Gastrointestinal functional and motility disorders A functional disorder refers to a disorder or disease where the primary abnormality is an altered physiological function (the way the body works) rather than an identifiable structural or biochemical cause. It characterizes a disorder that generally can not be diagnosed in a traditional way; that is, as an inflammatory, infectious, or structural abnormality that can be seen by commonly used examination, x-ray, or laboratory test. Gastrointestinal motility is defined by the movements of the digestive system, and the transit of the contents within it. When nerves or muscles in any portion of the digestive tract do not function in a strong coordinated fashion, a person develops symptoms related to motility problems. These symptoms may range from heartburn to constipation. Other symptoms may also include abdominal distention, nausea, vomiting, and diarrhea. What is a Functional Gastrointestinal Disorder
Functional gastrointestinal (GI) disorders can affect any part of the GI tract, including the esophagus, stomach, and intestines. They are disorders of function (how the GI tract works), not structural or biochemical abnormalities. As a result, xrays, blood tests, and endoscopies can have essentially normal results. Importantly, they are not psychiatric disorders, although stress and psychological difficulties can make the functional GI symptoms worse. The most common disorders are Irritable Bowel Syndrome (altered bowel consistency combined with abdominal pain that is usually relieved with a bowel movement) and Functional Dyspepsia (ulcer-like symptoms - upper abdominal pain, feeling of indigestion). Approximately 25 million Americans have functional GI disorders. 50 - 80% of them do not consult physicians, although they may take over-the-counter medications and report significantly more job absenteeism and disability.

Most gastrointestinal disorders in children cause pain. Many, such as celiac disease (see page 536 in Chapter 110, Malabsorption Syndromes ) and lactose intolerance, (see page 535 in Chapter 110, Malabsorption Syndromes ) also cause malnutrition and diarrhea. Pain may begin suddenly and may be severe, as in acute appendicitis, (see page 547 in Chapter 112, Gastrointestinal Emergencies ) or it may be less severe and may come and go. Gastrointestinal disorders in which pain typically comes and goes include recurring abdominal pain, peptic ulcer, and Meckel's diverticulum.
Recurring Abdominal Pain
Recurring abdominal pain is three or more episodes of abdominal pain occurring over a period of at least 3 months. More than 10 percent of all school-aged children have recurring abdominal pain. It is most common between the ages of 8 and 10 and is rare in children under age 4. Recurring abdominal pain is slightly more common in girls than in boys and is fairly common among girls in early adolescence.
