And, more than 43,000 die of a liver disease each year. T he liver is the largest internal organ in the human body. It is also one of the most important ones. The biliary system consisting of the bile ducts and the gallbladder and the pancreas are all closely associated with the functioning of the liver. S ome liver, biliary, and pancreatic diseases are congenital. Others can be prevented. Mild liver dysfunction sometimes occurs after major surgery and reflects poorly understood effects of anesthetic and operative stress. Patients with well-compensated liver disease (eg, inactive cirrhosis) usually tolerate surgery well. However, patients with underlying liver disease may develop more severe dysfunction postoperatively; eg, in a patient with viral or alcoholic hepatitis, laparotomy may precipitate acute liver failure. Postoperative jaundice in patients without previous liver disease can take several forms. The most frequent is multifactorial mixed hyperbilirubinemia caused by a complex interaction between enhanced bilirubin load and diminished hepatic clearance. This most often occurs after major surgery or trauma requiring multiple transfusions. Hemolysis, sepsis, resorption of hematomas, and blood transfusions can contribute to increased pigment production; simultaneously, hypoxemia, circulatory failure, and other poorly understood factors impair hepatic function. The result is variable, but severe jaundice with nondescript aminotransferase and alkaline phosphatase elevations often occurs. Frank liver failure is rare, and the syndrome typically resolves slowly but completely. ischemic hepatitis reflects hypoxic injury, not inflammatory necrosis, and characteristically resolves within a few days unless complicating factors are present.

Liver Diseases What Is It?
Diseases of the liver are the third most common cause of death in Americans during their most productive years and the seventh most frequent disease-related cause of death in the United States overall. Liver disease accounts for up to $8 billion yearly in economic losses. About 500,000 cholecyctectomies (gallstone removal operations) are performed each year due to gallstone disease, which is caused mainly by high cholesterol secretion into bile. Alcoholic liver disease is the most common cause of liver injury in the United States. Hepatitis B and Hepatitis C infect about 5 million persons. Hepatocellular carcinoma , or liver cancer, may be a consequence of these disorders and is a major interest of our physicians. With the onset of liver disease, the cells of the liver, called hepatocytes, are injured or die. If the injury is mild and reversible, the cells may regenerate and the patient may be left with an entirely normal liver. This remarkable capacity of the liver to regenerate is a unique feature of the organ. When the injury is more severe or sustained, regeneration may be incomplete or healing may occur with the development of fibrosis or scars, which can cause cirrhosis.

Alpha-1 Antitrypsin Deficiency
Alpha-1 antitrypsin deficiency is a hereditary disorder. The mechanism by which this deficiency leads to cirrhosis is unknown. In some varieties of the deficiency, alpha-1 antitrypsin is produced but trapped in the liver cells, where excess amounts of the protein can cause cirrhosis. In other organs like the lung, alpha-1 antitrypsin provides protection from damage by destructive enzymes. The disorder can lead to emphysema, even in non-smokers.
Hemochromatosis (Iron Overload)
Hemochromatosis, or iron overload, is a hereditary liver disease that causes the body to absorb too much iron. Iron overload damages internal organs and can cause cirrhosis. Left untreated, hemochromatosis can be fatal, but it can be successfully controlled if it is detected before organ damage occurs.
Wilson's Disease
Wilson's disease causes the body to store too much copper in the liver and brain. Wilson's disease is often confused for viral hepatitis, and it can be fatal if not diagnosed and treated. Treatment involves regular therapy to remove copper from the body.

LIVER DISORDERS The number and variety of functions performed by the liver—and its complexity—leave this organ susceptible to a number of disorders. Some experts find it surprising that liver disease is not more common in light of the liver's exposure to so many toxins and potentially dangerous microorganisms. But the liver is protected by its amazing capacity for self-regeneration and its division into semiindependent individual units called hepatic acini: The organ can undergo substantial injury and the uninjured units are still able to function. Common symptoms from which liver disease may be diagnosed include:
  • Jaundice: yellowish hue in the skin, whites of the eye, and other tissues. Jaundice may cause the urine to darken. Jaundice is caused by an excess of the bile pigments (bilirubin) produced by the liver during the breakdown of red blood cells. When these pigments don't follow their usual excretion route through the bile duct and intestines, they gather in the blood.
  • Liver enlargement: this condition is called hepatomegaly.
