COSTOCHONDRITIS
OVERVIEW:
DIAGNOSIS:
The patient with costochondritis will have chest pain. It can be either to the left or right of the sternum. Chest pain is a symptom that we should not ignore, as it may indicate a serious problem with our heart or lungs. The doctor will perform a careful history and physical exam, and possibly order x-rays or blood tests to rule out any serious problem. If costochondritis is present, the doctor will discover tenderness over the joints and ribs of the chest wall. The diagnostic studies will also be negative, ruling out serious abnormalities of the lungs and heart. Unfortunately, costochondritis itself will not show up on any x-ray, but in extreme cases a bone scan may be positive.

COSTOCHONDRITIS
Costochondritis is an inflammation of the ribs, usually where they attach to the breastbone or sternum. A person with costochondritis may have pain in the front of the rib cage that is dull and aching. Sometimes there is redness and swelling of the skin around the sternum. The pain gets worse if pressure is put on the sore ribs. It is very important to have all pain in and around the chest evaluated by your healthcare provider. Costochondritis is not life-threatening, but a heart attack is. Sometimes an injury to the chest wall will cause costochondritis. However, many times it is not known why the pain occurs. TREATMENT FOR COSTOCHONDRITIS INCLUDES: * anti-inflammatory medication like aspirin or ibuprofen * ice packs over the sore areas * avoidance of movements and activities that increase the pain * rest until movement becomes comfortable and * exercise, using slow stretching to avoid re-injury MEDICATION CAUTIONS: Do NOT give aspirin (acetylsalicylic acid) or aspirin-containing products to anyone age 18 years or younger because of the risk of a serious illness called Reye’s syndrome. Acetaminophen should not be taken by anyone with known liver or kidney disease. Do not drink alcohol when taking more than 1 dose of acetaminophen.

Post Followup Ask the Doctor Posted by Jestduckie on January 19, 1999 at 22:53:55: Could you explain to me why costochondritis would cause me to have charley horse type of cramps on either side of my chest? I have had these cramps not at the same time but on both sides of my chest. These are scary to me. I am wondering if they can affect the heart since the heart is a muscle? When I am sleeping they wake me up and at times so bad that it like takes my lung along with it and makes it so I can't breathe normally without EXTREME pain until the cramp goes away? Thank you.

Post Followup Ask the Doctor Posted by lupusDoc on January 25, 1999 at 23:05:47: In Reply to: Costochondritis posted by Jestduckie on January 19, 1999 at 22:53:55: Hello Jestduckie, We asked the doctor your questions, and this was his reply: Since I haven't examined you, and am not your doctor, what I have to say is only *guess-work* - please keep that in mind. Costochondritis is inflammation in the costal cartilage. Anytime there are an area of pain in the body, the surrounding areas tend to *guard* against movement of that area. In your case, your intercostal muscles may be spending the day tensed up, to keep movement at a minimum in your chest. Very often when muscles are used in a way that is not natural, they react by having spasms. This may be what's occuring. Your intercostal muscles and your heart, are two different types of muscle. What affects one does not have to (and often doesn't) affect the other. So if your spasms are from overuse, your heart will not be affected at all. You need to have this all checked out by your doctor. If it is as I described, then there are treatments and therapy that can help relax those muscles, so that they don't spasm. Keep in mind though, that there are other possible reasons for the spasms, which is why your doctor needs to examine you.
