching their hopes to reality consideration, therefore developing a cognitive process for maintaining hope.

From the author: Children and adolescents with end stage renal disease (ESRD) require life-long treatment. The purpose of this descriptive investigation was to identify coping strategies that adolescents with ESRD use to manage their chronic illness. Participants for this investigation were 35 adolescents, 13-18 years of age, with ESRD. The A-COPE survey instrument was used in a clinical and camp setting to measure the coping strategies used by adolescents with ESRD. Analyses revealed that adolescents with ESRD utilized a variety of coping strategies to manage the stresses of living with their chronic condition. Personal characteristics of gender, transplant status, age, and religious views were significantly related to the coping strategies the adolescents reported using.

From the author: Chronic irreversible renal failure affects not only the child but the entire family. Physical, psychosocial, and financial challenges must be met and overcome. The primary goal is for the child to grow and develop as normally as possible, receiving the modality of treatment to best achieve this goal, with the fewest complications. Renal assessment includes a thorough history and physical exam, laboratory and radiological testing, and possibly a kidney biopsy. Causes of renal disease in children include congenital or genetic anomalies or conditions acquired after birth. When normal renal functions are impaired, clinical manifestations develop. Progression of chronic renal failure relates to the amount of glomerular filtration rate (GFR) remaining, and follows four stages, from early failure without symptoms, to end stage renal disease (ESRD) with many clinical symptoms of varying severity. In the final stage, renal replacement therapy by peritoneal dialysis, hemodialysis, or renal transplantation is necessary to sustain life. Nursing goals and interventive measures include maintaining fluid, electrolyte, and acid-base balance, and blood pressure control; promoting adequate nutrition, growth, and development; preventing complications related to progressive renal failure or treatment modality; and providing educational and psychosocial support to the child and family.

From the author: This article presents an analysis of the experience of family members of individuals with chronic renal disease undergoing dialysis therapy. Themes are derived from interviewing family members regarding the sources of uncertainty experienced and the types of coping strategies used to manage the stress related to uncertainty. The sample consisted of 41 family members including spouses, adult children, siblings, a niece, and a daughter-in-law from Eastern Canada. Data were collected from focus group interviews and thematic analysis was used to derive the themes. Emerging themes of uncertainty arose from many sources including: the patient's health, dialysis treatment, potential loss of the patient, and/or the availability of a kidney transplantation. The family members coped with the stress generated by the uncertainty by living each day as it comes, finding positive meaning, hoping for a transplant, and/or drawing on God's strength. Findings from this study provide direction for renal health care professionals to assist family members to prepare for, to understand, and to adapt to constant sources of uncertainty in their lives.

From the author: This quasi-experimental pilot study used a convenience sample of 23 adult men and women with either Type 1 or Type 2 diabetes who received hemodialysis three times a week. The experimental group received a four-part intervention that included individual and group foot care education, foot assessment, and special shoes. Pre- and posttesting of foot care knowledge was done using the Siriraj questionnaire. Out of a possible score of 20, those in the experimental group had a mean score of 15.3 versus 14.33 in the control group on the posttest. The nephrology nurse has the opportunity to play a role in early detection and intervention in clients with ESRD.
