Erythema multiforme is a skin disease that causes lesions and redness around the lesions.
Description
Erythema multiforme appears on the skin and the mucous membranes (the lining of the mouth, digestive tract, vagina, and other organs). Large, symmetrical red blotches appear all over the skin in a circular pattern. On mucous membranes, it begins as blisters and progresses to ulcers. A more advanced form, called Stevens-Johnson syndrome, can be severe and even fatal.
Causes and symptoms
Erythema multiforme has many causes, most commonly are drugs. Penicillin, sulfonamides , certain epilepsy drugs, aspirin , and acetaminophen are the most likely medication-induced causes. Erythema multiforme can also be caused by certain diseases. Herpes virus and mycoplasma pneumonia are likely infectious causes.
Diagnosis
The appearance of the rash is sufficiently unique to identify it on sight. Having identified it, the physician will determine the underlying cause.
Treatment
Erythema multiforme is inadvertently treated when the causative agent, whether it be a drug or a disease, is treated. In severe cases, cortisone-like medication is often used along with general supportive measures and prevention of infection.

Erythema Erythema is an abnormal redness of the skin due to dialation of the superficial capillaries of the skin causing inflammation . It can result from many different causes, diseases of the skin and some systemic diseases. Simple erythema can be caused by cold, heat (as in contact with a heating pad or hot water bottle), or solar radiation . In its more severe forms it generally presents in one of two ways: erythema multiforme or erythema nodosum. Erythema Multiforme
Erythema multiforme is a reddening, swelling, possibly even blistering of the skin that comes in multiple forms. It is an allergic (hypersensitivity) inflammatory reaction to a stimulus (allergen). This type of erythema has innumerable causes that can be loosely categorized as infections, medications, blood-vessel disease, and neoplasms. In the United States, the majority of erythema multiforme cases are caused by herpes virus infections.

Report Identifies Case of Bupropion-Induced Erythema Multiforme
ROCHESTER, MINN. As bupropion is prescribed increasingly for smoking cessation and not just for depression, physicians need to aggressively follow up complaints of rashes that could be caused by use of the medicine, says a report in the June issue of Mayo Clinic Proceedings. The case report is what the authors believe is the first case of erythema multiforme, an allergic response, after a 31-year-old woman took sustained-release bupropion for treatment of depression. The report emphasizes that physicians should immediately discontinue the medication as soon as erythema multiforme is suspected and watch closely for the emergence of potentially life-threatening dermatologic conditions. In the case of the female patient, she had taken bupropion for 24 days before it was discontinued. Her skin lesions and itching resolved over the next few weeks, although she was left with postinflammatory hyperpigmentation, which had not fully faded after 8 months of follow up. Causes, incidence, and risk factors: Erythema multiforme is a type of hypersensitivity (allergic) reaction that occurs in response to medications, infections, or illness. Medications associated with erythema multiforme include sulfonamides, penicillins, barbiturates, and phenytoin. Associated infections include herpes simplex and mycoplasma infections.
The exact cause is unknown. The disorder is believed to involve damage to the blood vessels of the skin with subsequent damage to skin tissues. Approximately 90% of erythema multiforme cases are associated with herpes simplex or Mycoplasma infections. The disorder occurs primarily in children and young adults.
Erythema multiforme may present with a classic skin lesion with or without systemic (whole body) symptoms. In Stevens-Johnson syndrome, the systemic symptoms are severe and the lesions are extensive, involving multiple body areas (especially the mucous membranes). Stevens-Johnson syndrome is a severe and sometimes fatal form of erythema multiforme, a disorder that occurs in response to medications, illness, or infections. Erythema multiforme is believed to be an allergic reaction resulting in damage to blood vessels of the skin followed by infection of the skin tissue. More than half of the cases are caused by herpes simplex, and virtually any drug , including penicillins and barbiturates, may cause the viral infection. Erythema is characterized by patches of red, raised skin. In Stevens-Johnson syndrome, the systemic symptoms are more severe. Blisters break out along the mouth, throat, genital region, anus and eyes. Lesions are usually extensive and may cause acute damage of the mouth. Eating is often difficult, and closing the mouth may be painful. The eyes may swell and eventually seal shut. Urination may also become difficult. With proper treatment by a physician , symptoms of erythema multiforme generally resolve in two to six weeks. Stevens-Johnson syndrome, however, is associated with a high death rate.
