Overview
Urinary incontinence (UI) is a common but poorly understood problem in the elderly population. It is estimated that at least 15% of community-dwelling elderly individuals and 50% of institutionalized elderly persons have significant urinary incontinence. As the degree of functional dependence increases, the prevalence of incontinence increases as well. The prevalence of incontinence may be significantly underestimated, since physicians rarely ask patients about the problem, and patients seldom initiate discussions about incontinence with their physician. Older patients may assume that UI is a normal consequence of aging. Some patients may be embarrassed by their incontinence or fear invasive testing, and thus avoid evaluation. UI commonly results in medical, social, and economic consequences. Incontinence is a common reason for institutionalization in the elderly. The care of incontinent nursing home residents is significantly more expensive than continent residents, and requires more nursing time and frequent linen and clothing changes. UI can result in skin irritation and contribute to skin breakdown or pressure sores. The social consequences include guilt and isolation, with some elderly afraid to leave their homes. Absorbent undergarments may reduce this fear but are more expensive and can delay appropriate evaluation.

Urinary incontinence can result from anatomic, physiologic, or pathologic (disease) factors. A patient information handout on urinary incontinence , written by Elizabeth S. Smoots, M.D., Seattle, Wash., is provided on page 2688. In most cases, the evaluation of urinary incontinence requires only a history, a physical examination, urinalysis and measurement of postvoid residual urine volume. The initial purposes of the evaluation are to identify conditions requiring referral or specialized work-up and to detect and treat reversible causes that may be present. If the patient does not appear to require referral and a reversible cause is not identified, the next step is to categorize the patient's symptoms as typical of either urge or stress incontinence and treat the patient accordingly. If treatment fails or a presumptive diagnosis of urge or stress incontinence cannot be reached, the final step would be to perform more sophisticated tests or refer the patient for testing to define the cause and determine the best treatment. U rinary incontinence, the unintentional leakage of urine at inappropriate times, is a classic syndrome in geriatric medicine. It affects approximately 13 million persons in the United States, most of whom are older adults.

Urinary incontinence What is urinary incontinence?
It is the involuntary leakage of urine which causes a hygiene or social problem. How common is the problem?
It is estimated to affect 5-15% of the elderly persons living in Singapore. Can urinary incontinence in the elderly person be considered as part of the normal ageing process?
Even though there are some changes in the bladder and the urinary tract which are associated with ageing and which make the elderly person more prone to urinary incontinence, the problem is not and should not be considered as part of ageing. So what are the causes of urinary incontinence?
The causes can be generally divided into 2 groups: Transient causes: these are causes which are usually reversible if treated.
They can be represented by the mnemonic "Diapers" in which the "p" and "e" each represent 2 groups of causes.

Female urinary incontinence (UI) is a condition with severe economic and psychosocial impact . I have studied the epidemiology of UI in the general female population and the prevailing management of UI by Norwegian general practitioners (GPs). Those who are interested in more details, may take a look at this synopsis . The main conclusions that may be drawn from these studies are that UI is very prevalent among adult women, mostly in the form of stress incontinence. At least 6% may be regarded as potential patients. Although complicated by methodological difficulties, studies of GPs' management indicate that available therapeutic tools (described below) are not used to their full potential. A literature review demonstrates that the history is the most important diagnostic tool in general practice, and that the value of clinical investigations is poorly documented.
Pelvic floor exercises
In 1948 Arnold Kegel described pelvic floor exercises as a treatment option in stress incontinence.(1) The purpose of the exercises is to increase the muscle volume and to develop stronger reflex contractions following quick rise in intra- abdominal pressure.(2)

All appeared to go well and after a fortnight of lead only exercise we were back to normal. Then a few weeks later I realised to my horror she was driping urine and totally unaware of it. It soon became not so much a drip as a flood, I do not know who was more distressed me or my normally clean dog. When it happened to my dog I could find very little useful information, visits to the vet were both costly and fustrating as I could see no end in sight. I am not a vet, just a pet owner but I ended up spending a lot of time researching causes and treatments to try and help my dog. I have attemped to gather here much of the information I have gained in the hope it will help those of you who find yourselves in the hard position of owning a much loved but leaky bitch. Although I touch on other causes this page is primarily about spay incontinence, this is one of the most common forms and is often controllable.

Stress incontinence doesn’t actually have anything to do with emotional stress. The term refers to urine leakage during physical activity that results in increased pressure on the abdomen. This pressure need not be much: bending over, laughing and coughing can cause urine leakage in stress incontinence sufferers. Many people suffer silently, too embarrassed to seek help. There’s good news, however: treatment for urine leakage is readily available.
Male/Female Urology and Stress Incontinence
Female urology is the branch of medicine dealing with the urinary tract and the urogenital system in women. According to urologists, stress incontinence can strike either sex, although two thirds of all sufferers are women.
In either sex, pelvic muscles, which normally support the bladder, can become weakened due to injury or aging. This allows the bladder to “drop” and push down towards the urethra. Because of this, the muscles that usually tighten and help the urethral sphincter to control urine flow cannot operate as effectively as they should.
Risk Factors
Pregnancy often causes temporary bladder problems. In many women, pregnancy and childbirth can result in weakened pelvic muscles, and problems with incontinence can result. In these circumstances, according to female urology studies, stress incontinence is usually temporary, and goes away as the muscles and urinary system regains tone following pregnancy. In addition to childbirth, many types of pelvic surgery can also weaken pelvic muscles.
