I was very disappointed after reading this book. I was searching for an up-to-date text on the topics of reliability, availability and maintainability that would clarify relationships between a number of relevant concepts, such asmodeling frameworks (block diagrams, probability graphs, markov models) and solution techniques (factoring, decision diagrams etc).

Unfortunately, this book failed to clarify these relationships and present them in a unifying and clarifying manner. The style of writing was very verbose, repetitive and hard to follow. The solution techniques that were discussed were presented in isolation, with no unifying and clarifying view. Discussion of some solution techniques (such as Binary Decision Diagrams, which were the topic of my PhD dissertation) are on the border of incorrect!

In conclusion, I would not recommend buying this book - after reading it I felt that my understanding of the topics discussed in the book did not significantly improve. As a result, I was forced to search for other resources to learn more about reliability and availability. At the end, I found a number of freely available papers and presentations at [...]which were significantly more clarifying than this overpriced book. ... Read more

In the last paragraph on the bottom of page 58 it states, "and individuals who were involved in the accident".Per an FAA Investigator-In-Charge (Coordinator) and in my opinion, this is an inaccurate statement.

On page 136, figure 6-1, in my opinion, the arrowheads pointing at "unpredictable acts" should instead be pointing at "aircraft capabilities".

"Commercial Aviation Security" is a comprehensive book on various safety and security aspects of commercial aviation. This is one of the best technical books I've ever read on any subject--and I've read many of them!It should be must reading not only for those who conduct transportation safety studies--the subject of the book-- but also for those conducing any observational study ("before/after study"), whether safety-related or not.

This book is a valuable and much-needed contribution to the technical literature.The book begins by showing how classical statistical techniques are often misused in observational studies.Classical techniques assume a rigorous experimental design, with random assignment to treatment and non-treatement groups.These conditions do not apply to observational studies, such as those determining the influence of smoking on health.Errors such as assuming safety in the immediate "before" period is a good predictor of safety in the "after" period are discussed.The book then goes on to provide valid techniques that work.It shows how to compare the what would have been the safety in the after period without treatment, to the safety that was achieved.Complications of assessing the safety in the before and after periods are addressed.

The book is not only an important technical contribution, it is also extremely well-written.The author's writing style is extremely lucid and should serve as an example to others.The first section of the book provides a qualitative description of the material.Subsequent sections then repeat the material, but with mathemtatical rigor.Numerous numerical examples (some with real-world data; others with hypothetcial data for simplicity sake) make the material clear.Mathematical dervivations are provided for rigor, but isolated from the rest of the text to keep from overwhelming the reader.

The author writes with a conversational style, frequently re-emphasizing important points.He takes care to show at all points where he is in the logical discussion and how the current topic fits in to the overall picture.Some of the words are written tongue in cheek.My favorite is his description that
"they do not have a problem of congnition, but only of brittle bones".

In short, the book is outstanding for both its technical content and for the way the material is presented. ... Read more

Do not be confused of the book name. This book is not talking about the surgical device safety. It is a literature review of minimally Invasive surgical (MIS) device design problems. I borrow this book from library. After reading some chapter, I go to bookstore to figure it later, and give up the ideas of writing teaching materials. I also discard the papers and notes I collected, just keeping those not mentioned in this book.

A brief introduction can be found on Paul H. King, Engineering in Medicine and Biology Magazine, IEEE. 2006;25(1):14. You also can find some page through Google.

The text consists of 12 chapters. Chapter 1 and 12 is a introduction to MIS device design and training. Chapter 2 discuss about surgical procedure task analysis.

Chapter 3 "Observation and Manipulation in Laparoscopic Surgery", and Chapter 4 "Eye-Hand Coordination in Laparoscopy" cover the visual impendence, mis-location and misorientation. The interaction of device tip motion with the monitor height and position is demostrated clearly. You will not miss in the jargons of color science, visual theory. Though the authors discribed clear of fleible wrist of DaVinci system (US Patent No.5,797,900,1998)in chapter 3, the key image patent (US Patent No.7,397,934,2008): "Registration of Thoracic and Abdominal Imaging Modalities" is not see then.

chapter 5 "Force Transmission of Laparoscopic Graspers", Chapter 6 "Safe Manipulation of Bowel Tissue", and Chapter 7 "Design and Evaluation of Endoscope Positioners" review the device ergonomic problems. It discuss about how the force transmission from your hand to the grasp tip, the laparoscopic instrument sensation of surgeon, and the jaw design demage factor on bowel.

Chapter 3-7 is enough for bed-side teaching. You need not waste time of data mining in EI, SCI papers.

In chapter 9, the authors did not mentioned about Osirix TM (Mac software). It is the best free software for virtual colonscopy, and frendly in bed-side teaching.

I have no comment of chapter 8 discuss arthroscopy, and chapter 10,11 discuss catheters, because I am not a orthologist or cardiovascular surgeon.

The authors skip about the surgeon's muscle fatigue problems, including handle design, surgical table height, EMG and 3D-motion analysis. The direct energy source device is also not mentioned. But you can find excellent review through Pubmed.

Conclusion, If you are a laparoscopic surgeon, and not interested in bioengineering. Or if you are a bioengineering new investigating in laparoscopic device design. This is a book for you. Incidents That Define Process Safety

Many younger employees do not remember the process plant disasters of the 1980's. John Atherton and Frederic Gil have summarized many of those events and more in Incidents the Define Process Safety, published by the Center for Chemical Process Safety. Atherton has 40 years of experience, including 25 years working for BP in process safety management. He is a chartered engineer, a member of IChemE and was awarded the Ned Franklin medal by IChemE for his contributions to process safety. Gil has worked as a fire, process safety and loss prevention engineer for 18 years.

The incidents are grouped in chapters by common cause, though multiple factors contributed to all of the incidents. The first cause is blind operation where the hazards are not seen, including two plane crashes and the incident at Three Mile Island. Design has contributed to incidents as well, like the biggest process incident in history at Union Carbide in Bhopal. The Hindenburg fire is also covered with this group. External causes contributed to the fire storm at Pemex in Mexico City and fire at Tupas in Izmit, Turkey.

A lack of inspection and maintenance was a factor in the fire at Texaco in Milford Haven and the HF release at Marathon Oil in Texas City. Failure to provide adequate process knowledge is cited as a cause in the BLEVE (boiling liquid expanding vapor explosion) at Elf in Feyzin and the explosion at AZF in Toulouse. Other incidents were partially caused because process hazards were not understood, including the explosions at Esso in Longford and at BP in Grangemouth.

Improper management of change is associated with two of the most famous incidents, the Chernobyl disaster and the explosion at Nypro in Flixborough. Missing the opportunities to learn from previous near misses was a factor in the destruction of the Space Shuttle Columbia. Poor operating procedure was one of the causes for the most recent incident cited, the explosion at BP in Texas City.

Several disasters occurred in part due to poor practice around work permits, including the incident at Motiva in Delaware, the explosion at Phillips in Pasadena, and Piper Alpha platform fire that killed 167. Poor emergency response contributed to the consequences of the incident at ICMESA in Seveso which helped inspire the EU Seveso directive. Emergency response was also a factor in the boil over at Tacoa in Venezuela. The final group of incidents is related to human factors, including the Exxon Valdez oil spill in Alaska.

Sharing the lessons of history is always of benefit. Atherton and Gil broaden the usual list with many non-process incidents. While this may be interesting in a historical sense, it leaves little room to fully discuss the process incidents that really did define process safety. The diluted discussion makes Incident that Define Process Safety a more boring book (BB) than it had to be.

As a Process Reliability Engineer working in a major refinery I looked forward to reading this book. I ordered it hoping that it would be an extension of British Petroleum's fact-based and well-articulated sixteen volume process safety series available through Amazon and IChemE (ISBN 0852955219). The sections that explain the sequence of events behind refinery safety incidents are excellent. They contain meaningful lessons learned about equipment operation, maintenance, and human factors that readily apply in the processing and manufacturing industry.BP displays an incredible amount of humility by allowing the public to look deep inside some of their own process safety failures.It is our responsibility to constructively use this information to avoid repeat failures in our own facilities.

But what spoiled it for me is the insertion of "lessons learned" from some highly sensationalized, historic disasters. For example, the Hindenburg summary on pages 52 - 56 ties the safety message to theoretical arguments based on limited research and internet blogs instead of the sound forensic engineering practice that was used to determine a probable cause for the manufacturing industry case histories.This sometimes distracts from an important safety message that would have otherwise stood firm on its own merit.

This book goes a lot further than previous books, in that it looks at incidents outside the process industry, and explores the failings of all elements of process safety. I purchased two Fire Department Incident Safety officers manual for myself and a friend. When reading the manual I found that both books had several chapters missing.When I contacted Amazon they quickly sent me new manuals with no charge and pre-paid return labels to cover the return of the defective manuals. I found this book very informative, ignore Safety-Command's review. I attended David Dodson's 16 hour class during the FDIC conference in Indianapolis in 2005. The class was an overview of the book and the more important skill of learning to read smoke. Mr. Dodson also taught an Art of Reading Smoke class that was attended by over 1,000 people. This book is the basis for the FDSOA certification test. It is a must read for any Department who has not established procedures for implementing a scene safety officer. Read and learn. The book is simplified to better understand and function in the role of a Safety Officer.
