Kidney biopsies typically show deposition of IgA in the mesangium of the glomerulus and the proliferation of mesangial cells and increased amounts of mesangial matrix. There are often increases in the amount of circulating IgA in the plasma of patients. The cause of the disease is not fully understood. Several groups now suggest that the molecule CD89 may play a role. This is a receptor for the Fc portion of IgA antibodies. The Fc portion of the molecule is the part that is not normally involved in binding to the foreign antigen. The receptors are involved in binding IgA and transporting it across cells. CD89 is also termed an FcalphaR. A group from the famous Necker hospital in Paris now report in the Journal of Experimental Medicine, that in patients with IgA nephropathy there are high levels of soluble CD89 bound to IgA which circulate in the blood as a complex. They then put the gene for human CD89 into mice and showed that these transgenic mice developed a disease like IgA nephropathy. When serum from these affected mice was administered to unaffected mice, the serum transmitted the disease. Furthermore, if the CD89-IgA complexes were removed from this serum it was unable to transmit the disease. These finding suggest that the presence of high plasma levels of the complex formed between CD89 and IgA can cause a disease in mice similar to human IgA nephropathy. As such high levels are found in patients, but not in normal controls, these complexes may contribute to the pathology of human IgA nephropathy.

A clinical trial currently recruiting participants. What is IgA Nephropathy? IgA nephropathy is a kidney disorder caused by deposits of the protein immunoglobulin A (IgA) inside the glomeruli (filters) within the kidney. These glomeruli normally filter waste and excess water from the blood and send them to the bladder as urine. The IgA protein prevents this filtering process, leading to blood and protein in the urine and swelling in the hands and feet. This chronic kidney disease may progress over a period of 10 to 20 years. Why is this research study being done? The purpose of this research study is is to evaluate the safety and benefits of a drug called mycophenolate mofetil (MMF) in patients with IgA Nephropathy. The study has been designed to compare every-day treatment with MMF tablets versus placebo tablets (pills which contain no active medicine) in the treatment of IgA Nephropathy.

IgA NEPHROPATHY
IgA nephropathy is the commonest primary glomerular disease. It is seen in about 10 percent of all renal biopsies performed for primary glomerular disease in North America, 20% in Europe, and as high as 30 to 40% in the Asian Pacific area. This may reflect regional differences in the indications for renal biopsy rather than a true difference in geographic incidence. Genetic influence as well as environmental factors may also contribute. It is most common in the second and third decade, However all ages may be affected. Males are more commonly affected (2:1). Can be viewed as a monosymptomatic form of Henoch Schonlein purpura
CLINICAL PICTURE:
Microscopic or macroscopic hematuria is the commonest manifestation. May follow quickly upon some nonspecific virus-like syndrome or intercurrent infection (e.g. synpharyngitic nephritis) It may be precipitated by upper respiratory tract infection. Proteinuria (usually less than 1 gm per 24 hour). Blood pressure is usually normal.

Aged 27 and never having experienced a 'major life event' other than being born, when I learnt two years ago that my kidneys had failed, I was not sure to what pitch of melodrama I should sink. I discovered that I had kidney failure when my eyesight began to fade. I thought that my eyes were infected. I went to the ophthalmology department at the regional hospital. As a matter of course, the doctor took my blood pressure. It was abnormally high. I was wheeled to casualty, where they took blood samples and hooked me up to all sorts of fear-inducing machinery that puffed, beeped and sucked. A young doctor came and sat down on my bed. "Now. It looks quite serious." Gulp. "You have high levels of urea and creatinine (the end product of muscle metabolism) in your blood. Your blood pressure is dangerously high. All the indications point to the possibility that you have kidney failure." I grudgingly let them keep me in overnight. My blood pressure was brought under control and the intricacies of a renal diet were explained to me. I had never given any special thought to my kidneys. But during my first two-and-a-half week stay in hospital, I soon learnt that they have many more functions than just purifying blood. Kidneys are fist-sized organs situated in the small of the back, level with the bottom rib. They keep blood pressure stable, balance the body-fluid volume and filter waste products. They also produce urine to rid the body of waste substances; and hormones to do such things as manufacture red blood cells, so that oxygen can be transported around the body, and convert vitamin D to strengthen the bones. Far from being almost an evolutionary anachronism, as I had vainly hoped, kidneys are vital organs.

What's it really like - patient experiences of IgA Nephropathy IgA is short for Immunoglobulin A, one of the types of antibody our body produces to fight infection. This circulates in the blood. "Nephropathy" is a scientific term for kidney disease.
What is IgA Nephropathy?
Scarring and inflammation within the kidney, that can only be seen clearly under the microscope. Therefore it is normally only diagnosed after a biopsy test of the kidney. What is seen under the microscope is that the "glomeruli", which are the tiny structures which filter the blood to make urine, are damaged by deposits of IgA.
What are the symptoms of IgA Nephropathy?
These are variable from case to case. In many cases there are no symptoms, but the damage to the glomeruli causes some blood to appear in the urine. This blood is often invisible, and only detected on routine medical check-ups. In other cases the blood may be visible, coming in attacks every so often. Sometimes an influenza type illness may spark off an attack of blood in the urine, which then clears after a few days.

William Check, PhD Advances in understanding IgA nephropathy , like the disease itself, come in two classes: the slowly progressive type, which is more common, and the less common aggressive form. Consequently, recent reports on this disease contain many preliminary or "promising" findings but fewer
firm conclusions. Summarizing a clinical conference on IgA nephropathy that he co-chaired at the 2000 annual meeting of the American Society of Nephrology, J. Charles Jennette, MD, says, "The conference emphasized that there are still a lot of unresolved issues in clinical management, etiology and pathogenesis, and pathological evaluation of IgA nephropathy." Pathogenesis is "one of the most exciting issues," says Dr. Jennette, Brinkhous Distinguished Professor and chair of pathology and laboratory medicine at the University of North Carolina at Chapel Hill. "A potentially very important observation is that there may be abnormalities in the molecular structure of circulating IgA in patients with IgA nephropathy and the closely related disease Henoch-Schönlein purpura, particularly abnormalities in galactosylation in the hinge region of IgA molecules,"
he adds.

What is IgA nephropathy?
IgA nephropathy is a chronic kidney disease that may progress over a period of 10 to 20 years, and can lead to end-stage renal disease. It is caused by deposits of the protein immunoglobulin A (IgA) inside the glomeruli (filters) within the kidney. These glomeruli normally filter waste and excess water from the blood and send them to the bladder as urine. However, the IgA protein prevents this filtering process. This may lead to the following:
  • blood and protein in the urine swelling in the hands and feet
What causes IgA nephropathy?
The IgA protein is a normal part of the body's immune system. It is unknown what causes IgA deposits in the glomeruli, however, in more than half of families, IgA nephropathy is inherited by an autosomal dominant gene. Autosomal dominant inheritance means that the gene is located on one of the autosomes (chromosome pairs 1 through 22). This means that males and females are equally affected, and “dominant” means that only one gene is necessary to have the trait. When a parent has a dominant trait, there is a 50 percent chance that any child they have will also inherit the trait.

IgA NEPHROPATHY ACCOMPANIED BY SILICA-DEPOSITED INTERSTITIAL NEPHRITIS Abdur Rauf Khan, MD, FCAP; Mohammed Al-Homrany, FRCPC, FACP Occupational exposure to silica is associated with clinical renal disease. The nephrotoxicity due to silica is believed to be either due to putative immune complex deposition in the glomeruli or to direct deposition of crystalline material in the interstitium. We report a case in which both of these causative factors were operating at the same time. The patient had glomerular injury due to IgA deposits, accompanied by interstitial damage due to silica deposition. Case Report A 45-year-old Filipino male was admitted to Asir Central Hospital for bilateral leg swelling of three months' duration. He was a tunnel construction worker and was known to have had silicosis of the lungs for several years. The patient reported no history of hematuria, sore throat, skin rash, arthralgia, hypertension or diabetes. There was no history of renal disease in his family. On physical examination, he looked well, the heart rate was 80/min. and blood pressure was 160/100 mm Hg. There was a pitting edema of the leg (++). The conjunctivae were congested but there was no evidence of jaundice, cyanosis or fingernail clubbing. On auscultation of the chest, diffuse bilateral crepitations were heard, mainly in the lower zones. The first heart sound (S ) was normal but the second (S ) was loud. Abdominal and neurological examinations were within normal limits.
