Lymphedema is a swelling of some area of the body, usually an arm or leg, resulting from an accumulation of lymph fluid. Lymph, the transparent fluid that is collected from body tissues, is normally removed via the lymphatic system. However, in lymphedema the body’s ability to circulate lymph is compromised for some reason. Instead of the usual transport of proteins, fat, water, and cells from the lymph vessels into the circulatory system where they can be utilized or removed, the lymph fluid accumulates and causes swelling and pain in the limb. The lymphocytes, cells of the immune system which are vital in fighting bacteria, cannot circulate normally and the limb becomes more susceptible to infection. What causes Lymphedema? One type of lymphedema, called primary lymphedema, is a condition that a person may have from birth or may develop it in adolescence or even after age 35. Primary lymphedema appears to be due to some physical defect in the lymphatic vessels or nodes. Another type of lymphedema, one more commonly seen in physician’s offices, is called secondary lymphedema. Lymphedema of this type occurs after trauma to an area of the lymphatic system, most commonly from surgery which involves removal of or damage to the lymph nodes. It can also occur after radiation treatments of the lymph nodes. I always point out to 'newbies' that life isn't bad for most of us. Some unfortunate people end up in wheelchairs or bedridden but most people continue to be able to do most things-they just must adjust their lifestyle a bit. Lymphedema is a common complication of cancer and cancer treatment and can result in long-term physical, psychological, and social issues for patients. Overview The lymphatic system collects excess fluid and proteins from the body tissues and carries them back to the bloodstream. Proteins and substances too big to move through the walls of veins can be picked up by the lymphatic vessels because they have thinner walls. Swollen Legs (Lymphedema) Swelling of the legs may be caused by many diseases including heart failure, kidney disease, venous disease and disease of the lymphatics. This discussion will cover lymphedema which is an accumulation of fluid and protein that causes swelling in the arms and legs. This can occur when lymphatic vessels are impaired. Lymphedema can develop from idiopathic or unknown causes a situation known as primary lymphedema. This may start at birth (congenital lymphedema), in the teens (lymphedema praecox) or late in life (lymphedema tarda). Breast, abdominal or groin surgery may result in secondary lymphedema because of disruption of the lymph vessels or removal of lymph nodes from the surgical procedure. Radiation therapy, limb trauma and repeated or chronic infection may also affect the lymphatics. If a patient notices persistent swelling of a limb and the skin becomes hard and less elastic, the cause may be lymphedema. Patients who have lymphedema are at a high risk for infections. Depending on the severity of the lymphedema, a treatment plan will be developed based on decongestive therapy methods which consist of manual lymphatic drainage, bandaging, proper skin care, sequential gradient pumps and compression garments. Surgery is rarely used.
