Description:
Background: Nosocomially acquired respiratory syncytial virus (RSV) infections cause serious problems in hospitalized patients. An increased effort should be made to describe the problems connected with such infections in pediatric hospitals, with the aim of reducing the occurrence of nosocomial RSV infections (NI). Methods: A specialized database was introduced for surveillance and a multifaceted barrier concept based on the CDC recommendations was developed for the control of NI in a university children's hospital in Germany. Results: Between 1999 and 2002 (November 1-April 30), 283 RSV infections (general population) were prospectively documented. Thirty-nine cases (13.8%) were nosocomial infections (NI) with an incidence density (ID) of 0.99/1000 patient days; 48.7% of all NI were found in prematurely born infants. Following the introduction of a surveillance and prevention policy, a 9-fold decrease of the ID (1.67 vs. 0.18/1000 patient-days) was found when comparing the first and the last season. Intensive care treatment was required in 18% of all documented RSV-infections, in 48.7% of all NI cases and in 43.5% of all RSV-infected prematurely born infants. Overall RSV-related mortality was 0.71%. Conclusions: Early diagnosis, a strict cohorting and contact isolation policy, and prospective surveillance contribute to the reduction of nosocomial RSV infection. ... Read more

From the author: Respiratory syncytial virus (RSV) is the most common cause of bronchiolitis and pneumonia among infants and children. The purpose of this paper is to review the epidemiology, etiology, pathophysiology, clinical manifestations, risk factors, and assessment of RSV infection in infants and young children. There is a lack of consensus regarding the optimal treatment for children with RSV infection. Bronchodilators, racemic epinephrine, inhaled and systemic corticosteroids, RSV-immunoglobulin (RSV-IG), and ribavirin have all been used for treatment of children with RSV infection. A review of current research indicates supportive and symptomatic management should be the mainstay of treatment. Ultimately, prevention of infection through education and immunotherapy is the key to reducing the morbidity and mortality associated with RSV bronchiolitis.

From the author: Purpose: To compare two types of specimen collection methods for RSV (respiratory syncytial virus): nasal wash and nasopharyngeal wash. Methodology: An exploratory descriptive research design was used to examine nasopharyngeal wash and nasal wash methods of collecting specimens for RSV to determine if one method gives more consistently accurate results. Sample size was 66. Findings: Using chi square analysis on a contingency table, there was no significant difference between the two collection methods. Only 2 of the 66 samples showed a difference in the two collection methods by giving a positive result with the nasopharyngeal method. Conclusions: There is no significant difference between using nasal wash and nasopharyngeal wash methods for collecting specimen for RSV testing. Nasal wash is a safe and accurate method for obtaining specimen for RSV.

From the supplier: RespiGam, a respiratory syncytial virus (RSV) immune globulin, appears effective in preventing or minimizing the severity of RSV infection in premature infants and infants with bronchopulmonary dysplasia. In one study, RespiGam reduced RSV lower respiratory tract infections by 62% and moderate or severe RSV infections by 72% when compared with those in a control group. Side effects include fever, headache, and nausea, in addition to wheezing or respiratory distress in a small number of patients. RespiGam is administered intravenously, and is quite expensive.
