Most people have in their colons small pouches that bulge outward through weak spots, like an inner tube that pokes through weak places in a tire. Each pouch is called a diverticulum. Pouches are diverticula. The condition of having diverticula is called diverticulosis. About half of all Americans age 60 to 80, and almost everyone over age 80, have diverticulosis. When the pouches become infected or inflamed, the condition is called diverticulitis. This happens in 10 to 25 percent of people with diverticulosis. The Normal Colon
To understand diverticulosis, it is first necessary to know a bit about the anatomy and function of the intestine. Food is digested and nutrition absorbed in the 20 feet of small intestine. This is the long, thin segment of bowel that begins at the stomach and ends in the right lower abdomen. After the digestive process is complete, the liquid waste enters the 5 feet of large intestine, or colon, which ends in the rectum. Just above the rectum is the sigmoid (s-shaped) part of the colon where diverticulosis usually occurs. About two gallons of liquid stool enters the right colon each day where excess water is purified and recycled back into the blood stream. The remaining solid waste, or stool, eventually enters the rectum where the waste is stored until it is convenient to have a bowel movement.
What is Diverticulosis?
Diverticulosis is a condition, rather than a disease. It affects the large intestine, or colon. A normal colon is strong and relatively smooth. A colon affected by diverticulosis has weak spots in the walls. These defects allow the development of balloon-like sacs or outpouches - much like a bubble forming on a worn inner tube. These hollow pouches, - about the size of a large pea - called diverticulae, occur when the inner intestinal lining has pushed through weakened areas of the colon wall. A single pouch is called a diverticula. Although diverticulosis can occur anywhere in the colon, 80% are found in the lower left side - called the sigmoid colon - because that is where the colon is the narrowest and the inner pressure the highest.

Diverticula Diverticula form when a small spot of the inside layer of the intestine pouches out through the muscular layer. The area of the colon most commonly affected is the sigmoid colon (the colon that is the last section before stool leaves your body) and the descending colon but can form anywhere in the large intestine. Once these pouches form they remain for life but frequently cause no problems. Diverticulitis If the diverticula become inflamed they cause the condition known as diverticulitis. If this happens, usually one or all of the following symptoms occur.
  • Lower abdominal discomfort / pain
  • Change in bowel habits such as diarrhea or constipation
  • Bloating or flatulence = gas
  • Pain in the lower left area of the abdomen. The symptoms usually last for several days and may have to be treated with antibiotics and pain medication if severe enough. Treatment Antibiotics are used to fight the infection that may be in the inflamed pouch and the pain medication is to provide comfort until the episode stops.

    Etiologic factors in diverticulosis include increased luminal pressure, lack of dietary fiber, weakening of the bowel wall and hypersegmentation. Diverticulosis often presents with abdominal pain, excessive flatulence, and irregular defecation. Similar findings occur in patients who have irritable bowel syndrome (IBS) and colorectal cancer. The best diagnostic test for diverticulosis is the barium enema (BaE), although concomitant lesions, such as colorectal cancer are missed in up to 50% of patients who have BaE alone. Diverticulosis can be visualized during sigmoidoscopy and colonoscopy, however there is no role for BaE nor endoscopy in acute diverticulitis. The natural history of diverticulosis is that 70% remain asymptomatic, 5-10% are complicated by bleeding and 10-15% develop diverticulitis. Patients who experience painful diverticulosis may be manage with bulk laxatives, increased dietary fiber and antispasmodics. In diverticulitis the majority of patients present with uncomplicated disease and can be managed with antibiotics, fluids and analgesics. Following resolution of symptoms, these patients require workup to exclude colonic cancer as an etiologic agent in their disease. Most patients remain asymptomatic after the first attack, but those with a second attack often need surgery to prevent further complications, morbidity and mortality. Of those patients with complicated disease, they are managed with an appropriate surgical procedure based on the stage of disease.

    To understand diverticulosis, it is helpful to know about the anatomy and function of the intestine. The small intestine is the long, thin segment of bowel that begins at the stomach and ends at the large intestine or colon. The colon starts in the right lower abdomen and forms a large question mark across the entire abdomen ending in the rectum. Just above the rectum is the sigmoid (S-shaped) part of the colon. Liquid stool enters the right colon and, as it moves through the colon, is dehydrated so that a formed stool eventually enters the rectum. The Sigmoid Colon
    The sigmoid colon has the specialized job of contracting vigorously to maintain a high pressure. This action regulates the movement of stool into the rectum. And since the sigmoid is a high pressure part of the colon, it is here that most diverticuli occur. Diverticulosis
    Diverticulosis means that there are pockets or projections extending out from the colon. These occur very gradually over a long period of time. They occur at weak points in the bowel wall. The pockets develop because of the pressure exerted within by the contracting colon. As noted earlier, since the sigmoid colon has the highest pressure in the colon, it is here that most diverticuli occur. Because of these balloon-like projections, the sigmoid often becomes thickened and narrowed. When this happens, significant changes in the bowel function can occur, such as discomfort, diarrhea, and/or constipation.

    The American Society of Colon and Rectal Surgeons It should be recognized that these guidelines should not be deemed inclusive of all proper methods of care or exclusive of methods of care reasonably directed to obtaining the same results. The ultimate judgment regarding the propriety of any specific procedure must be made by the physician in light of all of the circumstances presented by the individual patient. Acute sigmoid diverticulitis results from inflammation of a diverticulum that can progress to perforation and abscess formation. Acute sigmoid diverticulitis is usually manifested by fever, leukocytosis, left lower quadrant abdominal pain, and at times, a left lower quadrant mass. The differential diagnosis of acute diverticulitis includes inflammatory bowel disease, irritable bowel syndrome, ischemic colitis, colon cancer, and gynecologic and urologic diseases. The initial evaluation of the patient with suspected acute diverticulitis should generally include a physical examination, complete blood count, urinalysis, and when clinically indicated, plain x-rays of the abdomen. Depending on availability, feasibility, and physician preference, computed tomography scan, contrast enema, or abdominal ultrasound may be used to document and assess the extent of the disease. Treatment of uncomplicated acute diverticulitis usually involves bowel rest and broad-spectrum antibiotics. Depending on clinical presentation, selected patients may be treated as outpatients. After the patient recovers from an initial episode of diverticulitis, the colon should be evaluated by either sigmoidoscopy and barium enema or colonoscopy. Depending on the clinical situation, patients who do not respond to medical therapy should undergo computed tomography or ultrasonography or surgery. Selected abscesses detected by computed tomography scan or ultrasonography may be drained percutaneously. Elective resection should be considered after one or two well-documented attacks of diverticulitis, depending on severity of the attack and age and medical fitness of the patient.
