P atients with irritative voiding symptoms and negative urine cultures frequently present a diagnostic challenge for primary care physicians. Interstitial cystitis is a severely debilitating disease of the urinary bladder. Symptoms of interstitial cystitis include excessive urgency and frequency of urination, suprapubic pain, dyspareunia and chronic pelvic pain. Interstitial cystitis interferes with employment, social relationships and sexual activity. The course of the disease is usually marked by flare-ups and remissions. Interstitial cystitis is a chronic disease that persists throughout the patient's life. On clinical evaluation, patients with interstitial cystitis should have no other definable pathology, such as urinary infections, carcinoma, or radiation-induced or medication-induced cystitis (Parsons CL. Two recent reports offer hope for people with this previously hopeless incurable disease (1,2,3). If you have terrible discomfort when your bladder is full, and your doctor cannot find a vaginal, prostate or urinary tract infection, he usually refers you to a urologist who will examine the bladder directly. If he finds no cause, he usually diagnoses interstitial cystitis (4). The bladder is a balloon lined by muscles that contract when it is full. The severe pain of interstitial cystitis may be caused by contractions of bladder muscles. The body produces a chemical called nitric oxide that relaxes muscles and blocks spasms. For example, hemorrhoids are felt to be caused by lack of nitric oxide with resultant spasms of the anal sphincter that cause blood vessels at the end of the intestines to enlarge. Several studies in the last year show that chemicals that raise nitric oxide levels relax the anal sphincter muscles and get rid of hemorrhoids (5,6,7,8,9,10). The study in the Journal of Urology shows that people with interstitial cystitis have low tissue levels of nitric oxide also, and that L-arginine, a protein building block, raises tissue levels of nitric oxide and relaxes bladder muscle spasms to help control the pain of interstitial cystitis (1,16).

The general opinion is that the cause is multifactorial, and that we may be dealing with a collection of symptoms rather than a specific disease. IC can only be diagnosed with cystoscopic examination under a general anesthetic where we look for bleeding areas or bladder ulcerations. While it most commonly afflicts middle-aged women with a peak incidence between 30-40 years of age, the condition occurs in all age groups. Generally, the symptoms do not progress continuously, but rather reach their final stage rapidly and then stabilize at that level. Major deterioration of symptoms is unusual as there is no evidence that mild cases progress to more sever forms. Up to fifty percent of patients experience spontaneous remissions, having flare-ups in between. Relatively few patients suffer such unremitting pain and frequency that they cannot maintain a normal lifestyle. There may be almost half a million patients with IC in the United States. Numerous studies have failed to find an infectious cause for IC, such as bacteria, fungal or viral. The most popular etiology postulates a deficiency in one of the chemicals that lines the surface of the bladder. Biopsies from the bladder in patients with IC have revealed more inflammatory cells in the lining of the bladder than usual.
