4.1 What is the connection between insulin resistance and PCOS?
At least 30 percent of women with PCOS are insulin resistant, although some investigators claim a much stronger association exists. Hyperinsulinemia produces hyperandrogenism by stimulating ovarian androgen production and by reducing serum sex-hormone binding globulin (SHBG). This can heighten PCOS symptoms. Reducing insulin resistance through the use of insulin-sensitizing medications can restore ovulatory function in many women with PCOS. It appears that even some patients who do not test as being insulin resistant may benefit from these medications. TOP

Since most people are able to conceive within this time, physicians recommend that couples unable to do so be assessed for fertility problems. In men, hormone disorders, illness, reproductive anatomy trauma and obstruction, and sexual dysfunction can temporarily or permanently affect sperm and prevent conception. Some disorders become more difficult to treat the longer they persist without treatment. Sperm development (spermatogenesis) takes place in the seminiferous tubules (ducts) of the testes. Cell division produces spermatozoa (mature sperm cells) that contain one-half of a man's genetic code. Each spermatogenesis cycle consists of six stages and takes about 16 days to complete. Approximately five cycles, or 2 ½ months, are needed to produce one mature sperm. Mitochondria (energy-generating organelles) inside each sperm power its tail (flagellum) so that it can swim to the female egg once inside the vagina. Sperm development is ultimately controlled by the endocrine (hormonal) system that comprises the

Reproductive Aged Population Affected: 9% About 5.3 million people in the U.S. suffer from infertility, affecting men and women equally. The average couple attempting conception has about a 20% chance of conceiving per menstrual cycle. This means that it is unlikely that a pregnancy will result after the first or even second month of trying. However, 95% of couples do conceive within 13 months. The minority who do not become pregnant after a year's time still have a good chance only about a quarter of these will never conceive. A tiny pipette is used to inject a single sperm into the awaiting egg in a revolutionary new procedure, Intracytoplasmic Sperm Injection. In any field of medical research, success is often measured in minute degrees, the impact of which may only be known to those whose careers are spent in the laboratories. Occasionally, however, there comes a discovery so astounding that it revolutionizes medical procedure. Such is the case with Intracytoplasmic Sperm Injection, or ICSI, a new infertility treatment utilizing micromanipulation technology that specifically addresses male factor infertility issues. ICSI is so remarkable, in fact, that most treatments previously used have been abandoned in its favor. MALE FACTOR INFERTILITY - A DEFINITION Male factor infertility can include any of the following problems: low sperm counts, poor motility or movement of the sperm, poor sperm quality, or sperm that lack the ability to penetrate an egg.

The 1999 report of pregnancy success rates is the fifth report to be issued under the Fertility Clinic Success Rate and Certification Act. The report includes a national overview that uses information from 370 U.S. fertility clinics to provide an in-depth picture of the type, number, and outcome of ART cycles performed in U.S. clinics; individual clinic tables that provide ART success rates and other information from each clinic that submitted and verified its 1999 data; and appendixes containing technical notes, a glossary of terms, and contact information for reporting and nonreporting clinics in the United States.

Live-Birth Rates and Multiple-Birth Risk Using In Vitro Fertilization
To maximize birth rates, physicians who perform in vitro fertilization (IVF) often transfer multiple embryos, but this increases the multiple-birth risk. Live-birth and multiple-birth rates may vary by patient age and embryo quality. One marker for embryo quality is cryopreservation of extra embryos (if embryos are set aside for cryopreservation, higher quality embryos may have been available for transfer). The study found that, the risk of multiple births from IVF varies by maternal age and number of embryos transferred. Embryo quality was not related to multiple birth risk but was associated with increased live-birth rates when fewer embryos were transferred. Source:
