This paper compares the portrayal of breast, testicular and prostate cancer in mass print English language magazines in the United States and Canada from 1996 to 2001. It is a follow-up of three papers that examined each of these three diseases separately in high circulating magazines up to 1995. It includes both quantitative and qualitative analyses of magazine stories and notes the continuing dominance of a medical perspective regarding disease as well as the association of each type of cancer examined with stereotypically individualized yet feminine and masculine characteristics and pursuits. It notes the conflation of breast cancer, since the discovery of BRCA1 and BRCA2, with the family. To be a 'feminine' woman is to be vulnerable to breast cancer and to be a 'masculine' man is to be vulnerable to testicular cancer when young and prostate cancer when older. The association of disease not just with personhood but also with the specifics of stereotyped masculinity and femininity may construct a more intimate, more personal link between disease and identity. This close attachment of gender and disease may shore up and exacerbate a fear reaction. It may also serve to diminish the awareness of other, more prevalent, causes of death for men and women. Each year there are approximately 8000 new cases of testicular cancer diagnosed in the United States, resulting in almost 400 deaths. The incidence of testicular cancer, the most common solid tumor in men between the ages of 20 and 35 years, is rising in the United States and Europe. The management of patients with testicular cancer has evolved significantly over the past 25 years, representing a model in the successful multidisciplinary approach to a solid malignancy with the appropriate integration of surgery and cisplatin- based chemotherapy resulting in improvements in survival from 60% to 65% in the 1960's to over 95% at present.Given the high probability of achieving cure, considerable effort has been focused on individualizing therapy for specific patients in an attempt to minimize morbidity, preserve quality of life without adversely impacting survival. This has been possible because of refinements in histologic, serologic, and radiographic parameters which has resulted in improved risk assessment for patients of all stages. Surgical techniques and the development of less toxic and more effective chemotherapy regimens has also played a critical role. The apparent overtreatmentof many patients, and the availability of multiple approaches that achieve cure, has resulted in a number of significant controversies in the management of testicular cancer.The long term survival of patients has identified significant issues that appear to have been underappreciated in the past, such as late relapse, reoperative surgery and the long-term sequelae of treatment, particularly chemotherapy. Some of these problems have prompted us to reexamine and challenge previously accepted treatment paradigms.This issue of the Urologic Clinics of North America describes the current state- of-the-art therapy for testicular cancer and specifically addresses some of the important controversies faced by clinicians today. From the author: This study compared high school health teachers' beliefs concerning teaching about testicular cancer (TC) and testicular self-examination (TSE) to actual instruction. The Teachers' Testicular Cancer Beliefs Survey was developed, using the Health Belief Model (HBM) as a theoretical framework. Multiple regression analysis of HBM subscales determined perceived seriousness of TC was the most signif cant predictor (p [is less than] .05) of TC instruction. Perceived barriers to teaching about TSE was the most significant predictor (p [is less than] to] .05) of TSE instruction. Male health educators were more likely (p [is less than] .001) than females to teach about TC and TSE. Teachers having previous preparation in TC and TSE provided more instruction (p [is less than] .001) than those without. Less than half of respondents provided TC instruction, while TSE instruction was provided by less than one-third Comprehensive cancer prevention and early detection education, including strategies for teaching about TC and TSE, should be part of each health teacher's training.
