    Retina Alternative names Return to top Age-related macular degeneration (ARMD or AMD); Senile macular degeneration (SMD) Definition Return to top Macular degeneration is a disorder that affects the macula (the central part of the retina of the eye) causing decreased visual acuity and possible loss of central vision. Causes, incidence, and risk factors Return to top The macula is the part of the retina that allows the eye to see fine details at the center of the field of vision. Degeneration results from a partial breakdown of the retinal pigment epithelium (RPE). The RPE is the insulating layer between the retina and the choroid (the layer of blood vessels behind the retina). The RPE acts as a selective filter to determine what nutrients reach the retina from the choroid. Many components of blood are harmful to the retina and are kept away from the retina by normal RPE. Breakdown of the RPE interferes with the metabolism of the retina, causing thinning of the retina (the "dry" phase of macular degeneration). These harmful elements may also promote new blood vessel formation and fluid leakage (the "wet" phase of macular degeneration).

This is my MD Story As it happens, Dr. M. was not kidding when he told me the reason I suddenly couldn't see was that I was suffering from Macular Degeneration , a disease which attacks some people slowly and insidiously, and others, like myself, with absolutely no warning. On Friday the 13th of November, 1998, as I was quietly typing, I suddenly could not see well enough to read. The reason? I had lost the central vision in my left eye. It had gone from 20/20 to 20/400+ quite literally "in the blink of an eye". This is a good illustration of what the written page looks like to me.

    SMOKING CAN LEAD TO BLINDNESS In two articles published in the October 9, 1996 issue of The Journal of American Medicine and in a report to the American Aging Association, researchers helped strengthen the link between cigarette smoking and Age-related Macular Degeneration (AMD). AMD is a condition in which the cell layer behind the retina begins to rot and rupture producing blurred vision and desensitivity to color. According to the researchers AMD is the main cause of new cases of blindness among people over 65. The researchers provided two hypothesis for the link between cigarette smoking and the degeneration. First, is that the cigarette smoking reduces levels of plasma antioxidant, a substance in the blood stream which protects the cells in the retina. This leaves the rather delicate cells of the eyes macula lutea (parts of the retina) left unprotected from oxygen molecules. The second hypothesis states that damage may occur when poor circulation (constricted blood vessels) due to smoking causes the protective layer between the retina and blood vessels to erode. This allows fluid to leak between membranes causing irritation and scarring. The fi rst study conducted by a team of researchers from the Massachusetts Eye and Ear Infirmary, Boston, followed 32,000 female nurses aged 50 and 59 for a twelve year period. Those who smoked more than 25 cigarettes a day had a two and half times greater risk of developing macular degeneration. The second study, conducted by doctors at the Brigham and Women's hospital and Harvard Medical School, focused on 21,000 male doctors over the same twelve year period and reported similar results.

    Researching the Cause of Macular Degeneration
    The causes of age-related macular degeneration (AMD) are still not understood, but researchers at the Medical College of Wisconsin continue to seek answers through research at the cellular level. Age-related macular degeneration is the leading cause of vision loss for individuals over age 60, affecting 13 million Americans. Macular degeneration affects light-sensitive photoreceptor cells in the macula, the center of the retina at the back of the eye. While it doesn't cause total blindness, the disease destroys central vision so reading, watching TV and driving are impossible. There is no cure, and treatments are very limited. Like most age-related diseases, AMD is likely to have many causes, including both genetic and "toll of years" components. A gene or several genes may predispose the retina to cellular changes resulting in macular degeneration. But whether or not a person with a genetic predisposition gets the disease also depends upon how the aging process itself affects cells.

    The Christmas of 1995 will be one which I shall never forget. With all the normal hustle and bustle of pre-Christmas organizing we sometimes ignore what's going on inside our own bodies. Then comes the stark reality that something is wrong. That is what happened to me and I hope it never happens to you. In these next few pages I shall tell you about an eye disease that I developed and how I came to grips with it. I spent the better part of a month scouring the Internet for information relating to AMD and CNV in hopes of understanding what my future held. I have included numerous links to other AMD and Ophthalmology pages and I'll show you some really gross pictures of my own eyes if you like. If you know what to watch out for, you might be able to receive treatment before its too late. Knowledge is a powerful drug that soothes the soul but only if taken in the right quantity and used with the right attitude.
