HODGKINS DISEASE
Hodgkin’s Disease is a group of cancers of the lymph nodes. The cancer is rare, but very important since it is curable in most cases. Many patients first notice their disease when to feel a swollen lymph node or develop fevers, night sweats or weight loss.
Treatment of Hodgkin’s Disease varies according to the stage of disease at diagnosis. Patients with disease limited to one area may be curable (in up to 70-80%) with radiation with or without chemotherapy. Patients with disease that has spread may still be curable (in up to 60%) with chemotherapy.
If the Hodgkins Disease fails to respond to initial treatment or recurs following treatment, consideration of high-dose transplantation therapy becomes appropriate. This is especially true if the disease returns within 12 months of initial treatment. A British National Lymphoma Trial compared transplantation at relapse with standard dose chemotherapy. More patients receiving transplantation remained in remission at 3-years compared to those patients receiving standard treatment (53% versus 10%). (Linch DC et al: Lancet 1993: 341) A French trial also suggested superiority to transplantation when one adverse prognostic factor was present (Gajewski JL et al: J Oncol 1996: 14:572)

Twenty per cent of children with lymphoma have the Hodgkin type. There may be an association with the Epstein – Barr virus infection. The histological diagnosis on biopsy is based on the finding of the Reed – Sternberg cell. Hodgkin's disease is rare under 5 years of age, the incidence peaking at about 25 years. The most common presentation (80%) is painless cervical lymphadenopathy. Lymphadenopathy at other sites is an occasional presentation. There may be multiple nonspecific signs such as fever, night sweats, malaise and weight loss. The definitive diagnosis is made by biopsy, and thereafter by staging and by imaging. Hodgkin's disease is classified as follows: Stage 1 involves one lymph node region.

Introduction
The lymphatic system includes the lymph nodes and related organs that are part of the body's immune and blood-forming systems. The lymph nodes are small, bean-shaped organs located in the neck, underarm, groin, chest, abdomen, and pelvis. Lymph nodes make and store infection-fighting white blood cells called lymphocytes. They are connected throughout the body by lymph vessels that carry lymphatic fluid containing the lymphocytes.
Cancers of the lymph system (lymphomas) are divided into Hodgkin's disease (HD) and Non-Hodgkin's lymphoma. As the cancer cells in HD grow, they can compress, invade, and destroy normal tissue and spread to other tissues either through the lymph system or the bloodstream.
The American Cancer Society has estimated that about 7,400 new cases of HD will be diagnosed in the year 2000. It tends to affect men more often than women. Hodgkin's disease can occur at any age, even in childhood, although there are two periods in which it is more common: between the ages of 15 and 24, and after age 55.

Increased Risk Of Cancer In Long-Term Hodgkin's Survivors
An international study of more than 32,000 Hodgkin's disease patients has shown that long-term survivors of the disease have double the normal risk of developing a subsequent cancer later in life. Researchers led by the National Cancer Institute measured the risk of these 'second cancers' among Hodgkin's disease survivors using data from cancer registries in the U.S., Canada, and four Nordic countries. The research tracked cancer incidence and had among the longest follow-up periods of any study to date. Of the study population, 1,111 patients were followed for 25 years or longer.

    Dear Dr. Dezube... My partner's pathology is being sent out of state because the docs here say they cannot determine if he has Hodgkins disease or Hodgkins lymphoma. I thought they were the same thing? I'm not confusing anything with non-Hodgkins lymphoma, because I specifically asked if that is what they meant, and they said no. Is Hodgkins disease different from Hodgkins lymphona? Thanks for all your great work. Brian
      Dear Brian: Hodgkin's disease is the same as Hodgkin's lymphoma. Two different names for the same disease I personally prefer the former name. As has been said, a rose by any other name would smell as sweet.

    What is Hodgkins Disease? Hodgkins disease is a type of lymphoma, a cancer of the lymph system. When lymphatic cells mutate (change) and grow unregulated by the processes that normally decide cell growth and death, they can form tumors. The lymph system is made up of thin tubes that branch out to all parts of the body. Its job is to fight infection and disease. The lymph system carries lymph, a colorless fluid containing white blood cells (called lymphocytes). Groups of bean-shaped organs called lymph nodes are located throughout the body at different sites in the lymph system. There are clusters of lymph nodes in the abdomen, pelvis, underarms, and neck. Other parts of the lymph system are the spleen, which makes lymphocytes and filters blood; the thymus, an organ under the breastbone; and the tonsils, located in the throat. Hodgkins disease most commonly affects lymph nodes, usually beginning in the neck or the area between the lungs, and behind the breastbone. It can also begin in other groups of lymph nodes, such as those under the arms, in the groin, or in the abdomen or pelvis. If Hodgkins disease spreads, spread to the spleen and liver is fairly common. Spread to other parts of the body can also occur, but is unusual.
