About the Book
The second part is devoted to short stories based on dreams and hallucinations experienced whilst he was in 'Intensive Care', which in themselves reveal something of what a patient was going through. They also serve to illustrate the narrow boundary between reality and the dream world, and which whilst making for entertaining reading, could possibly be the subject of further study. The foreword to the book has been written by Dr Frank Warshawski, MD, FRCPC, Director of Intensive Care, Rockyview General Hospital, Calgary. In addition to that prestigious involvement, the Author has been fortunate in receiving permission to use other material authored by exceptionally well qualified medical professionals, for which due credit is given in the 'Acknowledgements' section of the book, enabling him to produce not only an interesting publication, but an informative and educational one. In the C. Miller-Fisher variant, ataxia as well as ophthalmoplegia (internal and external) accompany the obligatory findings of areflexia. Dysautonomia may also be present, posing an additional concern regarding tolerance of water pressure and immersion changes, blood pressure and cardiac rhythm disturbances that may be especially life-threatening in the underwater environment. Return to diving can be considered after full recovery of strength and autonomic nervous system function. Tendon-stretch reflexes may never return but would not prohibit return to diving. Data needed to decide return to diving Therapy : Plasmapheresis and/or intravenous immunoglobulin (IVIG) therapy is warranted in those cases which involve weakness progressing to the point of impairing walking or respiratory abilities. Adrenocorticosteroid therapy is not beneficial and may actually worsen the outcome.

Guillain-Barre syndrome is a neurological disorder in which the body's immune system attacks part of the peripheral nervous system. The onset can be quite sudden and unexpected. The disorder can develop over a few days, or it may take up to several weeks. A person experiences the greatest weakness within the first two weeks after symptoms appear. What are the symptoms of Guillain-Barre syndrome?
The first symptoms include varying degrees of weakness or tingling sensations in the legs, which sometimes spreads to the arms and upper body. The symptoms may increase in severity until the following does or does not occur - in these cases, the disorder is considered a medical emergency:
  • muscles cannot be used at all the patient becomes nearly paralyzed it may become difficult to breathe blood pressure and heart rate are affected
Although symptoms can become life threatening, partial recovery is possible from even the most severe cases of Guillain-Barre syndrome (however, some degree of weakness may still be present).

It is related to inflammation of multiple nerves. Causes and Risks Guillain-Barre syndrome is an acute type of nerve inflammation. The inflammation damages portions of the nerve cell, resulting in muscle weakness or paralysis . The damage usually includes loss of the myelin sheath of the nerve demyelination ) ,which slows conduction of impulses through the nerve. The damage may also include destruction of the axon part of the nerve cell (denervation), which blocks conduction through the nerve.
The exact cause of this disorder is not known. It usually follows a minor infection, usually a respiratory (lung) infection or gastrointestinal (gut) infection.

Guillain-Barre Syndrome And The Miller Fisher Variant
Christopher P. Sobczak, M.D., Safwan Jaradeh, M.D. Medical College of Wisconsin, Milwaukee, WI Discussion: Miller Fisher syndrome is a variant of Guillain-Barre Syndrome involving the triad: ataxia, ophthalmoplegia, and areflexia. The pathophysiology of the syndrome can be explained through GQ1b ganglioside autoantibodies, which are seen in 96 percent of cases of Miller Fisher Syndrome. These autoantibodies recognize epitopes expressed specifically in the nodal regions of oculomotor nerves and cerebellar neurons. Organisms such as Mycoplasma Pneumoniae and Campylobacter jejuni contain cell walls that are capable of inducing the production of these antoantibodies. Treatment is thus involved in immune modulation, through the use of IVIg, plasma exchange, and steroids. Our patient required intubation and wan treated with all three modalities over a three-month hospitalization. He eventually recovered to be discharged to long term rehabilitation. West Nile virus usually causes mild symptoms, though rarely it can cause neurologic diseases, with a fatal outcome or permanent neurologic sequelae. We describe an elderly patient with West Nile virus infection who presented with Guillain-Barre syndrome.

Guillain-Barre Syndrome
Guillain-Barre' Syndrome: causes segmental demyelination (destruction or loss of the myelin sheath of a nerve or nerves) of the peripheral nerves. Guillain-Barre syndrome is an acute, rapidly progressive, and potentially fatal form of polyneuritis (inflammation of many nerves simultaneously). It causes both the posterior (sensory) and anterior (motor) nerve roots, signs of sensory and motor losses occur simultaneously. Cause: True cause for Guillain-Barre syndrome is unknown, but it may be a cell mediated immunologic attack on peripheral nerves in response to a virus. Predisposing factors may include mild febrile illness, surgery, viral illness, Hodgkin's disease or some other cancer, or systemic lupus erythematosus. Symptoms: Along with a history of preceding febrile illness (usually a respiratory tract infection) - other signs includes: Paresthesia ( morbid or perverted sensation; and abnormal sensation, as burning, prickling, formication, etc.) Muscle weakness - usually appears in the legs first, then extends to the arms and facial nerves in 24 to 72 hours. In some patients it may develops in the arms first or in the arms and legs simultaneously. May have facial diplegia (paralysis of like parts on either side of the body).
