Editorial Review

Book Description
Rome II presents the full reports from ten multinational working teams which not only detail the criteria for symptom based diagnosis of functional gastrointestinal disorders but also present the latest information on the pathophysiology, diagnostic approach and treatment of 24 functional GI disorders plus an additional new chapter on pediatric disorders.

This book can be used as both a textbook and an up to date reference source for anyone interested in clinical care or a comprehensive review of the field. It is must reading for clinicians and researchers. In bringing this new medical knowledge from the multinational working teams to primary care physicians and gastroenterologists throughout the world, this book advances a symptom based classification system developed as a new paradigm for the diagnosis and treatment of functional GI disorders so that patients suffering from these conditions may find the promise of relief contained in Rome II.

Rome II addresses some of the most puzzling and prevalent of chronic disorders. Functional gastrointestinal disorders, including irritable bowel syndrome, functional dyspepsia and chronic functional abdominal pain, are very commonly seen in specialty and general medical practices. Until recently, there have been no scientifically and psychologically based standards for approaching these patients. Nor have researchers found common frameworks within which to study the causes and treatments of functional gastrointestinal disorders.

Functional GI disorders historically have been considered second class. Clinicians and scientists paid little attention to these disorders, leaving patients dissatisfied with their care, and causing unnecessary diagnostic procedures. Funding agencies gave little attention to the investigation of these disorders. In the late 1980s, the multinational collaboration and publication of the work of the Rome Working Teams dramatically changed this field of medicine by legitimization of these disorders, resulting in an increase in research grants, pharmaceutical studies and publications.

Refining, expanding and updating that landmark work, the authors of Rome II report on validation studies of its original Diagnostic Criteria, new findings in basic and clinical research, the new classification these disorders in children, and promising new treatments on the horizon. ... Read more

Book Description

A child presents with weight loss, fatigue, and stomach pain. Is the diagnosis psychological, as in an eating disorder? Might it be medical, as in a GI disorder? Could it be both?

A young adolescent is assessed to have a gastrointestinal disorder. Do you take puberty into account when planning treatment?

Complex overlapping of physical and psychological symptoms is a hallmark of cases such as these - and a major factor in making diagnosis and intervention difficult.

In Pediatric Gastrointestinal Disorders:Biopsychosocial Assessment and Treatment, authors Cunningham and Banez pool their considerable expertise to give practitioners working with children a state-of-the-art, biopsychosocial grounding in this challenging area of practice:

- In-depth overview of pediatric gastrointestinal disorders, their prevalence and etiology

- Descriptions of the most common disorders (including irritable bowel syndrome, recurring abdominal pain, rumination disorder, and fecal incontinence), with their typical psychological and behavioral symptoms

- Conceptual and theoretical perspectives informing the authors biopsychosocial approach

- Reviews of empirically-based, clinically sound assessment and treatment strategies

- Case studies applying this knowledge in real-world detail, demonstrating collaborations between clinicians, patients, and families

Its lucid presentation, level of detail, and commitment to integrative care make Assessment and Treatment of Gastrointestinal Disorders in Children an important resource for childrens practitioners in both the medical and mental health fields. By featuring examples ranging from toddlers to teens, the authors have modeled a continuum of developmentally appropriate treatment.

5-0 out of 5 stars An Excellent Reference Book
Many of us in pediatric psychology working with patients with GI disorders did not aquire our knowledge of GI disorders while in graduate school.Instead, we sought out and amassed any literature on the topic we could find, learned from our GI colleagues, and perhaps most importantly, learned from clinical experience. This book is perhaps the first to condense all of that literature into one useful resource and make it applicable to the practitioner. The book provides a well-written explanation of the biopsychosocial approach to treatment, and case examples that bring research findings to life.

I sure wish I had this text when I was starting my work in GI/pediatric psychology!

5-0 out of 5 stars Excellent resource
This book is an excellent resource for both professionals (e.g., physicians, behavioral/mental health care providers) and parents.Well-written, thorough, and easy to understand, Cunningham and Banez's text covers topics ranging in content from IBD/IBS and pediatric toileting difficulties to esophageal disorders, and all discussion is couched within an informative biopsychosocial approach.I would highly recommend this text to anyone working with, or living with, a child or adolescent struggling with GI difficulties.

5-0 out of 5 stars Peds GI Better Understood
This book is a great resource for a wide variety of pediatric healthcare providers. It provides an in depth understanding of a broad spectrum of factors affecting pediatric gastrointestinal complaints and related problems. This book can better enable primary care providers to deal with many of these issues. In some cases the kids may even be able to avoid visits to specialists. In other cases, the primary care providers will be better able to understand, support and complement the care their patients are receiving from specialists.
Pediatric specialists-such as psychologists and those in the field of gastroenterology- can also benefit from this reference. It provides a very thorough look at GI complaints from a biopsychosocial perspective, allowing for a more holistic approach to treating patients.

Editorial Review

Book Description
Significant progress has been made in several areas ofgastroenterology over the last two decades. The understanding of physiologyof gastrointestinal tract covering motility, sensitivity, absorption, andmolecular biology of gastrointestinal cancers, particularly colorectalcancer, has immensely improved. Recent years were dominated by increasedpublic attention focused on functional bowel disorders. Irritable bowelsyndrome is a functional bowel disorder characterized by abdominal pain anddiscomfort without clearly demonstrable organic cause leading to alteredbowel habits. Since the prevalence of irritable bowel syndrome is high, itrepresents a significant societal and economic burden. Better understandingof the function of the enteric nervous system supported by the evidencethat pain and discomfort experienced by the patients is due tohypersensitivity of vagal and spinal sensory neurons allowed for selectivetargeting receptors expressed by the afferent neurons for therapeuticintervention. Emerging therapies for irritable bowel syndrome, such astegaserod, alosetron and cilansetron, affect the serotonergic system.

This publication is of interest to experts from a variety of disciplinesincluding medicine, physiology and pathophysiology, medicinal chemistry, pharmacology, and molecular genetics as well as to the general public. ... Read more
