From the author: A point-in-time observational study was conducted in over 1,000 nephrology offices between November 1999 and December 2000 to determine the prevalence of anemia in patients with chronic kidney disease (CKD). Trends in hemoglobin (Hb) and serum creatinine (SCr) levels were assessed among 4,831 evaluable patients to determine the relationship between renal function and anemia. Results demonstrated that anemia is common in patients with CKD, with progressive increases in prevalence and severity as renal function deteriorates. 26.3% of these patients had Hb levels below 10 g/dL, 46.7% had Hb levels below 11 g/dL, and 63.9% had Hb levels below 12 g/dL. Despite the high percent of patients with anemia, only 31.3% were being treated for this condition. These data suggest that anemia is prevalent and undertreated in patients with CKD. Nephrology nurses can be influential in providing proactive management of anemia throughout the spectrum of CKD to improve anemia-related outcomes.

From the author: Acute renal failure (ARF) is a common complication of critically ill patients in today's intensive care units. Intermittent renal replacement therapy for these types of patients may be limited or ineffective due to the critical nature of their illness. Volume overload and hemodynamic instability are complications that may not be treated adequately with conventional forms of dialysis, suck as hemodialysis or peritoneal dialysis. Continuous renal replacement therapy (CRRT) is rapidly gaining ground as the treatment of choice for ARF in the intensive care unit (ICU) due to its slow, gentle nature of water and solute removal. Critical care nurses are responsible for monitoring this therapy, but a collaborative effort with nephrology nurses' expertise and background in dialysis therapies is a key ingredient in implementation of a successful CRRT program. This article will review the causes of ARF, the history of CRRT, current treatment options, trends, and implementation of a successful CRRT program.

From the author: The effectiveness of anemia management in patients with end stage renal disease (ESRD) has increased over the past 4 years. However, approximately 26% of treated patients still do not meet the minimum hemoglobin (Hgb) value of 11 g/dl that is recommended by the K/DOQI Clinical Practice Guidelines (National Kidney Foundation [NKF], 2001). One of the main obstacles to good patient outcome may be iron deficiency, which is common in both the predialysis and dialysis period. Since iron is needed for Hgb synthesis, iron depletion exacerbates anemia and reduces the response to recombinant erythropoietin (rEPO) therapy. Health care providers can significantly improve patient outcome by addressing iron deficiency more rigorously. A good starting point is the establishment of an iron deficiency management protocol that includes early evaluation of iron status and aggressive iron therapy. Iron therapy, in turn, can be optimized by administering safe and effective iron supplements and by implementing maintenance iron regimens to prevent the recurrence of iron deficiency. By making these simple improvements to their treatment approach, clinicians can enhance the effectiveness of anemia management in patients with ESRD.
