Oral candidiasis, which is also called thrush, is a fungal infection of the mouth and/or throat. Candidiasis of the throat is called esophageal candidiaisis (see chart While oral candidiasis can sometimes occur without symptoms, the most common symptoms are discomfort and burning of the mouth and throat and an altered sense of taste (often described as "bad"). Creamy white or yellowish spots on the mouth and throat that can be removed by light scraping are also common. These may be accompanied by cracking, redness, soreness and swelling at the corners of the mouth. A bad case can include mouth sores. Oral candidiasis is rare if CD4+ cell counts are above 500. Outbreaks are more common as the count drops to 100. Oral candidiasis may be more difficult to treat when CD4+ cell counts fall below 50. Table of Contents Cause

Getting Rid of Yeast Infections
by Judith Levine Willis It's an itchy feeling you might hardly notice at first. Maybe, you muse, it's just that your jeans are too tight. Actually, tight jeans may have something to do with it. But if the itch keeps getting itchier, even when your jeans have been off for awhile, then there's something else involved. That something else could very well be a fungus whose technical name is Candida, and which causes what is often called a "yeast" infection. Such infections are most common in teenage girls and women aged 16 to 35, although they can occur in girls as young as 10 or 11 and in older women (and less often, in men and boys as well). You do not have to be sexually active to get a yeast infection. The Food and Drug Administration now allows medicines that used to be prescription-only to be sold without a prescription to treat vaginal yeast infections that keep coming back. But before you run out and buy one, if you've never been treated for a yeast infection you should see a doctor. Your doctor may advise you to use one of the over-the-counter products or may prescribe a drug called Diflucan (fluconazole). FDA recently approved the drug, a tablet taken by mouth, for clearing up yeast infections with just one dose.

    Clinical Features Oropharyngeal infection (OPC): white mucosal patches. Vulvovaginal infection (VVC): pruritus, vulval erythema, with or without discharge. Systemic infection usually presents as fever and chills unresponsive to antibacterial therapy. May manifest as renal or hepatosplenic infection, meningitis, endophthalmitis, endocarditis, osteomyelitis and/or arthritis. Etiologic Agent Candida albicans and C. glabrata . Less commonly, C.tropicalis C. parapsilosis , and C. krusei . Rarely, other Candida species. Reservoir Forms part of the normal microbial flora of the mouth and gastrointestinal tract. Incidence Fourth most common cause of nosocomial bloodstream infections. Incidence is 8 cases per 100,000 in the general population. Higher incidence among neonates and African-Americans. OPC used to be a common opportunistic infection in HIV-infected persons (prior to the introduction of HAART). Sequelae None with appropriate antifungal therapy. Mortality rate is almost 40% with bloodstream and disseminated infection.

    Clotrimazole comes in lozenge, topical cream and solution formulas, as well as over-the-counter vaginal cream and tablet form. Clotrimazole is the most common topical treatment for thrush. A 1% solution is applied twice daily. Clotrimazole comes in 10 mg lozenges that are held in the mouth until they dissolve, 5 times daily. For vaginal fungal infection, 100 mg tablets daily for 7 days or 200 mg for 3 days (non-pregnant only) are prescribed. If the infection is uncomplicated, you may be treated with one 500 mg dose, or 5 g of 1% vaginal cream daily for 7-14 days. Vaginal preparations should be used at bedtime for maximum contact with mucous membranes. If you miss a dose, do not double doses. How long it may take to work
    You should experience a decrease in skin irritation and discomfort, followed by a clearing of the infection. Length of time to improve depends on the organism and site of infection.

    Nationwide, bloodstream infections with Candida species have increased by 219-487% between 1980-1989, and these organisms now account for 10% of all nosocomial bloodstream isolates. This incidence equals that of Escherichia coli and surpasses Klebsiella species. In addition to hematogenously disseminated candidal infections, mucocutaneous candidal infections are becoming increasingly problematic, especially in patients with the acquired immunodeficiency syndrome (AIDS); 80% of these patients have candidal infections. Adherence to epithelial cells is the first step in colonization by Candida and subsequent establishment of mucocutaneous infection. Similarly, adherence to intravascular structures is considered to be a critical step in the egress of the blood-borne fungi from the intravascular compartment, as they hematogenously infect target organs. Because the mortality rates associated with candidal infections remain disturbingly high despite the presence of antifungal agents with excellent in vitro activity against Candida , optimal therapy requires strategies to increase host resistance to candidal infection (such as blocking adherence), combined with the use of antifungal agents.     Candida / Lavt Blodsukker. Candida er en svamp som vi alle har i tyktarmen, men som under gode vækstbetingelser breder sig og belaster hele vort immunforsvar.
