IgA Nephropathy What is it? IgA nephropathy, also known as IgA Disease or Berger’s Disease after the doctor who first recognised it, is the commonest of a group of conditions, which have the general name glomerulonephritis. Glomerulonephritis is an inflammation of the kidneys, in particular the glomeruli, the microscopic filters which allow the kidney to clear waste products and excess water from the blood. IgA is an antibody. Antibodies are a part of the body’s natural defences against infection. Everyone has IgA. It is usually found in our blood, the inner lining of the intestine and the airways of the lungs. If you have IgA nephropathy, a very small amount of the IgA has stuck in your kidneys causing inflammation and is stopping the kidneys working properly. The rest of your IgA will be working quite normally, defending you against infection. It is not yet known why the IgA sticks in the kidney. What are the symptoms of IgA nephropathy? The commonest symptom of IgA nephropathy is episodes of blood in the urine (haematuria . This often coincides with a sore throat or respiratory infection. Sometimes the episodes of blood appear to be caused by a bowel infection or even exercise. The urine may look like anything from slightly smoky, through tea without milk (most common), to bright red. It is not usual to see blood clots. It is usually painless, although may be accompanied by kidney discomfort.

Medline Citation Abstract The evaluation of familial glomerulonephritis in patients with IgA nephropathy who were from central and eastern Kentucky resulted in the discovery of potentially related pedigrees containing 14 patients. An additional 17 members of the pedigrees had clinical glomerulonephritis, and 6 had "chronic nephritis" noted on their death certificates. Six patients with IgA nephropathy had a common ancestor. In addition, both parents of six patients with the disease came from families with other cases of IgA nephropathy. No single HLA haplotype or antigen was found in all the patients with IgA nephropathy. Our data on these pedigrees strongly support an inherited mechanism in the pathogenesis of IgA nephropathy in some patients.

What is IgA nephropathy?
IgA nephropathy is a chronic kidney disease that may progress over a period of 10 to 20 years, and can lead to end-stage renal disease. It is caused by deposits of the protein immunoglobulin A (IgA) inside the glomeruli (filters) within the kidney. These glomeruli normally filter waste and excess water from the blood and send them to the bladder as urine. However, the IgA protein prevents this filtering process. This may lead to the following:
  • blood and protein in the urine swelling in the hands and feet
What are the symptoms of IgA nephropathy?
IgA nephropathy is a silent disease that may go undetected for years, but usually onsets before the age of 40. The most common symptom is blood in the urine. It takes many years to progress to the stage where it causes detectable complications such as swelling, recurrent upper respiratory infections, or intestinal disease. How is IgA nephropathy diagnosed?

[Methods] Total IgA1 in serum from IgA nephropathy patients and healthy controls was isolated as jacalin-bound protein (JBP) by jacalin affinity chromatography with jacalin-agarose column. Monomeric IgA1 and polymeric IgA1 were separated by size exclusion chromatography. [Results] [Conclusion] Both forms of IgA1 are elevated in patients of IgA nephropathy and they may be important in the pathogenesis of IgA nephropathy. It is characterized by constant presence of mesangial deposition of immunoglobulin A (IgA), and often associated C3 and IgG and/or IgM, in a granular pattern in the immunofluorescence studies of renal biopsy specimens. It has been confirmed that IgAN is an immune-complex mediated disease . However, the immunopathogenesis is not completely understood and some data are contradictory.

Donna J. Lager, M.D. Peer Review Status: Internally reviewed IgA nephropathy, first descibed in 1968, is the most common form of primary glomerulonephritis in the world. It is an antibody-mediated glomerular disease in which the immune deposits localize to the mesangium. It is not certain whether the deposits form in situ or from circulating immune complexes. Patients with IgA nephropathy usually present with one of three syndromes:
  • Macroscopic hematuria concurrent with an upper respiratory infection; so-called synpharyngitic hematuria.
  • Asymptomatic microscopic hematuria and variable proteinuria.
  • Henoch-Schonlein purpura is the systemic form of the disease process causing IgA nephropathy, and occurs more frequently in children than adults. Patients with Henoch-Schonlein purpura manifest skin, joint and intestinal involvement.
A less common presentation is with the nephrotic syndrome. These patients may have advanced disease or normal renal function. In the latter case, the light microscopic features are of minimal change disease but with intense mesangial staining for IgA. A. Morphologic Features
