Juvenile Rheumatoid Arthritis

Introduction
Chronic arthritis in childhood is not a single entity but a heterogeneous group of disorders. The term juvenile chronic arthritis (JCA) had been common in Europe while Juvenile Rheumatoid Arthritis (JRA) was used in North America. In 1997, these terms were discarded for a broader, universal term, Juvenile Idiopathic Arthritis (JIA). JIA defines arthritis beginning at less than 16 years of age and persisting for at least 6 weeks and with no known cause.
Rheumatoid factor positive polyarthritis (with RF testing positive on at lest two occasions three months apart) include approximately 10% of children with polyarticular JIA. The majority of affected children are adolescent girls who develop symmetrical polyarthritis affectingmany joints but usually including the small joints of their hands. This will frequently progress to an erosive arthropathy with a disease course similar to that of adult rheumatoid arthritis. This is essentially early onset 'adult-type' rheumatoid arthritis.
As with adults, treatment of Juvenile Arthritis is aimed at controlling inflammation and minimizing its effects on the joints. Localized joint problems can be minimized by good early control of the inflammatory process. Drug treatment is combined with physical therapy and the judicious use of splinting to maintain correct joint position and function.

What Is Arthritis?
Arthritis means joint inflammation, and refers to a group of diseases that cause pain, swelling, stiffness and loss of motion in the joints. "Arthritis" is often used as a more general term to refer to the more than 100 rheumatic diseases that may affect the joints but can also cause pain, swelling, and stiffness in other supporting structures of the body such as muscles, tendons, ligaments, and bones. Some rheumatic diseases can affect other parts of the body, including various internal organs. Children can develop almost all types of arthritis that affect adults, but the most common type of arthritis that affects children is juvenile rheumatoid arthritis.
What Is Juvenile Rheumatoid Arthritis?
Juvenile rheumatoid arthritis (JRA) is arthritis that causes joint inflammation and stiffness for more than 6 weeks in a child of 16 years of age or less. Inflammation causes redness, swelling, warmth, and soreness in the joints, although many children with JRA do not complain of joint pain. Any joint can be affected and inflammation may limit the mobility of affected joints.

New Drug Therapy for Juvenile Rheumatoid Arthritis
It's hard to believe that not long ago, a seven-year-old Wisconsin girl was in such constant pain that she used a tricycle to move between classrooms at school, she could not walk across the street or even use coloring books. Once ravaged by juvenile rheumatoid arthritis (JRA), the girl now rides a bicycle, walks to the park and back on her own, rough-houses with her older brother, and even runs and skips like a little dervish. It's all thanks to a new drug therapy she received under the care of James Nocton, MD , assistant professor of pediatrics at the Medical College of Wisconsin. Dr. Nocton is based at Children's Hospital of Wisconsin, an affiliate of MCW. The little girl was one of the first 50 juvenile rheumatoid arthritis patients in the country to try Enbrel, a genetically engineered human protein which seems to halt progress of the disease in some patients. She began taking Enbrel as part of a national clinical trial by the U.S. Food and Drug Administration and the drug's manufacturer, Immunex Corp. Children's Hospital is one of nine participating test sites.

Known cases of arthritis date as far back as 4500 BC with Native Americans in the Tennessee and Green Rivers region. Times have changed, yet arthritis persists. Today, approximately 300,000 children face the challenges of arthritis or rheumatic diseases with millions of other children at risk. The most common form of the disease among children is juvenile rheumatoid arthritis. Juvenile rheumatoid arthritis (JRA) affects the entire body. Characterized by inflammation of the membrane lining the joint, JRA invades the synovium, the joint lining, and damages bone and cartilage by releasing enzymes that digest the bone and cartilage. JRA may affect growth during active periods of the disease. Onset occurs between the ages of 2 to 5 years of age and 9 to 12 years of age. Girls are also at a higher risk than boys.
