    GLAUCOMA THE SNEAK THIEF OF SIGHT Glaucoma is the second leading cause of blindness in the United States. It is THE leading cause of blindness among African-Americans, and the leading cause of PREVENTABLE blindness in the U.S. Glaucoma may occur at any age but happens with increasing frequency during advancing years. Approximately 2% of the population over the age of 40 is affected. It's estimated that over 2 million Americans have glaucoma, and more than half of them are not aware they have the disease.
    THE EYE'S ANATOMY: HOW THE EYE WORKS Your eye is like a camera. Light enters the clear dome in the front of the eye (cornea), passes through the focusing part (lens), and comes to rest on the "film" or recording part (retina). The nerve attached to the eye carries these recorded pictures "back to the brain" and WE SEE. Eyes must stay inflated and round, just like a basketball. To stay inflated, the eye constantly makes fluid (aqueous humor) which circulates inside the front of the eye (anterior chamber), and leaves the eye by an intricate drain (trabecular meshwork). The balance between how much fluid is made and how much fluid leaves the eye determines how inflated the eye is (intraocular pressure). If all is working well, the fluid made inside the eye equals the amount of fluid leaving the eye, and the intraocular pressure is normal. If fluid cannot leave the eye properly, it builds up inside and the eye's pressure rises. This is the basic fact of glaucoma: high pressure inside the eye damages the eye's delicate optic nerve.

Because glaucoma usually has no obvious symptoms, you may not know you have the disease until a serious loss of vision occurs. In fact, half the people suffering from glaucoma do not realize they have the disease. An eye examination is the only way to tell if you have glaucoma. That is why you should ask your doctor about glaucoma. Who is at risk for glaucoma?
Because glaucoma is one of the most common causes of preventable blindness, everyone should be concerned about the disease. You may be at greater risk for glaucoma if you have:
  • Older than 45 years
  • A family history of glaucoma
  • High intraocular pressure (IOP), or the pressure inside the eye
  • African ancestry
  • Diabetes
  • Severe nearsightedness (myopia)
  • Used steroids or cortisone for a long time
  • Had a previous eye injury

Overview Glaucoma is a disease caused by increased intraocular pressure (IOP) resulting either from a malformation or malfunction of the eye’s drainage structures . Left untreated, an elevated IOP causes irreversible damage the optic nerve and retinal fibers resulting in a progressive, permanent loss of vision. However, early detection and treatment can slow, or even halt the progression of the disease. What causes glaucoma? The eye constantly produces aqueous , the clear fluid that fills the anterior chamber (the space between the cornea and iris ). The aqueous filters out of the anterior chamber through a complex drainage system. The delicate balance between the production and drainage of aqueous determines the eye’s intraocular pressure (IOP). Most people’s IOPs fall between 8 and 21. However, some eyes can tolerate higher pressures than others. That’s why it may be normal for one person to have a higher pressure than another. Common types of glaucoma Open Angle Open angle (also called chronic open angle or primary open angle) is the most common type of glaucoma. With this type, even though the anterior structures of the eye appear normal, aqueous fluid builds within the anterior chamber, causing the IOP to become elevated. Left untreated, this may result in permanent damage of the

Definition Nail-Patella syndrome (NPS) is a rare genetic disorder that causes abnormalities of bone, joints, fingernails and kidneys. NPS is commonly characterized by absent or under-developed kneecaps and thumbnails. It is estimated to occur in one in 50,000 newborns. Researchers at the U-M Kellogg Eye Center have found strong evidence of a link between glaucoma and NPS. A research study was initiated after Paul Lichter, M.D., noticed a glaucoma patient had no thumbnails and recalled the patient's mother, whom he had treated years earlier, also lacked thumbnails. Both patients had NPS in addition to glaucoma. Other family members were subsequently examined, and some who previously were unaware of NPS were diagnosed with the disorder. Lichter's research team examined 24 people with Nail-Patella syndrome in two families and found that more than half of them suffered from glaucoma. "As a result of these findings, we recommend people with Nail-Patella syndrome have regular ophthalmologic exams so glaucoma can be diagnosed and treated as early as possible," said Lichter, who is chairman of the University of Michigan Health System's Department of Ophthamology and director of the W.K. Kellogg Eye Center. The gene which mutates to cause NPS is located on a portion of chromosome 9 called 9q34. The U-M study provides indirect evidence that glaucoma-related genetic material is located at that same chromosomal site. It's not clear, however, whether glaucoma results from the NPS mutation or from a separate but closely linked gene.

GLAUCOMA What is glaucoma? Glaucoma is the elevation of pressure inside the eye ( intraocular pressure ) beyond a specific point at which vision is no longer possible. Glaucoma is a frequent cause of blindness in humans and in our pets. To understand glaucoma, it is necessary to understand how the fluid inside the eye normally flows and maintains normal intraocular pressure. Fluid inside the eye ( aqueous humor ) is produced behind the colored area of the eye ( iris ) in a portion of the eye called the ciliary body. This aqueous humor is made by filtering blood. The fluid flows through the dark hole in the eye ( pupil ). Finally the aqueous humor drains from the eye at the junction of the clear cornea and the colored iris ( drainage angle ) inside the eye and then the aqueous rejoins the blood. The drainage angle is a sieve-like network. This aqueous humor is made inside the eye and passes from the eye at the same rate. This results in a stable intraocular pressure of 15-25 mm of Hg. Glaucoma is the consequence of a blockage of the outflow of aqueous humor and a subsequent buildup of pressure inside the eye.
