What is a Helicobacter Pylori? Helicobacter pylori, (pronounced HELI-co-back-ter pie-Lorrie ) also called H. pylori is a spiral shaped bacterium. It lives in the stomach and duodenum. The duodenum is the section of intestine just below stomach Helicobacter pylori has a unique way of adapting in the harsh environment of the stomach. It protects itself by covering itself with the mucus of the stomach. Once it is covered with mucus, it is able to fight the stomach acid that does reach it with an enzyme it possesses called urease. Because of its spiral shape and the way it moves, H. pylori can penetrate the protective lining of the stomach. When it penetrates the stomach, excess acid can irritate the stomach and duodenum (pronounced Doo-o-DEE-num ) eventually causing an ulcer.

Description of Helicobacter pylori Helicobacter pylori is a spiral-shaped, Gram-negative rod approximately 0.5 x 3.0 micrometers in size. The catalase-positive organism has 4-6 sheathed flagella attached to one pole which allow for motility.
History Barry Marshall and Robin Warren of Perth, Western Australia, discovered H. pylori in 1983. Originally, the organism was named Campylobacter pyloridis because it was structurally similar to other Campylobacter species, such as C. jejuni C. jejuni is a gut pathogen which has the ability to colonize the gastric mucosa. C. pyloridis was renamed Campylobacter pylori to fit in with the names of other enteric pathogens. In 1989, it was finally named Helicobacter pylori based on functional and enzymatic properties.

Biology of Helicobacter pylori Helicobacter are Gram-negative, aerobic or microaerophilic, spiral-shaped bacilli that are motile by way of flagella at one end of the cell. Closely related to Helicobacter are species of the genera Aquaspirillum Azospirillum Spirillum , and Camplyobacter . The genetics of Helicobacter pylori are complex; there are many strains of H . pylori which are distinguished by the human disease with which they are associated. Once Helicobacter pylori colonizes its host, it lives in the interface between the surface of gastric epithelial cells and the overlying mucus gel layer, often clustering at the junctions of epithelial cells. In addition H. pylori can also be found on top of the gastric epithelium in the duodenum and esophagus. It was not until 1983 that H . pylori was recognized as having any medical importance. Conclusions
The results of the Workshop were presented and discussed at the NIAID Advisory Council meeting on February 18, 2000. It was the recommendation of the Council that the Institute should not pursue the volunteer challenge model at this time. Several factors entered into this decision including: 1) the lack of a compelling need for vaccination in the U.S. where the rate of natural infection is decreasing, 2) the lack of a clear vaccination strategy for the U.S. (who would be vaccinated?), 3) the fact that volunteers would be at risk of unsuccessful antibiotic eradication of the challenge strain for an infection with serious sequelae (ulcers and cancer) that may not be clinically diagnosed for many years after the trial, 4) the availability of animal models, which have not been thoroughly investigated as models for vaccine efficacy, and 5) the ability to measure vaccine efficacy by more conventional field trials. Introduction
Bacterial and viral infections of the gastrointestinal tract can lead to diarrheal disease. Many of these pathogens are transmitted through contaminated food or water. In the United States, diarrhea is the second most common infectious illness, accounting for one out of every six (16 percent) of all infectious diseases. Data compiled by the World Health Organization indicate that diarrheal diseases account for 15 to 34 percent of all deaths in certain countries. Conservative estimates place that death toll at 4 million to 6 million per year, with most of these occurring in children of preschool age, the elderly, and the immunocomprised, all of whom are at high risk. This tremendous disease burden requires continued active research aimed at a better understanding of the pathogenic mechanisms employed by these organisms, more comprehensive and accurate surveillance, and better ways to diagnose, treat and prevent these diseases. DMID supports research in each of these areas.
