Interstitial Cystitis - What is it? The Canadian Interstitial Cystitis Society has kindly granted permission for the reprinting of their brochure on our web site. Interstitial Cystitis (I.C.) is a bladder condition, characterized by chronic inflammation of the urinary bladder wall. Its cause is unknown, but early diagnosis may alleviate some of the long term problems. It is known, however, that I.C. is not a psychosomatic disorder nor is it caused by stress. Although stress is not a cause it can increase symptoms. "Cystitis" refers to a condition of the bladder. Common cystitis is normally treated with antibiotics. Interstitial cystitis is an inflammation of the bladder wall and has a variety of possible symptomatic treatments. Both men and women may be affected, although 85% of patients are women. WHAT IS INTERSTITIAL CYSTITIS? Many cat owners are familiar with the signs of feline lower urinary disease in the cat. Whether we call the syndrome FUS for feline urologic syndrome or use the new name of FLUTD for feline lower urinary tract disease, it amounts to bladder pain, straining, and bloody urine. Interstitial cystitis (“cystitis” simply means inflammed bladder) is human disease that is also characterized by excruciating bladder pain and straining. In 1990, Dr. M. Clasper made the suggestion that these two conditions might be closely related such that humans could benefit from studies involving affected cats and affected cats could benefit from studies of affected humans. YOU KNOW A PERSON HAS INTERSTITIAL CYSTITIS WHEN: There are several diagnostic and historical features that “clinch” this diagnosis in a human:
  • The person must experience urinary pain and/or urgency. The problem usually has been going on for more than 9 months and the person must have a urination frequency of at least eight times a day.
    The person must have less than 350cc (about a 1 and ½ cups) of bladder capacity while awake.

This is a bladder condition that produces the same symptoms as a bladder infection but with no obvious cause. Symptoms include the need to urinate frequently (sometimes up to 50 times a day), a feeling of urgent need to urinate and urinary discomfort including pain, burning or cramping. Pain may also accompany sexual intercourse. Unlike bacterial cystitis, which is an actual infection of the bladder, there is no obvious cause for interstitial cystitis.
Interstitial cystitis (I.C.) affects mostly women (9 women affected for every 1 man) and generally occurs after the age of 40. I.C. is diagnosed after ruling out other causes such as bladder infection, sexually-transmitted disease, bladder cancer, vaginal infection, endometriosis or other causes. On cystoscopy (insertion of a tiny picture tube into the bladder), there is pinpoint bleeding (hemorrhage), in the lining of the bladder. This can be confirmed by a biopsy of the bladder.
Bladder 'retraining' (by increasing time between each urination) and avoiding chocolate. alcohol and spicy foods may be helpful. Call for an appointment with your doctor if you have symptoms suggestive of interstitial cystitis. Make sure you mention that you suspect this disorder; it is not well recognized nor is it easily diagnosed. Many doctors still mistakenly consider interstitial cystitis a psychological problem.

Interstitial Cystitis Interstitial cystitis (IC) is a chronic (ongoing) condition that causes recurring discomfort or pain in the bladder and the surrounding pelvic area. The pain is caused by inflammation or irritation of the bladder wall, which can lead to scarring and stiffening of the bladder; decreased bladder capacity; and pinpoint bleeding (known as glomerulations) on the bladder wall. Although its cause is unknown, researchers believe that IC is a medical condition that is not a result, symptom, or sign of an emotional problem, or caused by stress. There is no evidence suggesting that IC increases the risk for bladder cancer. IC is under-diagnosed because there is no definitive test for it, and its symptoms are similar to those of other urinary disorders. Therefore, no one knows for certain how many people have this condition. Data from surveys suggest that more than 1 million people in the United States have IC. Of the patients who are diagnosed with IC, women account for about 90%. Twenty-five percent of those affected are under the age of 30, and the average age of onset is 40. Symptoms

Interstitial cystitis is a condition in which the walls of the bladder are irritated and inflamed. Causes, Incidence, and Risk Factors
Interstitial cystitis (IC) is an inflammation (irritation with increased presence of immune cells) of the tissues of the bladder wall, with no known infectious (bacterial, viral, or fungal) cause. The condition is identified through a diagnosis of exclusion. There are about 10-30 cases per 100,000 people. Women are 10 times more likely to be affected than men. The condition is more common in Jewish women and is rare in African-Americans. IC generally occurs during the third or fourth decades of life. For the average person with interstitial cystitis, there is a 4-year delay between the time the first symptoms occur and the diagnosis. This demonstrates both the difficulty of diagnosis and how infrequently this condition is suspected.

I had a hydro/cysto three weeks ago and I have been miserable with severe burning pain and discomfort which has decreased my activities greatly. With Les Kaplan, MD WHAT IS INTERSTITIAL CYSTITIS? Interstitial cystitis has been known by many other names over the years, including chronic cystitis, painful bladder syndrome or urgency-frequency syndrome. A condition often referred to as urethral syndrome, originally thought to be a non-infectious irritation of just the urethra, is probably a mild form of IC. Some women who assume they are having recurrent urinary tract infections, or think they are getting bladder infections after intercourse, may, in fact, have early symptoms of IC. While their urine cultures usually do not grow bacteria, which would indicate infection, these women are often treated with antibiotics all the same. Some women have had persistent symptoms of bladder pain or urinary frequency for months after proper treatment for a true bladder infection. These women may also have IC. The onset of IC is usually gradual, but the symptoms often worsen over time. WHAT CAUSES INTERSTITIAL CYSTITIS?

Interstitial Cystitis
Interstitial cystitis, or I.C., is one of the most disabling non-malignant disorders of the adult urinary tract. IC is most frequently seen in women, though men can also be affected. Patients with IC suffer from a spectrum of intense bladder symptoms including severe bladder and lower abdominal pain, intolerable and disruptive frequency of urination - often voiding as frequently as every 15-20 minutes. Additionally, the patients will have pain during sexual intercourse. The exact cause of Interstitial Cystitis remains unknown, however, there are numerous theories as to its origins. Some researchers believe that it is a type of autoimmune disease while others feel it is an unusual infectious disease - either bacterial or viral. Physicians and urologists have also speculated on inflammatory, neuropathic and/or dietary causes - to date none has been confirmed.

Interstitial Cystitis: A Bladder Disorder
The urinary system consists of the kidneys, ureters, bladder, and urethra. The kidneys, a pair of purplish-brown organs, are located below the ribs toward the middle of the back. The kidneys remove liquid waste from the blood in the form of urine, keep a stable balance of salts and other substances in the blood, and produce erythropoietin, a hormone that aids the formation of red blood cells. Narrow tubes called ureters carry urine from the kidneys to the bladder, a triangle-shaped chamber in the lower abdomen. Like a balloon, the bladder's elastic walls relax and expand to store urine and contract and flatten when urine is emptied through the urethra. The typical adult bladder can store about 1 1/2 cups of urine. Adults pass about a quart and a half of urine each day. The amount of urine varies, depending on the fluids and foods a person consumes. The volume formed at night is about half that formed in the daytime. Normal urine is sterile. It contains fluids, salts and waste products, but it is free of bacteria, viruses and fungi. The tissues of the bladder are isolated from urine and toxic substances by a coating that discourages bacteria from attaching and growing on the bladder wall.
