WHAT IS JUVENILE ARTHRITIS? Juvenile arthritis is a chronic condition that causes inflammation in one or more joints and begins before the age of 16. There are several different patterns of juvenile arthritis. Though all have joint inflammation in common, they behave very differently, may require different treatment approaches and have different outcomes. Systemic onset type begins with very high fevers, frequently has a skin rash and shows evidence of inflammation in many internal organ systems as well as the joints. About 10 percent of children with arthritis have this type. Pauciarticular onset disease affects fewer than five joints. About half of all children with arthritis are in this category. Some of these are very young, from infancy to about age 5, and have a risk of developing inflammatory eye problems. Regular eye exams are essential. Others are older and may evolve into one of the adult forms of arthritis. Polyarticular disease has more than five joints affected (often many more), and can begin at any age. Some of these children have adult type rheumatoid arthritis that begins at an earlier age than usual.

University of Sydney, Australia. Knee replacement Pagets disease with OA Charcot forefoot Osteoarthritis (degenerative joint disease)- is the most common form of arthritis. Aetiology - On a cellular level, osteoarthritis may be a result of a failed attempt of chondrocytes to repair damaged cartilage as well as increased water content may result from childhood (SUFE see case 14 , Perthes see POT, CDH, Sepsis, necrosis) or in adulthood from abnormal bone (Pagets, AVN see case 13 ), abnormal cartilage (see POT), abnormal loads (malunion). Characteristics - On exam include decreased ROM and crepitus. The knee is the most common joint affected. Treatment begins with supportive measures (activity modification, cane, etc.) and includes NSAIDs and a variety of surgical procedures ranging from arthroscopy to arthroplasty. Neuropathic Arthropathy (Charcot Joint) An extreme form of osteoarthritis caused by a disturbance in the sensory innervation of a joint. Causes include diabetes (foot), tabes dorsalis (lower extremity)

    This material is presented mainly for parents of a child with arthritis. As parents, you carry much of the responsibility for the health care and emotional well-being of your child with arthritis. You will be the ones to:
    • make sure she sees all members of her health-care team as needed see that she takes her medications as prescribed watch for any side effects the drugs may cause help her do exercises to relieve pain and stiffness provide encouragement and support to her and other family members work with teachers and school officials to make sure her needs are met
    For this reason, it is very important for parents to know as much as possible about their child's arthritis and its care, as well as the effects it can have on family life. This information provides a starting point for discussions with your child's health-care team.

    Enbrel Shown Safe, Effective in Children and Teenagers with Juvenile Rheumatoid Arthritis
    Enbrel (etanercept) has been shown to be a safe and effective drug in the treatment of children and teenagers with polyarticular juvenile rheumatoid arthritis (JRA), according to clinical trial results reported in the New England Journal of Medicine In this clinical trial, 69 children, ages 4 to 17, were injected with Enbrel The trial was coordinated at the National Institute of Arthritis and Musculoskeletal and Skin Diseases (NIAMS) Multipurpose Arthritis and Musculoskeletal Diseases Center at the Children's Hospital Medical Center of Cincinnati, Ohio. It was performed by investigators in the Pediatric Rheumatology Collaborative Study Group.
