Childhood Hydrocephalus: Clinical Features, Treatment, and the Slit-Ventricle Syndrome

Abstract
The use of shunting for hydrocephalus has a long history of improvements made through basic science, as well as clinical innovations and biomedical products. Shunting has dramatically changed the outlook of children with hydrocephalus, with many of them having normal life expectancies and attaining normal intelligence. The use of shunts, however, has created many unique problems of shunt dependence with frequent shunt revisions being the rule for most hydrocephalic children. Shunt problems assume a major amount of all neurosurgeon’s efforts. In addition, the slit ventricle syndrome has posed many new questions regarding what the post-shunting ventricular size should be, and what problems are created by having a slit-like ventricular system. This article reviews current concepts of childhood hydrocephalus with respect to the various hydrocephalic syndromes, and provides detailed descriptions of problems unique to various shunting systems. In addition, the slit ventricle syndrome is distinguished from asymptomatic post shunted slit-ventricles. The various forms of the slit-ventricle syndrome are described , and proposed guidelines for classifying, working up, and treating the slit ventricle syndromes are provided.

What is Hydrocephalus?
Hydrocephalus is commonly known as 'water on the brain'. A watery fluid, known as cerebro-spinal fluid or CSF, is produced constantly inside each of the four spaces or ventricles inside the brain. The CSF normally flows through narrow pathways from one ventricle to the next, then out over the outside of the brain and down the spinal cord. The CSF is absorbed into the bloodstream and recirculates, and the amount and pressure are normally kept within a fairly narrow range. If the drainage pathways are blocked at any point, the fluid accumulates in the ventricles inside the brain, causing them to swell - resulting in compression of surrounding tissue. In babies and infants, the head will enlarge. In older children and adults, the head size cannot increase as the bones which form the skull are completely joined together.
What causes Hydrocephalus?
The condition is caused by the inability of CSF to drain into the bloodstream. There are many reasons why this can happen:

This means that hydrocephalus is present at birth. It is important to remember that this term does not imply that it is hereditary. Often the exact cause of congenital hydrocephalus cannot be determined.

Congenital hydrocephalus... can be traced to a birth defect or brain malformation
In 1983, at the age of 13, I acquired hydrocephalus when I was hit in the head by a baseball. It was a freak accident.
The term "communicating hydrocephalus" means that the site of increased resistance to CSF drainage resides outside of the ventricular system in the subarachnoid space.
Non-communicating, or obstructive, hydrocephalus is caused when there is an obstruction in the flow of CSF within the ventricular system of the brain.
Hydrocephalus is often associated with Dandy-Walker syndrome, neural tube defects, and spinal bifida.
CSF is constantly being produced, flowing through, bathing, protecting, and cleansing the structures of your brain....
To communicate effectively with neurosurgeons, patients who have hydrocephalus need to become familiar with the basic structures of the brain. The effect that hydrocephalus has on an area of the body greatly depends on where the blockage or abnormal accumulation of CSF is located. Hydrocephalus occurs when there is an obstruction of one of the ventricles or ventricular foramina (CSF passageways) that restricts the flow of CSF

Hydrocephalus The term hydrocephalus is derived from two words (HYDRO = water, CEPHALUS = head). Hydrocephalus is a condition in which too much cerebrospinal fluid accumulates within the ventricles of the brain and may increase pressure within the cranium. The condition may occur at any age. The human brain is encased within the skull of the cranium which provides it with significant protection. Inside the cranium, the brain floats in a sea of liquid, which is called cerebrospinal fluid (CSF). This fluid is constantly being produced in the brain and circulated around the brain and spinal cord in a regular pattern. The CSF is subsequently absorbed by mushroom-like structures over the brain called arachnoid villae, and is then returned to the bloodstream. The amount of CSF within the brain remains fairly constant; consequently, the water pressure which contributes to the overall pressure inside the skull - known as the intracranial pressure (ICP) - also remains fairly constant. Hydrocephalus occurs when CSF circulation is altered in some way. For instance, the flow of CSF may be obstructed, CSF may not be adequately reabsorbed into the bloodstream, or less often, too much CSF is produced. Obstruction of CSF flow can be the result of a birth defect, brain tumor, infection, hemorrhage, or brain injury. In some infants, particularly those born with

Causes, incidence, and risk factors Return to top Cerebrospinal fluid (CSF) is formed in a region of the brain known as the choroid plexus. CSF usually circulates through channels of the brain known as ventricles, as well as flowing around the outside of the brain and through the spinal canal.
When the circulation or absorption of this fluid is blocked, or excessive fluid is produced, the volume of fluid in the brain becomes higher than normal. The accumulation of fluid puts pressure on the brain, forcing it against the skull and damaging or destroying the tissues.
Symptoms vary depending on the cause of the obstruction to CSF circulation, the age at which the problem develops, and the extent of damage to brain tissue caused by the swelling.
