Lightdale, M.D., FACG Introduction The management of esophageal cancer presents some of the greatest challenges in the current practice of gastroenterology. Highly curable in its earliest stages, esophageal cancer is treatable but is usually fatal when more advanced (1, 2, 3). There is, therefore, an intense interest in methods for prevention and early diagnosis. Because outcome is so strongly associated with stage (4, 5), a new emphasis has been placed on the clinical staging of esophageal cancer before considering the application of treatment options with an intent to cure, to prolong survival, or to palliate symptoms. In the United States, most patients currently present with advanced stage disease. Extractions: GASTROENTEROLOGY GROUP OF NAPLES 1064 Goodlette Road Telephone: 649-1186 Perry Gotsis MD Raymond Phillips MD Susan Liberski MD Irritable bowel syndrome (IBS) is a common disorder of the intestines that leads to crampy pain, gassiness, bloating, and changes in bowel habits. Some people with IBS have constipation (difficult or infrequent bowel movements); others have diarrhea (frequent loose stools, often with an urgent need to move the bowels); and some people experience both. Sometimes the person with IBS has a crampy urge to move the bowels but cannot do so. What Causes IBS? The colon, which is about 6 feet long, connects the small intestine with the rectum and anus. The major function of the colon is to absorb water and salts from digestive prod ucts that enter from the small intestine. Two quarts of liquid matter enter the colon from the small intestine each day. This material may remain there for several days until most of the fluid and salts are absorbed into the body. The stool then passes through the colon by a pattern of movements to the left side of the colon, where it is stored until a bowel movement occurs. Colon motility (contraction of intestinal muscles and movement of its contents) is controlled by nerves and hormones and by electrical activity in the colon muscle. The electrical activity serves as a "pacemaker", similar to the mechanism that controls heart function. Movements of the colon propel the contents slowly back and forth but mainly to ward the rectum. A few times each day strong muscle contractions move down the colon pushing fecal material ahead of them. Some of these strong contractions result in a bowel movement.

Extractions: General Features History . Patients often describe a long history of chronic or intermittent diarrhea, which usually starts before age 50 and is exacerbated by anxiety or stress. Diarrhea is often worse in the morning and after meals. These patients often complain of a sensation of incomplete evacuation, distention, passage of mucus, or associated abdominal, pelvic, and back pain. Pain is relieved by evacuation. Alternating constipation, diarrhea, and bloating is typical. Diarrhea is not bloody unless accompanied by an anorectal lesion such as hemorrhoids or a fissure. Systemic symptoms or weight loss are not present. Nutritional status is not compromised. Proposed criteria for diagnosis of IBS are found in Box 5-1 Etiology . The following factors are believed to play a mechanistic role in IBS and more than one may operate in a given individual: (1) abnormal motility, (2) abnormal visceral perception, (3) psychologic factors, (4) luminal compounds (lactose, short chain fatty acids, food allergens, and bile acids), which may irritate the bowel.

Extractions: The Division of Gastroenterology at Dalhousie University currently comprises 10 physicians, 1 pathologist cross-appointed to the Division, and a basic scientist cross-appointed between the Division and the Department of Pharmacology. The Division functions in a multidisciplinary role in close cooperation with colleagues in the Department of Surgery and the Department of Nursing. We currently have two full time nurses working in Extended Role Nurse positions. One of the nurses is primarily involved with hepatitis and the other is assigned to the developing program in inflammatory bowel disease. All Division members have academic appointments at Dalhousie University. Physician descriptions vary but all Division members have a portion of time allocated to research and education. Two members of the Division, Dr. Sander van Zanten and Dr. Neena Abraham have 75% research positions. The Division provides secondary and tertiary care to patients with gastrointestinal diseases in both Nova Scotia and the Atlantic Provinces at large. The Division provides expertise in liver disease for the entire Atlantic Region and it is the coordinating centre for liver transplantation for patients from this area.

Extractions: Medstudents' Homepage Colonic diverticuli are mucosal outpouchings that usually appear with aging(50% of people beyond 60 will develop it),mainly in the left colon,leading to what we call diverticular disease. Formerly, this condition was largely misanderstood, since only the inflamation of these structures were taught to cause clinical manifestations( diverticulitis), something like a "left-sided acute appendicitis". Nowadays, we do already know that diverticula, even in the abscence of inflamation may cause pain with a cramping pattern in the lower abdomen that may sometimes be continual. However, most of people who has colonic diverticula will never develop any clinical manifestation. In these cases the diverticuli are found in necropsy or radiologic studies for other reasons. Recently, there has been an increase in the incidence of divertilitis, but this was atributted to the aging of the population. In general, there is an hipertrophy in the musculature of diseased segments. The longitudinal taenias are also proeminent, and little dilations of 0.5-1.0cm can be seen in their margins. The perforation of the diveticula initiate the inflamatory process. Iniatially, inflamation spreads locally to pericolic fat, resulting in peridiverticulitis. Lately, dissemination to peritoneum is the rule, leading to peritonitis and maybe acute abdomen or abscess or fistulas, as we will see in greater details later. With time fibrotic stricture of the colon may develop.
