Studies of DNA damage in gastric epithelial cells of Helicobacter pylori (H. pylori)-infected patients are conflicting, possibly due to different methods used for scoring DNA damage by Comet assay. Therefore, we compared the sensitivity of visual microscopic analysis (arbitrary units-scores and comets%) and image analysis system (tail moment), in the gastric epithelial cells from the antrum and corpus of 122 H. pylori-infected and 32 non-infected patients. The feasibility of cryopreserved peripheral blood lymphocytes and whole-blood cells for DNA damage biomonitoring was also investigated. In the antrum, the levels of DNA damage were significantly higher in H. pylori-infected patients with gastritis than in non-infected patients with normal mucosa, when evaluated by image analysis system, arbitrary units and comets%. In the corpus, the comets% was not sufficiently sensitive to detect the difference between H. pylori-infected patients with gastritis and non-infected patients with normal mucosa. The image analysis system was sensitive enough to detect differences between non-infected patients and H. pylori-infected patients with mild gastritis and between infected patients with moderate and severe gastritis, in both antrum and corpus, while arbitrary units and comets% were unable to detect these differences. In cryopreserved peripheral blood lymphocytes, the levels of DNA damage (tail moment) were significantly higher in H. pylori-infected patients with moderate and severe gastritis than in non-infected patients. Overall, our results indicate that the image analysis system is more sensitive and adequate to measure the levels of DNA damage in gastric epithelial cells than the other methods assayed. ... The format of the meeting, with short fifteen-minute state-of-the-artpresentations by world experts closely involved in Helicobacterresearch followed by ample time for panel discussions, was againfollowed this year. Traditional aspects included detailed study of themicrobial characteristics, the novel Helicobacters, theinteraction with the human host, the peculiarities of the inflammatoryimmune response, the short and long-term mucosal consequences, theeffects on acid secretion, the problem of gastric malignancy, and thetherapeutic possibilities.
However, a series of short debates was introduced to highlightcontroversial issues, such as the pathogenic or commensal role of theorganism, the role of virulence factors, the characteristics of theinflammatory immune response, the reversibility of mucosal atrophy andmetaplasia, and the primary or secondary role of quadruple therapy, toname just a few. This provocative approach was very successful andprovided fuel for further in-depth discussions. These debates havebeen included in this book in the form of conventional manuscripts inorder to maintain the shortest possible production time for thepublication of the proceedings.
Helicobacter pylori has come a long way since the first meetingin Amelia Island, in 1993, and the science of this organism and theunderstanding of the consequences of the infection have advanced ourknowledge fundamentally and revolutionized our thinking aboutgastrointestinal disease. Helicobacter pylori is responsible for one of the most frequently encountered infectious diseases world-wide. Infection with this organism can lead to the development of gastritis and peptic ulcer disease. The presence of Helicobacter pylori in the human stomach also represents an increased risk for gastric cancer and gastric lymphoma. Epidemiological data obtained in adults suggest that the actual colonization with Helicobacter pylori is in fact determined by childhood factors. Therefore, the pediatric age group represents the ideal target population for studies concerning the pathogenesis and epidemiology of Helicobacter pylori infection. The present work reflects our experience with regard to the diagnosis, epidemiology and pathogenesis of Helicobacter pylori infection in childhood and adds information to the actual knowledge about this disease. By investigating large series of asymptomatic children and young adults, we were able to determine that Helicobacter pylori is present in young children and that its prevalence increases with age. Significant differences exist in the prevalence of Helicobacter pylori seropositivity between children of different ethnic background living in the same country. Almost 50% of first degree family members of children with Helicobacter pylori associated gastritis are positive for this bacterium. Anti-Helicobacter pylori specific IgG antibodies cross the placental barrier and disappear by the age of 3 months. However, despite the close personal contact with their mothers, children born from Helicobacter pylori positive women do not seem to have an increased risk to become actively infected with this bacterium during the first year of their life. In conclusion the present work demonstrates that Helicobacter pylori infection is an important issue of public health in children, both symptomatic and asymptomatic. Very recently it has been suggested that Helicobacter pylori might play an important role as co-factor in the evolution towards gastric cancer and -lymphoma. This is of special importance in view of the fact that asymptomatic carriers of Helicobacter pylori can harvest this infection for a long period of time. The possibility exists that these individuals might be at a greater risk for developing gastric cancer at a later stage in their life. ... Read more
