P ANIC D ISORDER IN C HILDREN AND A DOLESCENTS No. 50 Panic disorder is a common and treatable disorder. Children and adolescents with panic disorder have unexpected and repeated periods of intense fear or discomfort, along with other symptoms such as a racing heartbeat or feeling short of breath. These periods are called "panic attacks" and last minutes to hours. Panic attacks frequently develop without warning. Symptoms of a panic attack include:
  • Intense fearfulness (a sense that something terrible is happening)
  • Racing or pounding heartbeat
  • Dizziness or lightheadedness
  • Shortness of breath or a feeling of being smothered
  • Trembling or shaking
  • Sense of unreality
  • Fear of dying, losing control, or losing your mind
More than 3 million Americans will experience panic disorder during their lifetime. Panic disorder often begins during adolescence, although it may start during childhood, and sometimes runs in families. If not recognized and treated, panic disorder and its complications can be devastating. Panic attacks can interfere with a child's or adolescent's relationships, schoolwork, and normal development. Children and adolescents with panic disorder may begin to feel anxious most of the time, even when they are not having panic attacks. Some begin to avoid situations where they fear a panic attack may occur, or situations where help may not be available. For example, a child may be reluctant to go to school or be separated from his or her parents. In severe cases, the child or adolescent may be afraid to leave home. This pattern of avoiding certain places or situations is called "agoraphobia." Some children and adolescents with panic disorder can develop severe depression and may be at risk of suicidal behavior. As an attempt to decrease anxiety, some adolescents with panic disorder will use alcohol or drugs.

Paul Newhouse, M.D., Director The research of the Clinical Neuroscience Research Unit is focused on studies that investigate the mechanisms, treatment, and amelioration of complex neuropsychiatric disorders. Disorders of particular interest include those that have a behavioral and cognitive component. We are particularly interested in neurodegenerative disorders such as Alzheimer's disease and Parkinson's disease. Our laboratory has particular expertise in understanding the role of central nicotinic receptors in human cognition and motor functioning. We have performed or are performing studies on CNS nicotinic receptor function and Alzheimer's disease , Parkinson's disease, and Attention Deficit-Hyperactivity Disorder We are also studying CNS function after menopause . We are particularly interested in examining the role of estrogen and central cholinergic function on cognition and behavior after menopause. We are also actively studying how estrogen may interact with catecholamines such as serotonin and norepinephrine in regulating mood and cognitive function after menopause. We also conduct treatment studies of new pharmaceutical agents in Alzheimer's disease, both for cognition and behavior, in ambulatory and long-term care settings. In addition, we also perform studies of the treatment of depression, post-traumatic stress disorder and anxiety disorders in young and elderly populations.

    Panic Attack A Panic Attack is defined as the abrupt onset of an episode of intense fear or discomfort, which peaks in approximately 10 minutes, and includes at least four of the following symptoms:
    • a feeling of imminent danger or doom; the need to escape; palpitations; sweating; trembling; shortness of breath or a smothering feeling; a feeling of choking; chest pain or discomfort; nausea or abdominal discomfort; dizziness or lightheadedness; a sense of things being unreal, depersonalization; a fear of losing control or "going crazy"; a fear of dying; tingling sensations; chills or hot flushes.
    There are three types of Panic Attacks:
    1. Unexpected - the attack "comes out of the blue" without warning and for no discernable reason.
    2. Situational - situations in which an individual always has an attack, for example, upon entering a tunnel.
    3. Situationally Predisoposed - situations in which an individual is likely to have a Panic Attack, but does not always have one. An example of this would be an individual who sometimes has attacks while driving.

    Agoraphobia
    Agoraphobia often, but not always, coincides with Panic Disorder. Agoraphobia is characterized by a fear of having a panic attack in a place from which escape is difficult. Many sufferers refuse to leave their homes, often for years at a time. Others develop a fixed route, or territory, from which they cannot deviate, for example the route between home and work. It becomes impossible for these people to travel beyond what they consider to be their safety zones without suffering severe anxiety.

    Panic Disorder: Panic Attacks and Agoraphobia What is panic disorder?
    Panic disorder is a common condition in which a person has episodes of intense fear or anxiety that occur suddenly (often without warning). These episodescalled panic attackscan last from minutes to hours. They may occur only once in a while, or they may occur quite frequently. The cause, or "trigger," for these attacks may not be obvious. What happens during a panic attack? Panic attacks are associated with physical symptoms that include the following:
    • Shaking
    • Feeling that your heart is pounding or racing
    • Sweating
    • Chest pain
    • Shortness of breath
    • Feeling that you are choking
    • Nausea
    • Cramping
    • Dizziness
    • Out-of-body feeling
    • Tingling or numb feeling in your hands
    • Chills or hot flashes
    A person may also have an extreme fear of losing control, going crazy or dying during a panic attack. It is very rare for a person to have all of these symptoms at once. However, the presence of at least 4 symptoms strongly suggests that a person has panic disorder. Many of the symptoms that occur during a panic attack are the same as the symptoms of diseases of the heart, lungs, intestines or nervous system. The similarities between panic disorder and other diseases may add to the person's fear and anxiety during and after a panic attack.
