This randomized trial compared a combined Acceptance and Commitment Therapy/Habit Reversal Training (ACT/HRT) to a waitlist control in the treatment of adults with trichotillomania (TTM). Twenty-five participants (12 treatment and 13 waitlist) completed the trial. Results demonstrated a significant reduction in hair pulling severity, impairment ratings, and hairs pulled, along with significant reductions in experiential avoidance and both anxiety and depressive symptoms in the ACT/HRT group compared to the waitlist control. Reductions generally were maintained at a 3-month follow-up. Decreases in experiential avoidance and greater treatment compliance were significantly correlated with reductions in TTM severity, implying that targeting experiential avoidance may be useful in the treatment of TTM. Description:
Post-treatment evaluation studies of behaviour therapy (BT) for trichotillomania (TTM) have shown that BT is successful in reducing symptoms in this impulse-control disorder. The present study was aimed at investigating gain maintenance at long-term follow-up. TTM-related symptoms and other symptom characteristics were evaluated in 28 patients suffering from TTM before and after brief BT and at a 3-month and 2-year follow-up. The manual-based BT consisted of self-control procedures offered in six sessions. Pre-post effect sizes for TTM symptoms at post-treatment evaluation and at the two follow-ups were 2.91, 1.47, and .87. Compared to the post-treatment effects, the 3-month and 2-year follow-up effect sizes had decreased by 49% and 70%, respectively. Better 2-year follow-up results were associated with lower pre-treatment levels of depressive symptoms and with complete abstinence from hairpulling immediately after treatment. ... Read more

Background: Obsessive-compulsive disorder (OCD) and trichotillomania (compulsive hair-pulling) share overlapping co-morbidity, familial transmission, and phenomenology. However, the extent to which these disorders share a common cognitive phenotype has yet to be elucidated using patients without confounding co-morbidities. Aim: To compare neurocognitive functioning in co-morbidity-free patients with OCD and trichotillomania, focusing on domains of learning and memory, executive function, affective processing, reflection-impulsivity and decision-making. Method: Twenty patients with OCD, 20 patients with trichotillomania, and 20 matched controls undertook neuropsychological assessment after meeting stringent inclusion criteria. Results: Groups were matched for age, education, verbal IQ, and gender. The OCD and trichotillomania groups were impaired on spatial working memory. Only OCD patients showed additional impairments on executive planning and visual pattern recognition memory, and missed more responses to sad target words than other groups on an affective go/no-go task. Furthermore, OCD patients failed to modulate their behaviour between conditions on the reflection-impulsivity test, suggestive of cognitive inflexibility. Both clinical groups showed intact decision-making and probabilistic reversal learning. Conclusions: OCD and trichotillomania shared overlapping spatial working memory problems, but neuropsychological dysfunction in OCD spanned additional domains that were intact in trichotillomania. Findings are discussed in relation to likely fronto-striatal neural substrates and future research directions. ... Read more

Many individuals with trichotillomania (TTM) do not disclose their condition to others. Although disclosure may have beneficial effects, little is known about the consequences of it. The purpose of the current study was to examine the effects of a brief, simplified preventative disclosure of TTM. Young adults (N=225) read one of four vignettes, which varied in a 2 (male versus female character)x2 (preventative disclosure of TTM versus nondisclosure) design, and answered several questions regarding the character in the vignette. Although some potential positive effects of disclosure were found (e.g., reduced likelihood of misattribution), some areas of concerns also surfaced. The findings suggest that a brief, simplified preventative disclosure of TTM led to an increase in negative social perceptions compared to non-disclosure. More specifically, those characters that disclosed TTM were evaluated more negatively and more socially rejected than those that did not disclose. However, the overall perceptions of persons with noticeable hair loss, regardless of disclosure status, were not necessarily negative. This is the first book I read on the subject of trichotillomania, and I felt as if Cheryn were talking to me personally.I cried tears of relief to know that others had this disorder, and that they shared so much in common with me.Trichotillomania is a disorder of isoaltion and secrecy.That is almost worst than the hair pulling itself.This book truly helped me to understand that I was not alone, and to know that others felt the same way and had gone through many of the same things.Cheryn is personable, sincere, compassionate and loving, and this book will reach out and touch any body who reads it.

I bought this book a couple of years ago and it immediately gave me hope and success with my lash/brow pulling; Cheryn and I talked via email many times and she is a wonderful, caring person and it helps to have someone to talk with about this disorder.The book is a MUST for anyone suffering from trichotillomania.I cannot understand why anyone would give the book less than five stars.Buy it, read it, and read it again and again when you pull again and again.

I read Cheryn's book when I first discovered I was not the only one (about 2 years ago).It was so comforting to hear her personal struggle with trichotillomania, and I was amazed by how much of it paralleled my own life.I experienced the full range of emotions while reading-- from laughing out loud to sobbing.Knowing that I was not alone gave me the courage to go seek (and find!) the help I needed.Cheryn truly helped me to change my life.I recently had the honor of meeting her in person; she's absolutely one of the most amazing women ever!

I only found out a couple of years ago that trichotillomania is not really a rare disorder.....I have had it for 34 years. I read Cheryn's book and I have to admit that upon reading the poem on the first page, I cried so much I couldn't read the rest of the book until the next day. It was so incredible to find out others shared my secret and Cheryn did a wonderful job of putting it into words, which must have been really hard, since none of us want to talk about it.I also think it's a great book for anyone to read because Cheryn is an inspiration!

If you or someone you know suffers with trichotillomania, this book offers very helpful insights from a personal sufferer. I know this firsthand as I also have trich. While reading Cheryn's accounts of her struggles with this condition, I felt like I had found a long-lost friend, as I shared so many of the same emotional and social snags associated with it. But more than a simple account of growing up with a very trying and persistant handicap, Cheryn invites the reader to accept the condition for what it can bring sufferers in a positive light. As she explains, for those who have trich, the challenge of continually learning to accept oneself as a whole person is magnified, but this can lead to some wonderful opportunities to grow in ways that only other sufferers can know.
