This book provides a thorough and useful survey of the history of our understanding of psoriasis and its treatment, spanning mainly the last two centuries.The writing is clear and the ~120 pages don't take too long to read, although the technical level is moderately high, so the book is targeted mainly at professionals.

A couple of quirky aspects are that the author's credentials aren't stated (though she appears to be a qualified researcher) and, more significantly, the book doesn't have an index.

The following are some interesting and/or important points from the book:

- The presence of increased vascularization (with increased capillary tortuosity and permeability) and leukocytes in psoriatic lesions was identified more than a century ago.Later studies revealed the presence of epidermal hyperproliferation driven by an external stimulus, inadequate epidermal differentiation, and identification of T cells (particularly Th1 CD4+ cells) as the primary leukocytes involved.

- Psoriasis hasn't been observed in non-human animals, which limits the usefulness of animal models.

- There are a wide variety of treatments for psoriasis, with diverse mechanisms of action.No treatment works for everyone, no treatment is curative, and treatments often eventually lose efficacy.Spontaneous remission of psoriasis is also sometimes observed.

- The earlier psoriasis treatments were discovered mainly by trial and error, yet many are still in use decades after their discovery.

- Some of the early psoriasis treatments, such as arsenic, mercury, and X-rays, were highly toxic.Some of the treatments still in use today, such as methotrexate and hydroxyurea (both of which are cancer drugs), are also quite toxic at higher doses.

- Corticosteroids were among the first psoriasis treatments with reasonably well understood mechanisms of action.Slight modifications in the molecular structure of these steroids can greatly change their potency.

- More than 80% of patients who have tried Dead Sea bathing have seen complete or nearly complete clearing of their psoriasis (but not permanently), and many have also seen improvement in their arthritis.

- Phototherapy (a type of radiotherapy) and photochemotherapy (phototherapy typically combined with a photosensitizer) have been found effective against psoriasis, although they carry some carcinogenic risk.

- Cold weather, stress, poor diet, and skin trauma are known to contribute to psoriasis.

- Microbes appear to contribute to triggering or exacerbating psoriasis in some patients.

- Some drugs used for other conditions, such as chloroquine, lithium, beta-blockers, and indomethacin (a COX inhibitor), can trigger or exacerbate psoriasis.

- A genetic contribution to psoriasis became clearly evident many decades ago.Later studies showed that psoriasis is polygenic, which is part of the reason for the clinical heterogeneity of psoriasis.Some of the genes linked with psoriasis also appear to be linked with inflammatory and autoimmune diseases such as atopic dermatitis, Crohn's disease, rheumatoid arthritis, and lupus.

- Cytokines appear to play a large role in driving psoriasis.TNF-alpha is an unusual cytokine because it inhibits keratinocyte proliferation, but it also indirectly contributes to epidermal growth by inducing IL-8 and TGF-alpha production, with the net result that inhibiting TNF-alpha clinically improves psoriasis.

- The key role of the immune system in psoriasis, particularly modulation by T cells, is evidenced by the clinical efficacy of broadly immunosuppressive agents (eg, cyclosporin and tacrolimus) as well as targeted immunotherapies ("biologics").Data on the long-term safety of targeted immunotherapies (most of which target TNF-alpha) isn't yet available.

- Antigens are hypothesized to drive psoriasis, but specific antigens haven't been identified, and thus it's not yet known whether psoriasis is an autoimmune disease involving self-antigens (though many people suspect this).

- Gene therapies (anti-sense RNAs) for psoriasis are under development.In addition, pharmocogenomics may eventually enable individualized treatment of psoriasis.

One feature of psoriasis not mentioned in this book, and infrequently mentioned in the broader literature, is the high level of bilateral symmetry of psoriasis.It seems to me that this is a very striking and important pathophysiologic clue which researchers have neglected to adequately investigate.

Overall, I think this book is a must-read for psoriasis researchers and clinicians, and patients and caregivers with a good grasp of biomedicine may also find the book beneficial. ... Read more
