Wat is het? Erythema multiforme (EM) is een huidaandoening die gepaard gaat met rode vlekken en/of blaren op de huid. Het ontstaat meestal na een (virus) infectie of na medicijngebruik. Zoals de naam al aangeeft kan EM vele vormen aannemen. Om toch een indeling te kunnen maken worden in de dermatologie 3 groepen van EM onderscheiden.
  • Erythema multiforme minor

  • Deze vorm komt verreweg het meeste voor en bestaat uit rode tot paarse vlekken die vooral aan de armen en benen, en in meer uitgebreide gevallen ook op de romp gelocaliseerd zijn. In de meest typische gevallen is er sprake van "schietschijf"-plekken: vlekken uit enkele concentrische ringen opgebouwd, vaak met een blaasje in het centrum. Er zijn ook vaak rode vlekken en blaasjes op het mondslijmvlies te zien.
  • Erythema multiforme major

  • Behalve huidafwijkingen zijn er bij de "major" variant van EM ook uitgebreide erosies aan de slijmvliezen. De mond, ogen, vagina en anus kunnen aangedaan zijn.
    De patient is vaak ziek, heeft veel pijn en kan moeilijk eten door de open plekken.
    Opname in het ziekenhuis is aangewezen.

Erythema multiforme is a skin disease that causes lesions and redness around the lesions. Description Erythema multiforme appears on the skin and the mucous membranes (the lining of the mouth, digestive tract, vagina, and other organs). Large, symmetrical red blotches appear all over the skin in a circular pattern. On mucous membranes, it begins as blisters and progresses to ulcers. A more advanced form, called Stevens-Johnson syndrome, can be severe and even fatal. Causes and symptoms Erythema multiforme has many causes, most commonly are drugs. Penicillin, sulfonamides , certain epilepsy drugs, aspirin , and acetaminophen are the most likely medication-induced causes. Erythema multiforme can also be caused by certain diseases. Herpes virus and mycoplasma pneumonia are likely infectious causes. Diagnosis The appearance of the rash is sufficiently unique to identify it on sight. Having identified it, the physician will determine the underlying cause. Treatment Erythema multiforme is inadvertently treated when the causative agent, whether it be a drug or a disease, is treated. In severe cases, cortisone-like medication is often used along with general supportive measures and prevention of infection.

ERYTHEMA MULTIFORME - A rash that results from an allergic response, most often secondary to a drug. The rash is described as pink-red macules (flat), that may have clear centers (iris lesions ) or appear as a dusky violet color. Erythema multiforme most often results from reactions to sulfa drugs , penicillin's, phenytoin barbiturates , phenolphthalein, and carbamazepine. In some cases, erythema multiforme can result secondary to a Herpes simplex infection or Mycoplasma infection.

  • an acute, sometimes recurrent, inflammatory disease of the skin and mucous membranes
Incidence
  • can occur at any age - most common in childhood
Clinical features
  • acute, vasculitic hypersensitivity reaction
  • target shaped (annular) lesions occurring 1-2 weeks after prodromal upper RTI
    • change appearance with time (hence called multiforme)
  • cutaneous
  • mucous membranes of mouth, eyes and genitals may be affected
  • spectrum of disease :
    • EM minor
      • symmetric erythematous macules or papules which become anular target lesions
      • primarily on upper extremities and trunk
      • may develop intradermal haemorrhage, may show as petechiae
      • oral lesions in 25% patients - erythematous macules with vesiculobullae which become necrotic with pseudomembranous surface
      • usually self-limiting
      • conservative treatment, cool compresses, anti-histamines, mouthwashes
    • EM major (Stevens-Johnson syndrome)
      • blistering of at least 2 mucosal surfaces accompanies cutaneous lesions
      • purulent conjunctivitis and uveitis are usual
      • haemorrhage often occurs
      • internal organs eg kidneys and lungs sometimes affected
      • treat with intensive care admission, topical antibiotics (mupirocin to skin and nose, chloramphenicol or erythromycin to eyes) and if cutaneous infection, UTI or suspected bacteraemia give systemic antibiotics (infection is leading cause of death)

Erythema multiforme appears as a rash and large ulcers or blisters in the mouth, the genital area, and the eye. It can also include hepatitis, nephritis, gastrointestinal bleeding, pneumonia, painful and swollen joints, fever, and muscle pain. It is sometimes fatal.

ERYTHEMA MULTIFORME Apoptotic keratinocytes in the epidermis adjacent to a subepidermal vesicle that contains 'gossamer threads' is characteristic of some examples of erythema multiforme. Superficial and mid dermal perivascular lymphocytic infiltrates are common. The keratinocytes are larger than normal. Liquefaction change at the dermoepidermal junction is best seen in early cases. There are other variants of erythema multiforme that are not shown here. Apoptosis above the basal layer and keratinocyte enlargement are also seen in viral dermatitis such as herpes. It is interesting that HSV DNA has been detected by polymerase chain reaction in biopsies of HSV related erythema multiforme.

Behandling Kontakt lege. Eventuelle medisiner du bruker, må vurderes på nytt, og legen vil undersøke om du har en infeksjon som trenger behandling. I et mildt tilfelle er det ikke nødvendig med behandling utover dette; tilstanden vil gå tilbake av seg selv i løpet av 2-3 uker.

Definition
Acute, vesico-bullous disorder with numerous manifestations on the skin, mucous membranes and, occasionally, internal organs. Pathogenesis
Pathogenesis is unknown, but erythema multiforme is generally regarded as a hypersensitivity reaction triggered by drugs (sulphonamides, phenytoin), infections (herpes simplex, Mycoplasma ) or exposure to toxic substances. Complications include conjunctivitis, uveitis, corneal scarring, fluid loss, infections, anaemia, and oesophageal strictures. Diagnostic criteria
  • Erythema multiforme minor: Prodromal symptoms are generally absent. Symmetric crops of skin lesions of diverse morphology, primarily on the extensor surfaces of the arms and legs (often including soles and palms) with relative sparing of the mucous membranes and the trunk. Iris or target lesions (pathognomonic for erythema multiforme) consisting of a dark centre, an inner pale ring and an erythematous outer border. Erythematous macules that evolve into papules, vesicles, bullae, urticarial plaques or patches of confluent erythema. The centre of the lesion may be vesicular, purpuric or necrotic.
