CANPAP is a multi-centred randomized trial designed to explore a new non-pharmacologic therapy (Continuous Positive Airway Pressure - CPAP) for patients with Congestive Heart Failure and Central Sleep Apnea.
The CANPAP trial is currently recruiting in many large metropolitan cities in Canada. The trial began in January 1999 at six clinical sites in Canada. An additional four sites have since joined the trial. The clinical sites participating in the CANPAP trial are as follows:

First described in 1965, sleep apnea is a breathing disorder characterized by brief interruptions of breathing during sleep. It owes its name to a Greek word, apnea, meaning "want of breath." Sleep apnea afflicts more than 12 million people in the United States. It takes its name from the Greek word apnea, which means "without breath." People with sleep apnea stop breathing repeatedly during their sleep, often for a minute or longer, and as many as hundreds of times during a single night. Sleep apnea can be caused by complete obstruction of the airway (obstructive apnea) or partial obstruction (obstructive hypopnea; hypopnea is slow, shallow breathing), both of which can disrupt sleep. There are three types of sleep apnea — obstructive, central, and mixed. Of these, obstructive sleep apnea (OSA) is the most common. OSA occurs in approximately 2% of women and 4% of men over the age of 35. Physiology and Causes The cause of OSA is unclear. The site of obstruction in most patients is the soft palate, extending to the region at the base of the tongue. The body has no rigid structure, such as cartilage or bone, in this area to keep the airway open. During the day, muscles in the region keep the passage open. As a person with OSA falls asleep, these muscles relax to a point where the airway collapses and becomes obstructed. When the airway closes, breathing stops, and the sleeper awakens to open the airway. The arousal from sleep usually lasts only a few seconds, but brief arousals disrupt continuous sleep and prevent the person from reaching the deep stage of rapid eye movement (REM) sleep, which the body needs to rest and replenish its strength. Once normal breathing is restored, the person falls back to sleep, only to repeat the cycle throughout the night.

People most likely to have or develop sleep apnea are those who snore loudly, are overweight, or have some physical abnormality in the nose, throat or other parts of the upper airway. Sleep apnea seems to run in some families suggesting a possible genetic basis. Sleep Apnea is treatable through a variety of methods, both surgical and non-surgical. The specific therapy for sleep apnea is tailored to the individual patient based on medical history and the results of a diagnostic sleep study. Some people may need additional testing called a Multiple Sleep Latency Test (MSLT). This test is used to assess and evaluate daytime sleepiness. The test is a series of naps administered at two-hour intervals throughout the day.

O bstructive sleep apnea is a disorder in which complete or partial obstruction of the airway during sleep causes loud snoring, oxyhemoglobin desaturations and frequent arousals. As a result, affected persons have unrestful sleep and excessive daytime sleepiness. The disorder is associated with hypertension, impotence and emotional problems. Because obstructive sleep apnea often occurs in obese persons with comorbid conditions, its individual contribution to health problems is difficult to discern. The disorder has, however, been linked to angina, nocturnal cardiac arrhythmias, myocardial infarction, stroke and motor vehicle crashes. Even though obstructive sleep apnea is fairly common, it often remains undiagnosed in primary care practice. Because the disorder is associated with significant morbidity and even some mortality, family physicians need to be familiar with its clinical presentation and treatment. Pathophysiology Obstructive sleep apnea is caused by repetitive upper airway obstruction during sleep as a result of narrowing of the respiratory passages.
